Background : Paranasal sinuses are lined by the respiratory mucosa which is psuedostratified ciliated columnar epithelium. Cholesteatoma of maxillary sinus is characterized by alteration in the normal respiratory epithelium which gets replaced by the hyperkeratotic squamous epithelium leading to the formation of sheets of keratin and this is known as nasal cholesteatoma which has the property to erode the adjacent bony structures and should be considered as differential diagnosis of other nasal conditions of gradual expanding masses of the maxillary sinus. We are reporting one such case of 44yrs old female patient who presented with unilateral nasal obstruction diagnosed as maxillary sinus cholesteatoma with aid of CT scan nose & PNS and successfully corrected surgically.

Axillary or Langer's arch muscle is one of the rare anatomical variations in the axillary region. It is a muscular band running from latissimus dorsi to pectoralis major, biceps brachii or coracoid process and it can be deep or superficial. In its course it is closely related to the nerves and vessels of the axillary region. The evolution of this muscle states that it is a remnant of panniculus carnosus, which is an extensive subcutaneous sheet of muscle. In clinical practice it is seen associated with costoclavicular compression syndrome, neurovascular entrapment, thoracic outlet syndrome and it leads to confusion in routine axillary clearance in breast cancer. The anatomical knowledge of this variation is of great use for surgeons, clinicians and radiologists in interpreting axillary mass. We report an anomalous axillary arch muscle, its morphology and clinical significance had been reviewed.

Background: Tympanoplasty is a procedure to eradicate disease in the middle ear and to reconstruct the hearing mechanism. Objectives: To assess the graft placement time, surgical and audiological improvement and to estimate the fibroblast count in dry and wet temporalis fascia. Materials & Methods: Out of 50 patients, two groups comprising of 25 patients each underwent type 1 tympanoplasty using dry and wet graft respectively. A comparative analysis was done on the graft placement time, fibroblast count and hearing improvement postoperatively between the two groups. Results: Complete graft uptake was seen in 96% patients in Wet graft group and 100% patients in Dry graft group. Majority of wet grafting was done within 10-12 minutes compared to dry grafting which was done within 3-6 minutes. Even though Fibroblast count was increased in wet graft tissue (88%) when compared with dry graft tissue (52%), surgical outcomes were similar for dry and wet grafting. We observed that there was statistically insignificant difference between the success rates of graft up take and hearing improvement on both groups. Conclusion: There is equal success rates, in terms of graft up take and hearing improvement, whether it is dry or wet temporalis fascia grafting, but Graft placement time will be less in dry grafting as compared to wet grafting.

Clavicle is a frequently fractured bone in the body. It accounts for 44% of all fractures around the shoulder girdle and 2.6% of all fractures. Bilateral clavicle fracture is rare with incidence of 0.48% of all clavicle fracture. Undisplaced and minimally displaced fractures are mostly managed conservatively. Displaced fractures are treated operatively which improves the functional outcome and lowers the rate of malunion and non-union. A 45 years old male met with a road traffic accident and was brought to our emergency department. He was diagnosed to have bilateral displaced midshaft fracture of clavicle. Fracture was classified as Allman Group 1, Robinson type 2B1 bilaterally. Both fractures were fixed with precontoured clavicle locking plate by classical superior incision in two different sitting 1 week apart. Treating bilateral clavicle fracture with locking compression plates is a safe and effective method. It helps in early pain relief, rapid recovery, good functional outcome and decreases the period of morbidity.

Leiomyoma of uterus is a benign tumor commonly seen in females of Reproductive age group. It arises from pelvis which can be of variable sizes. We are reporting a case of 40 yrs old women with history of cough & generalised weakness for two weeks. Abdominal examination revealed a 22 weeks mass arising from pelvis, firm to cystic in consistency, mobile and non tender on palpation. Ultrasound showed a large mass filling the abdominal cavity, but MRI suggested degenerative fibroid. The patient underwent total abdominal hysterectomy with bilateral salpingoophorectomy. In general fibroids can be managed medically, surgically, or expectant management, or combination of all the three treatments.
