A Nationwide Survey of Dermatology Faculty and Mentors on Their Advice for the Dermatology Match Process. 一项关于皮肤科教师和导师对皮肤科匹配过程的建议的全国性调查。

While strong relationships with mentors and advisers are critical to navigating the dermatology match process, the advice around the match process that medical students receive from different individuals can be contradictory. In this study, we sought to examine the advice that mentors provide to medical students applying to dermatology residency programs via a 14-question anonymous survey covering topics such as research years, away rotations, dual applying, couples matching, program signaling, PGY-1 year, geographic signaling, interviewing, and volunteering in medical school.

Spironolactone for Acne: Practical Strategies for Optimal Clinical Outcomes. 痤疮的螺内酯:最佳临床结果的实用策略。

Spironolactone is an effective systemic treatment for acne in women and likely is an underutilized alternative to oral antibiotics. It is used off label for all types of acne and has shown effectiveness for both facial and truncal acne. We recommend a starting dose of 100 mg/d, as most patients can tolerate it and it has superior effectiveness to lower doses. The most common adverse effects of spironolactone are lightheadedness; headache; and menstrual irregularities, which tend to be dose dependent and can be prevented or treated using combined oral contraceptives. Potassium monitoring is of low usefulness in young healthy women, and studies have not identified an association between spironolactone use and increased risk for cancer. This article provides an overview of spironolactone for the treatment of acne, including recent clinical trials and practical strategies for patient selection, dosing, adverse effect management, and monitoring.

Infection and allergic contact dermatitis may be encountered in postoperative and chronic wounds. Since symptoms overlap, differentiating these complications in real time could help guide management. We propose applying potentially allergenic wound care agents to an area of healthy skin while managing postoperative and chronic wounds. This method can help identify allergic contact dermatitis rapidly, thereby reducing misdiagnosis and improving patient care.

Rosacea is a chronic inflammatory skin condition of the central face driven by immune dysregulation, genetic predisposition, epidermal barrier dysfunction, and microbial dysbiosis. Recent evidence implicates both the gut and skin microbiomes-through direct immune interactions and the gut-skin axis-in driving cutaneous inflammation and disease severity, with an increase in proinflammatory species such as Demodex folliculorum, Staphylococcus epidermidis, and Bacillus oleronius alongside a decline in protective species such as Cutibacterium acnes. Therapeutic strategies now aim to restore microbial balance using narrow-spectrum antibiotics, anthelmintics, topical and oral probiotics, and microbiome-friendly skin care to reduce inflammation, reinforce skin barrier function, and improve clinical outcomes. Future research to refine precision treatments that target specific microbial and immune pathways would be beneficial to modulate dysbiosis and improve outcomes in rosacea.
