Demystifying C shaped canals 揭开C形运河的神秘面纱

Introduction: Knowledge of root canal anatomy is essential in order to achieve a successful root canal treatment. C shaped canal anatomy is most commonly seen in mandibular second molars and to some extent mandibular premolars. Such canals are difficult to negotiate, debride/disinfect and obturate due to its high incidence of fins, webs, lateral canals and anastomoses. Inability to identify and debride such canal anatomy may lead to endodontic failure. Case Report: These case series highlights on two different c shaped canals with two different obturation techniques. Conclusion: Complexity of C shaped canal morphology can be managed successfully with proper irrigation and obturation techniques.

The effect of different air blasting pressure of polyetheretherketone (PEEK) on surface roughness and bond strength to adhesive resin cement: An in-vitro study 不同空气爆破压力对聚醚醚酮(PEEK)表面粗糙度和与胶结树脂水泥结合强度的影响:体外研究

Statement of the Problem: Polyetheretherketone material shows good physical and mechanical properties but research is lacking optimum air blasting parameters for good bond strength to resin cement. Aim: To evaluate the effect of different air blasting pressure of PEEK on surface roughness and shear bond strength to adhesive resin cement. Materials and Methods: 24 discs of BioHPP PEEK (bredent, UK) were manufactured. Samples were air blasted with 110 μm Al 2 O 3 particles for 10 seconds at distance of 10 mm and divided into 3 groups according to pressure. Group A: 3 bar, Group B: 2.5 bar and Group C: 3.5 bar. Surface roughness was measured by SEM, Visio Link primer (Bredent, Germany) was applied and cured then G-CEM resin cement (GC, Japan) was applied and cured. Shear bond strength was measured using universal testing machine. EDX analysis was done, and failure mode was detected by stereo-microscope. Results: Statistical analysis showed that roughness average (Ra) increased by increasing pressure, Group B was significantly the lowest ( p < 0.05), while there was insignificant difference between other groups (P > 0.05). Regarding shear bond strength, Group A recorded (12.6864 ± 2.486 MPa), Group B (11.2842 ± 1.555 MPa) and Group C (10.1024 ± 2.317 MPa), there was significantly statistical difference between Groups A & C ( p < 0.05). The failure mode was mainly adhesive in group C and mixed in groups A & B. EDX analysis showed an increase in Carbon and Silica and a decrease in Oxygen and Aluminum weight percentages after the bond strength test. Conclusion: Increasing the air blasting pressure up to 3 bar increases the surface roughness and bond strength to resin cement, but more than 3 bar showed unfavorable surface roughness and decreased bond strength.

Nonsurgical periodontal therapy for the management of inflammatory gingival enlargement: A case report 非手术牙周治疗治疗炎症性牙龈肿大1例报告

Inflammatory gingival enlargement is a common type of gingival enlargement caused by dental plaque. It can lead to problems like difficultly in oral hygiene maintenance, speech as well as may impair esthetics. This case report describes a case of an inflammatory gingival enlargement in a systemically healthy young male involving lower anterior teeth. It was successfully managed solely by non-surgical periodontal therapy. This case report shed light to the importance of non-surgical periodontal therapy.

Intranasal midazolam for management of down syndrome patient: A case report 鼻内咪达唑仑治疗唐氏综合征1例报告

Down syndrome (DS) is a congenital autosomal disorder characterized by generalized growth and mental deficiency. Patients with down syndrome may be anxious and uncooperative during dental treatment thus oral health professionals may need to use physical restraints, which causes further emotional trauma. Intranasal sedation is a non-invasive, safe and well tolerated method by children. This paper presents a case report of a 12-year-old female patient with known diagnosis of Down syndrome, who was treated successfully under intranasal sedation using midazolam (0.5 mg/kg), in which multiple extractions were performed. Satisfactory levels of sedation were achieved with no adverse effects. Post-operative healing of socket was seen without any complications after 7 days of follow up.

Effect of curing cycles on vertical dimension of complete Dentures: An in vivo study 固化周期对全口义齿垂直尺寸影响的体内研究

The Purpose of the study is to determine the effect of long curing cycle and short curing cycle on changes in vertical dimension of occlusion of complete. The study was conducted in 20 completely edendulous patients, 10 complete dentures were fabricated with short curing cycle and 10 dentures with long curing cycle. All of the initial clinical steps involved in fabrication of dentures, with the exception of the curing cycle, were the same for both the groups. Vertical dimension of occlusion was measured with digital vernier caliper from specific points of reference on upper and lower member of articulator at 3 stages, stage 1 before polymerization, stage 2 after remounting the processed complete dentures and stage 3 after remounting of finished and polished complete dentures. In order to determine the change in vertical dimension, the measurement recorded prior to processing the dentures was subtracted from the measurement recorded just after processing as well as the one recorded after finishing & polishing. The readings were statistically analyzed and compared. Both the groups demonstrated a highly significant increase in VDO after processing of dentures. On comparison between both the groups, non significant results were obtained in the mean difference in VDO just after processing and after finishing & polishing of dentures. The clinical relevance of these changes does not confirm the superiority of any one curing cycle in terms of dimensional changes in VDO after processing.

Apical sealing with MTA in a tooth with incomplete rhizogenesis and enamel hypoplasia 牙根发生不完全,牙釉质发育不全,用MTA封堵牙尖

Introduction: In teeth with incomplete rhizogenesis, the canal is frustoconical and broad, and the foramen is absent. The disinfection and obturation protocol must be adapted to keep the treatment within the root limits. Mineral Trioxide Aggregate (MTA) performs an apexification by creating a hard tissue barrier with cement deposits, it is not toxic to periradicular tissues. Hypoplasia is a quantitative enamel defect caused by hereditary factors and/or physiological stress. Clinical case: 12-year-old patient with generalized enamel hypoplasia, asymptomatic irreversible pulpitis, and normal periapical tissues as a diagnosis. Treatment: Apexification with MTA. Conclusion: MTA enables apexification, as it is biocompatible and does not cause inflammation in the periradicular tissues, forming an apical seal with hard tissue deposit.

Comparative clinical evaluation of giomer and glass ionomer in primary teeth caries 异构体与玻璃离聚体治疗乳牙龋的临床比较评价

Introduction: The giomers are a family of dental materials that allow a faster effect and better functional, esthetic and mechanical properties. Objective: To analyze the restorative ability between giomer and type II glass ionomer in class I cavities. Methodology: A mouth split design was carried out in which 27 permanent molars ICDAS 4, 5 or 6 were filled with giomer (14) and glass ionomer type II (13), respectively, in patients with high or medium caries risk. Intraoral radiographs and polyvinyl siloxane impressions were taken every 3 months. FDI criteria were used to evaluate radiographic success, material fracture, and marginal adaptation. Results: Compared to glass ionomer, giomer had better marginal adaptation at 6 and 9 months ( p< 0.05), was also better in fracture restoration at 3, 6 and 9 months ( p< 0.05), and better radiographic success at 6 and 9 months ( p< 0.05). Conclusions: Giomer has better clinical results in terms of marginal adaptation, radiographic success and fracture of the material than glass ionomer.
