Introduction: Polyneuromyopathy constitutes a common complication in critically ill patients of the Intensive Care Unit (ICU) and in the last few years it appears to be identified as a syndrome detectable in the limbs and respiratory muscles. It is associated with the difficulties during weaning from mechanical ventilation. Aim: The present study investigates the reflective reaction of the soleus muscle following an electrical stimulation of the tibial nerve in intubated critically ill patients hospitalized in ICU with no medical history prior to their admission. Methods: Thirteen (13) patients who had been hospitalized for more than five (5) days and had a high APACHE II score (>15) and 13 age-matched control subjects were asked to participate in the present study on a volunteer basis. During the study, as reflective response parameters the range of the H-reflex and M-wave of the soleus muscle, as well as the conduction velocity of the tibial nerve, after electro-stimulation of the tibial nerve at the popliteal-fossa level, were assessed Results: Statistical analysis revealed significantly lower values in the ICU patients compared to healthy controls in both H-reflex range (p<0,049) and the M-wave range (p<0,041), as well as conduction velocity (p<0,001) of the tibial nerve. Conclusions: It is concluded that the reflective response of the soleus muscle as well as the tibial nerve’s conduction velocity are affected in critically ill patients hospitalized in ICU. The study of the above neurological parameters can provide further insights into the establishment and progress of polyneuromyopathy of critically ill patients in ICUs.

A lot of women choose the immediate breast reconstruction after mastectomy. The most common method is breast reconstruction using tissue expanders. Women's way of life and the economic crisis have created the need for adopting enhanced recovery after surgery (ERAS) protocols and early hospital discharge. The present study aims at informing perioperative nurses about the ERAS protocols and encouraging their implementation. ERAS protocols in breast cancer patients undergoing immediate post-mastectomy breast reconstruction include a combination of evidence-based interoperative interventions with the participation of surgeons, anesthetists, nurses, physiotherapists, and dietitians as members of the multidisciplinary team. This is a multifactorial approach to postoperative rehabilitation and care of the surgical patient. ERAS protocols are easy to apply, reduce morbidity, perioperative anxiety, postoperative pain, postoperative complications and hospitalization. In addition, they reduce the cost of treatment and hospitalization. The patient's approach to breast cancer following an ERAS protocol is divided into three phases: pre-surgical, intraoperative and post-operative. It starts before the patient’s hospitalization and continues until they are discharged, requiring active participation throughout the process. The total average hospitalization for post-mastectomy rehabilitation decreased from 3.6 days, prior to ERAS implementation, to 0 (discharging the same day) to 1.2 days post-surgery, releasing 30% of the beds. In conclusion, the ERAS protocols, following evidence-based perioperative care, promote the rapid recovery of patients aiming at reducing post-operative stress and its effects.

Introduction: Lymphedema is a frequent complication in women after mastectomy. It affects the patient's quality of life and functionality. The aim of this systematic review was to investigate and report appropriate, and evidence-based, guidelines for the prevention and treatment of secondary lymphedema of the upper extremity after mastectomy. Methodology: Relevant Greek and international literature was searched in the electronic databases PubMed, Cinahl, Sciverse, Scopus and Proquest by using keywords and time restriction. Results: Thirty-six papers were retrieved, of which 29 fitted the study’s inclusion criteria. These papers were addressing five categories of post-surgery interventions: Manual Lymphatic Drainage (MLD), Pneumatic Compression Devices (PCDs), physiotherapy exercises, weight lifting and the use of low frequency laser. MLD is a conventional method with controversial therapeutic effects. Similar are the effects of PCDs, which are predominantly used by patients even at home setting. Physiotherapy exercises are indicated to prevent lymphedema immediately after surgery, while weight lifting does not seem to have a negative effect despite current beliefs. Alternative treatments such as hydrotherapy, yoga, and low laser treatment are under investigation. Standard therapeutic approach includes Complete Decongestive Therapy (CDT), i.e. the combination of Manual Lymph Drainage (MLD), Pneumatic Compression Devices (PCDs), skin care, compression bandages and physiotherapy exercises. Conclusion: Complete Decongestive Therapy (CDT) is the basis of therapeutic interventions with the remaining methods having a complementary role. However, the effectiveness of interventions is not well documented and more research is needed.

Sleep disorders have reached epidemic rates in the modern world. It is important to note that they can affect children as well as adults. Sleep is of the utmost importance for the maintenance of body homeostasis. Many disease processes affecting sleep have been associated with numerous symptoms, including cognitive impairment, fatigue as well as a wide range of behavioral, hormonal and metabolic derangements. Sleep disorders in childhood can be classified by age of incidence (in infants, toddlers, school-age children or adolescents) and by the exact nature of the symptoms. They are classified in insomnias, hypersomnias, parasomnias, circadian rhythm disorders and respiratory disorders which affect sleep. Parasomnias are the most common sleep disorders in toddlers, with night terrors, nightmares and sleepwalking being the most common. Respiratory disorders are the most important in older children, especially obstructive sleep apnea syndrome. All sleep disorders are clinically manifested with symptoms of sleep deprivation. Their management always involves improvement of sleep hygiene, though in some cases (such as obstructive sleep apnea) specific medical treatment is also available and warranted. A three-way relationship of trust, compassion and cooperation between child, family and healthcare professional is a prerequisite for the effective management of childhood sleep disorders. Nursing staff are the main factor cultivating that relationship in the context of inpatient care.

Introduction: Despite the recent advances in ulcer treatment and management, and the immense advancements in technology, pressure ulcers are still a major challenge for nurses and doctors who are responsible for their management. Aim: The aim of the present study was to investigate the knowledge, practices and attitudes of nursing staff in a large public hospital regarding prevention and management of pressure ulcers. Methods: This study is a research cross-sectional study which involved all nursing staff (nurses and nursing assistants) working in medical and surgical wards of the General Hospital of Nikaia “St Panteleimon”. The questionnaire used for data collection has been developed by Gouda et al. (2014). The reliability of the questionnaire was high (Cronbach a=0.714) and the response rate was 76.7%. Statistical analysis was performed with the SPSS for Windows (21.00) statistical software. Results: The sample of the study consisted of 92 nurses aged 40.5(± 7.9) years. A total of 41 (44.6%) participants were nurse assistants, 33 (35.9%) had graduate from applied science universities, 14 (15.2%) had postgraduate degree and 4 (4.3%) had a university degree. The participants’ working experience was 15.1(± 9.1) years, while 45 (48.9%) nurses were working in surgical departments and 47 (51.1%) in medical ones. Of the total sample, twelve (13.3%) nurses have claimed that training seminars on prevention and management of pressure ulcers have been organized in the hospital that they are working. In a total of 27 questions, the correct answers given by the respondents were 18.59(±2.8) or 68.6% (±10.4%) with a minimum of 12 (44.4%) and a maximum of 25 (92.6%) correct answers. Participants with a higher level of education and those who had attended educational seminars [18.7(±2.8) versus 17(±2.6)] answered significantly more questions than those with a lower level of education (p <0.05). Conclusions: The nursing staff working in the medical and surgical wards had moderate levels of knowledge and high level of knowledge for the definition and classification of pressure ulcers. It is proposed that clinical protocols be drawn up concerning the staging, prevention and management of pressure ulcers, as well as a more organized continuing training of nursing staff.
