Acute’ Acquired Comitant Esotropia with myopia (AACEM) is an unusual form of non-paretic, diplopia-inducing strabismus. It is a subset of (acute) acquired comitant esotropia with recent reports of increasing prevalence. No reports exist comparing botulinum toxin (BTX) to surgery beyond the six-month therapeutic effect of BTX in a western population. Clinical characteristics of AACEM are defined and immediate and longer-term results of both interventions are reported in this retrospective review of 16 cases from two centres. Eight cases received BTX and eight cases had primary surgical intervention. Mean distance and near angle pre-surgery was 37.50±10.35 prism diopters (PD) and 35.63±12.66 PD respectively. Mean distance and near angle at final follow-up post-surgery (475.88±301.21 days, median 466.5) was 5.62±3.78 PD and 5.88±5.25 PD respectively. Mean distance and near angle pre-BTX was 36.00±15.33 PD and 38.75±14.58 PD respectively. Mean distance and near angle at final follow-up post-BTX (789±548.98 days, median 700) was 20.13±15.57 PD and 22.25±17.85 PD respectively. In this the longer-term follow-up study surgery was found to be superior as compared to BTX post-intervention. Surgical alignment also appeared more stable

Case Report, Maxime Dantouma, Sovogui, Abdoul Karim Baldé, Christophe Zoumanigui, Mamadou Bobo Doukoure, Kokou Vonor
Objective: To describe the ophthalmological manifestations of idiopathic intracranial hypertension at the Mauritius Clinic. Patients and Observations: We report two cases of idiopathic intracranial hypertension revealed by papillary edema, received in consultation at the Mauritius Eye Clinic. The symptomatology was dominated by visual eclipses, headaches and tinnitus. Decreased visual acuity was associated in patient No. 2. Papillary edema was found in both grade III cases and respectively. Consultations, paraclinical and I examinations in neurosurgery and infectiology were not contributory. The patients all benefited from a drug treatment based on acetazolamide 250mg. Cortancyl 20mg with a low-calorie diet have been combined with the treatment of patient No. 1. The evolution was favorable in patient No. 1 while patient No. 2 was at risk of visual loss due to the stage of papillary edema. Conclusion: Faced with papillary edema associated with visual disorders, headaches, tinnitus and/ or not a decrease in visual acuity, it is necessary to suspect idiopathic intracranial hypertension until proven otherwise.

Phacomorphic glaucoma is the term used for secondary angle-closure glaucoma due to lens intumescence, leading to pupillary block and closure of the irido corneal angle (ICA). This condition presents as an acute glaucoma crisis due to angle closure, which can lead to serious complications. The aim of our study is to determine the usefulness of ultra biomicroscopic ultrasound (UBM) in the evaluation of the iridocorneal angle before and after cataract surgery in phacomorphic glaucoma. The prospective analytical study included 25 eyes diagnosed with phacomorphic glaucoma. The intraocular pressure, the visual acuity, the UBM parameters including; the Anterior Chamber Depth (ACD), the Trabecular-Iris Angle (AIT), the Angel Opening Distance (AOD500), were assessed preoperatively and postoperatively. The mean age of our patients was 74.7 years, there was no significant gender preference. The mean intraocular pressure (IOP) under maximal treatment was reduced from 45.36 5.4 to 25.8 mmHg in our study. With IOP at 17+/-2.5 to 15+/-1.3 after surgery within 1-3 days. The gonioscopic results were in agreement with the UBM results, which provided more specific angle measurements; all angle parameters are improved after cataract surgery. All eyes had a mean angle of more than 20° in all four quadrants with a mean angle of (37.1+/-4.5), very similar to the angle status of the uninvolved eye. In conclusion, after an acute attack of angle closure of phacomorphic origin, all parameters of the ICA measured by UBM increased significantly as well as normalization of IOP after cataract surgery
