Incidence of Morbidity Among Children and Adolescents in Kazakhstan 哈萨克斯坦儿童和青少年发病率

Purpose of the research: To study the dynamics of changes in the structure of morbidity among children in Kazakhstan for 2018-2020.Methods: The comparative analysis of the data of the annual form «Report on the number of diseases registered in patients living in the service area of a medical organization and patients' groups under the clinical supervision» for 2018-2020 has been conducted. Results. According to the results of 2020 data analysis in Kazakhstan, the number of registered diseases among children aged 0-14 and adolescents aged 15-17 decreased by 7% compared to 2018. Respiratory diseases in the 0-14 age group accounted for 51,8 per cent of all reported cases. Among adolescents aged 15-17, respiratory illnesses accounted for 33,81 per cent. Digestive diseases are second in the 0-14 age group, accounting for 9,2 per cent and 10,65 per cent for adolescents. Nervous system diseases in children from 0 to 14 years of age accounted for 6 per cent and for the treatment in third place. Among 15-17-year-olds, the third place is occupied by eye disease and its appendage apparatus, which accounted for 10,35 per cent.Conclusions. Analysis of the obtained data showed that the morbidity rate among children and adolescents for 2018-2020. has a tendency to decrease. The most common diseases among children are diseases of the respiratory system, diseases of the digestive system, diseases of the eye and appendage apparatus, diseases of the nervous system. At the same time, there has been an increase in the number of diseases of the nervous system

5-year analysis of the effectiveness of the use of the bed fund in the healthcare system of the Republic of Kazakhstan 对哈萨克斯坦共和国医疗保健系统床位基金使用效果的5年分析

The purpose of the study: to evaluate the effectiveness of the use of the bed fund (including oncological beds) in the healthcare system of the Republic of Kazakhstan for 2016-2020.Methods: balance method, mathematical and statistical methods, method of comparison.Results. In the Republic of Kazakhstan, there is a tendency to increase both the absolute number and the provision of beds per 10 thousand population. So during the analyzed period, the number of beds, for all departments, increased from 87.172 beds in 2016 to 107.595 beds in 2020 (18.9%). The provision of beds per 10 thousand population has increased in beds of all departments since 2016 from 48.6 to 57.4 per 10 thousand population in 2020, the increase was 15.3%. The provision of beds for all forms of ownership and departments in 2020 amounted to 55.4 beds per 10 thousand population. At the same time, the provision of beds per 10 thousand population in the system of the Ministry of Health of the Republic of Kazakhstan is 44.6, which corresponds to the average level of bed provision in the OECD countries (43.9). Over the period from 2016 to 2020, there has been an increase in the number of beds of private ownership, an increase of 41.31%. In Kazakhstan, for the period from 2016 to 2020, the average bed occupancy tends to decrease - 35.0%, from 311 days in 2016 to 202.3 in 2020, this indicator corresponds to the level of bed occupancy of 82.6% (with an optimal load level of 85%). There is an increase in the bed turnover rate from 30.1 to 33.9 in 2020. Conclusions. The provision of beds for medical care in Kazakhstan as a whole is sufficient, with a relative shortage of certain (palliative, rehabilitation, etc.) bed profiles. At the same time, the need of patients for inpatient care remains, as evidenced by the order in the portal of the bureau of hospitalizations.

Health Indicators of the Population of Khromtau District of Aktobe Region 阿克托别州赫罗姆托县人口健康指标

The study presents the dynamics and characteristics of demographic indicators, as well as health indicators of the population of the Khromtau district of the Aktobe region in the period 2019-2020, in the aspect of studying the impact on the statistical indicators of the SARS-COVID-19 pandemic.The purpose of the study was to assess the territorial features of primary health care indicators in Khromtau district of Aktobe region for 2021-2020.Research methods included elements of descriptive statistics. The general demographic indicators, the results of screening studies, the performance indicators of the Disease Management Program, the dynamics of morbidity and mortality rates of the population due to socially determined and monitored diseases were analyzed.Results. In 2020, there is a decrease in primary morbidity by 29.4% (8 807.5 0/0000 (2019 – 12 447.10/0000 )). Primary morbidity in the district decreased due to the child population – 10 703.70/0000 (2019 year - 21 533.70/0000 ).This can be explained in general by a decrease in the number of people seeking medical care for chronic diseases during the pandemic. However, the mortality figures for individual nosologies show an increase in rates, which in turn may also be due to late seeking medical attention Mortality from tuberculosis in 2020 was 1.60/0000 (2019 - 1.30/0000 ), from diseases of the circulatory system - 28.50/0000 (2019 - 15.30/0000 ), from diseases of the gastrointestinal tract for 2020 - 39.50/0000 (2019 - 15.30/0000 ); from diseases of the urinary system for 2020 - 37.30/0000 (2019 - 32.80/0000 ); as a result of injuries and accidents for 21.90/0000 (2019 - 6.60/0000 ).Conclusions. Despite the decrease in primary morbidity, there has been an increase in mortality, which most likely represents the consequences of the pandemic, including the restriction of the population's access to medical care, and in the future, primary health care efforts should be directed to active prevention and detection of chronic diseases.Keywords: primary health care, population health indicators, socially significant diseases, morbidity, mortality.

