Coronary artery fistula is a rare congenital anomaly. Thebesian veins are multiple coronary artery microfistula that cominicates coronary arteries and ventricles. Most patients with this anomaly are asymptomatic, but some patients may be symptomatic and have a presentation of myocardial ischemia, thrombosis and embolism, heart failure and arrhythmias. We present a case with exertional dyspnea and angina whose myocardial perfusion scintigraphy demonstrated inferior and apical defect with complete reversibility. Coronary angiography showed no coronary stenosis, but capillary blush draining into the left ventricular cavity through multiple microfistulae from the left anterior descending artery, circumflex artery and right coronary artery mimicking left ventriculography. Because of these small multiple fistulas are managed medically, the patient was discharged on a beta blocker, aspirin and ranolazine therapy. The patient stated a reduction in his symptoms one month later.

Babesiosis is tick-borne zoonotic infection caused by Babesia microorganism that settles into erythrocytes. Although it is seen as sporadic in our country, there are not so many cases published on in this subject. In this case, we present a case which was investigated for fever and pancytophenia, and in case of not having tick contact, intra-erythrocythe parasites were seen on the peripheral blood smear and Babesiozis pre-diagnosis and quinine and clindamycin upon Babesiozis prediagnosis, as a result of which the patients’s fever decreased and the complete blood count improved on the second day of the treatment.

Objective: We aimed to compare the rates of denosumab and zoledronic acid in patients receiving osteoporosis treatment and the side effects of these drugs. Material and Method: In our study, the files of the applicants were searched between January 2016-December 2017; those with denosumab and zoledronic acid. Clinical and demographic data, laboratory values of beginning of treatment, 6 months and 1 year and side effects were recorded. Results: 171 patients were included. Denosumab group was older, their urea and creatinine levels were higher and GFRs were lower (respectively, p: <0.001, p: <0.001, p: <0.001, p: <0.001). Hypocalcemia was observed only in denosumab group (1 (1.23%) patient). Acut renal failure occured in 4 (% 5,19) patients with denosumab and 2 (%2,12) patients with the other group during 1 month follow up. Flu-like symptomps were observed only in zoledronic asit group (2 (%2,12) patients). Bilateral spontaneous fracture of the ischium pubis was observed in 1 (1.29%) of the former, spontaneous 6 teeth loss in 1 (1.06%) of the latter group. Conclusion: There were no significant differences between two groups in terms of side effects. Side effects were lower than those reported in the literature; in addition to comorbidity, multiple drug use and rare side effects should be kept in mind.

Introduction: Granulomatosis polyangitiis (GPA), eosinophilic granulomatosis with polyangiitis (EGPA and microscopic polyangiitis(MPA) are small vessel vasculitides associated with anti-neutrophil cytoplasmic antibody (ANCA). İn this study, the frequency of ANCA associated vasculitis and treatment responses in our clinic are evaluated. Methods: This retrospective study was performed between January 2015- December 2018. 18 patients who older than 18 and diagnosed ANCA associated vasculitis according to the 2012 Chapel Hill Classification were included. Results: İn totals, 18 patients who diagnosed ANCA associated vasculitis were included, 12 of whom females, 6 of whom males. The mean age of the patients was 57.8. Two of the patients were diagnosed with EGPA, 3 with MPA and 13 with GPA. 11 of the patients followed up with GPA with systemic involvement and 2 with limited involvement. The most common involvement in patients with GPA was pulmonary involvement with 76% and renal involvement in 53% of the patients. Pulmonary involvement was detected in 2 patients and lung involvement in 2 patients in MPA. Pulmonary involvement was observed in 2 patients with EGPA. RF and ANA positivity was highest in GPA. İt was observed in combination with pulse steroid and cyclophosphamide in the induction treatment of ANCA-associated vasculitis. It was seen that 3 GPA patients had recurrence and followed up with rituximab. Discussion-conclusion: ANCA related vasculitides treatment and clinic are similar. İn the treatment of ANCA-associated vasculitis, remission should be achie- ved in a short time. In recurrent cases, rituximab may be preferred.
