A 49-year-old male patient with diffuse myeloma is reported. Although obvious punched-out radiolucency is frequently seen in multiple myeloma, in this case it was not observed, with only a faint resorption of the alveorar ridge being noticed. It is emphasized that a biopsy, radiographic examination and other laboratory findings are necessary to diagnose this disease.

Radionuclide imaging has been suggested as a means to assess healing and viability of bone grafts and implants. This study deals with the in vivo uptake of 99mTc-MDP in the initial postoperative period in rats. Isogeneic or xenogeneic bone was implanted in subcutaneous pouches. The isogeneic grafts and implants were treated with saline or free2e-treated or demineralized. The xenogeneic implants consisted of Kiel bone or collagen sponge. One day after surgery, the accumulation of 99mTc after an intraperitoneal injection of 99mTc-MDP was seen in all grafts and implants containing bone mineral independent of prior treatment of the graft and implant. In implants composed of only the organic part of bone, no accumulation was observed. One prerequisite for accumulation of 99mTc-MDP is the presence of the inorganic phase. Thus, the interpretation of the initial postoperative scan is based on whether or not the graft or implant initially contained mineral.

A jaw tumour in a child presented diagnostic difficulties because of the presence of myxomatous elements and actively forming cementum-like calcified material. The 5-year follow-up suggests that this was an unusual benign odontogenic tumour.

A review of the literature suggests that traumatic bone cysts have been detected more frequently since the advent of panoramic radiography. Some authorities believe that these lesions may regress spontaneously without intervention within a period of time. Surgical exploration will enhance the bony repair of these cysts and is the usual recommended treatment. A case is reported in which complete regression of a traumatic bone cyst has occurred within 30 months of aspiration of its contents.

A qualitative and quantitative determination of serum immunoglobulins was made in 34 patients (27♀, 7♂) with histologically verified oral lichen planus (OLP). The subjects ranged in age from 33 to 80 years and 28 of the cases had OLP changes of atrophic-erosive type. The possible presence of rheumatoid factors (RF), antinuclear antibodies (ANA) or antibodies to mitochondrias, renal glomeruli and smooth muscles was studied in 30 of the patients. The investigation also comprised a control group of 23 patients (17♀, 6♂) without mucosal changes. Immunoelectrophoresis showed an abnormal pattern in 10 OLP cases (29%), the most common change being a polyclonal increase of IgG in 8 persons. Elevated levels of serum IgG (> 15 g/l) were seen in a total of 9 OLP patients (26%), but only in 1 of the control cases (4%). The mean IgG value was significantly higher in the OLP than in the control group, 13.4 and 10.3 g/l, respectively. Regarding levels of IgA and IgM, no difference was noted between the groups. Autoantibodies were registered in 27% of the OLP patients and in 9% of the controls. The immune changes could be correlated in some respect to positive fungal cultures, allergies and decreased unstimulated saliva, but not to age, endocrine disorders or joint diseases.

54 teeth from 20 patients irradiated for treatment of malignant tumors of the head and neck region were examined clinically, histologically and microradiographically. 18 teeth from 8 patients were used as controls. A significantly higher degree of hyposialia, radiation mucositis and atypical caries was found in the original group as compared to the control material. Occurrence of narrow gaps between dentin and cementum was also higher in the former group as compared with the latter.

The purpose of the present double-blind study was to investigate the preventive effect of 4 mg dexamethasone (1 ml Decadronphosphate®) on swelling, trismus and pain after removal of impacted mandibular third molars. The population comprised 30 healthy individuals (17♀, 13♂) who needed removal of 2 identical, impacted mandibular third molars. Each patient served as his own control as the teeth were removed in 2 sittings with either steroid or placebo injected into the masseter muscle just before starting the operation.
Control visits took place 48 h and 7 days after the operations. The results showed that steroid administration led to about 50% reduction of postoperative swelling and trismus and to about 30% reduction of postoperative pain. No general or local complaints/complications occurred due to the steroid injection.
It is concluded that prophylactic steroid treatment is effective in reducing postoperative complaints and that the administration is safe in the absence of contraindications for such administration.
Prophylactic steroid treatment is therefore recommended in third molar surgery when pronounced postoperative reaction can be expected.