Analysis of the Environment for the Development of a Strategic Planning Strategy for Drug Supply 药品供应战略规划发展的环境分析

This manuscript discusses the issues of strategic planning of drug supply for the population of the Republic of Kazakhstan on the example of a medical organization in the city of Nur-Sultan.The main goal of the Drug Supply Strategy is to form a rational system of drug supply for the population with sufficient financial resources to meet the needs of patients in affordable, high-quality and effective medicines.The work used methods of comparative analysis of scientific literature on the topic under study, analysis of primary and secondary statistical data on strategic planning and drug provision of patients of the Center for the Rational Use of Medicines and Medical Technologies, the National Scientific Center for Health Development named after Salidat Kairbekova, as well as the results of surveys of patients and medical personnel.The data obtained in the course of the study made it possible to identify problematic aspects of drug supply and further develop recommendations for improving the strategic planning of drug supply in a medical organization. We have given recommendations for improving the strategic planning of drug supply in a medical organization.Key words: drug supply, medicines, medical care, strategic planning.

The purpose of the study: to assess the degree of availability of medical personnel in the field of traumatology and orthopedics in Kazakhstan.Methods. In this work, an analysis was made of the number of orthopedic traumatologists in Kazakhstan for 2019-2020. A comparative analysis of the indicators of the number and traumatologists-orthopedists by region was carried out.Results. A low availability of orthopedic traumatologists was revealed in a number of regions of Kazakhstan: in 2020, in Atyrau, Mangystau regions and in the West Kazakhstan region, the number of orthopedic traumatologists was 0.3 per 10 thousand of the population. In the Kyzylorda region, this indicator was 0.4 per 10 thousand people. The highest rates of the number of traumatologists-orthopedists are observed in the cities of Almaty and Nur-Sultan.Conclusion.There is a significant uneven distribution of specialists in the field of traumatology and orthopedics in the regions of Kazakhstan. To address the issue of lack and uneven distribution of human resources in the field of traumatology and orthopedics, it is necessary to improve the system of training, distribution and remuneration of traumatologists-orthopedists in Kazakhstan.Keywords: medical personnel, traumatologists-orthopedists, healthcare system, availability of human resources.

The purpose of the work: to conduct a comparative analysis of the list of essential medicines of the World Health Organization with the Kazakhstan National Medicinal Formulary to determine the similarities and differences in their composition.Methods: a comparative analysis of the 21st main list of medicines of the World Health Organization with the Kazakhstan National Medicinal Formulary was carried out to search for common and distinctive features of the studied lists of medicines. For the convenience of the study, spreadsheets were used, also available on the websites of the studied lists of medicines.Results: the total number of medicines listed in the 21 list of essential medicines of the World Health Organization is 567, of which 385 are included in the Kazakhstan National Drug Form. The total number of medicines listed in the Kazakhstan National Drug Formulary is 2020-702. Based on the common identification of analogues from both lists, the Kazakhstan National Drug Form has fully complied with the recommendations of the World Health Organization regarding essential medicines in 4 out of 29 standard therapeutic sections.Conclusions: during the study, we did not reveal any obvious differences in the choice of the Main medicines of the Kazakh National Medicinal Formulary. According to the results of our research, it became clear that the Kazakh National Medicinal Formulary contains 67.9% of the Main Medicines of the World Health Organization, that is, more than half of the medicines of the Kazakh National Medicinal Formulary correspond to the 21 Approximate List of Essential Medicines of the World Health Organization.

Problems of the Effectiveness of the Work of General Practitioners 全科医生工作的有效性问题

In the course of the literature review, four main factors were identified that affect the effectiveness of the GP and, in most cases, have a complex effect on the doctor's activity: the quality of education, organizational factor, financial motivation and psychological factor. These factors are common to all employees of the healthcare system and, in particular, affect the efficiency of the GP. Identification of the causes of their occurrence is necessary when developing methods to improve the quality of PHC services.Keywords: primary health care, general practitioner, work efficiency, polyclinic, labor organization.

Providing Rural Population of the Republic of Kazakhstan with Medical Services Involving Mobile Medical Complexes 向哈萨克斯坦共和国农村人口提供流动医疗中心的医疗服务

The purpose of the study is to analyze the provision of medical services to the population living in remote rural areas with the involvement of mobile medical complexes.Methods. In the course of the study, we analyzed statistical data on providing the population living in remote rural areas of Kazakhstan with medical services involving mobile medical complexes in 2020-2021.Results. The analysis indicated, that the coverage of rural residents with medical examinations provided by mobile medical complexes increased 3.8 times from 6.1% in 2020 to 23.5% in 2021; the amount of specialized vehicles increased 2.6 times from 58 to 152 vehicles, the load per 1 unit of specialized vehicle decreased 2.6 times; the volume of diagnostic studies increased 4.3 times, the volume of laboratory studies increased 2.9 times; the coverage with diagnostic studies per 1 rural resident slightly decreased by 0.3% on average; the coverage with laboratory tests per 1 rural resident increased 1.3 times on average; the number of specialists’ consultations per 1 rural resident increased on average 3.1 times.For the analyzed period of 2020-2021, we see a positive trend as the number of new identified cases for all studied medical conditions increase; however, the proportion of followed-up cases on primary care level remains as low as 17% of the identified patients.Conclusions. The issue of providing population in remote rural areas with medical services involving mobile medical complexes is supported by the government, however, it still requires significant support from the regional health departments and the top managers of healthcare organizations where mobile medical complexes are deployed
