The Effect of Aromatherapy with Citrus aurantium Aroma on Pain after Orthopedic Surgery: A Randomized Clinical Trial. 柑橘香薰治疗骨科术后疼痛的随机临床试验

Introduction: Postoperative pain is one of the most common physiological and psychological stress in patients that disrupts body function and can endanger patients’ health. This study aims to determine the effect of aromatherapy with Citrus aurantium essential on pain after orthopedic surgery. Methods: This randomized clinical trial was performed on 60 candidates for orthopedic surgery. Patients were selected through convenience sampling and divided into intervention and control groups through randomized block allocation. If the visual analogue scale (VAS) score was above 3, patients in the intervention group received aromatherapy with C. aurantium essential and the patients in the control group received a placebo (almond oil). VAS was used to measure pain. Data analysis was performed using independent t test, paired t test, and analysis of variance with repeated measures using SPSS software version 13. Results: Mean (SD) of pain intensity after intervention in experimental and control groups within 4, 8, and 12 hours after surgery was 7.30 (1.23) vs. 7.90 (0.99), 5.30 (0.98) versus 5.53 (0.68) and 2.53 (0.9) vs. 3.60 (0.77) respectively. The findings indicated that there was a significant difference in mean pain intensity between the experimental and control groups at 4 and 12 hours after surgery. Use of analysis of variance with repeated measures test with taking into account the interaction of time and group also showed a significant difference in mean pain intensity between the two experimental and control groups. Conclusion: Aromatherapy with Citrus aurantium essential can be effective in reducing mild to moderate pain after orthopedic surgery. Further studies are recommended to confirm this finding.

The Effects of Ginger (Zingiber officinale) Extract Ointment on Pain and Episiotomy Wound Healing in Nulliparous Women: A Randomized Clinical Trial 姜提取物软膏对未产妇女会阴切口疼痛和伤口愈合的影响:一项随机临床试验

Introduction: Episiotomy is a usual midwifery surgery. Iran is a country with an abundant source of medicinal plants. This study aimed to investigate ginger extract ointment’s effects on the pain and recovery of episiotomy incisions in nulliparous women. Methods: This randomized clinical trial was conducted in a public hospital in Iran on 70 nulliparous women with an episiotomy incision. The women were randomly assigned to ginger extract ointment and placebo groups. The primary outcomes included pain and wound healing that were assessed using a visual analog scale (VAS), redness, edema, ecchymosis/bruising, discharge, and an approximation scale (REEDA). The participants were followed up before discharge from the hospital and 5×1 and 10×1 days after the intervention. The secondary outcome was the number of painkillers used during the study. Data were analyzed by chi-square, independent t test, and the Mann-Whitney U via SPSS-13. The significance levels were determined to be P≤0.05. Results: There was no significant difference between participants treated with ginger extract ointment and placebo in the pain and wound healing scores before the intervention, 5×1 and 10×1 days after the intervention. But, the pain intensity decreased, and the recovery speed increased clinically. Also, regarding the secondary outcome of this study, no significant difference between the placebo and intervention groups in the number of painkillers participants took. Conclusion: The ginger ointment could not significantly improve episiotomy wounds’ pain and healing rate, but it was clinically helpful. So more studies with different doses of this ointment are needed.

Health-Promoting Lifestyle among the Survivors of Colorectal Cancer: An Integrative Review 结直肠癌幸存者中促进健康的生活方式:一项综合综述

Introduction: Health-promoting lifestyle (HPL) among the survivors of colorectal cancer (CRC) is essential to reduce CRC complications, prevent its recurrence, and improve survival. Nonetheless, there is no comprehensive definition for the concept of HPL in CRC survivors. This study aimed to define the concept of HPL among CRC survivors based on the existing literature. Methods: This integrative review was conducted in 2021 using Whittemore and Knafl’s method. The national and international databases of SID, Magiran, ProQuest, Medline, ScienceDirect, Web of Science, and Scopus were searched. Quality appraisal was performed using the Mixed Methods Appraisal Tool (MMAT) and the data were analyzed using the four-step approach proposed by Whittemore and Knafl. Results: After data evaluation, 167 documents were included in final analysis. In total, 1863 codes were generated and categorized into eighteen main categories and the three main themes of antecedents, attributes, and consequences. Accordingly, HPL among CRC survivors was defined as "a set of behaviors in the areas of health responsibility, physical activity, nutrition, spiritual growth, psychological management, and interpersonal relations which are affected by socio-demographic characteristics, clinical characteristics, psychological status, physical conditions, time and place limitations, and patient education, and lead to better disease prognosis, better general health status, better bio-psycho-social status, and better quality of life". Conclusion: The definition of HPL among CRC survivors provided in the present study can be used in counseling, educational, supportive, and care programs for CRC survivors in order to improve their quality of life and survival.

Lessons Learned from COVID-19 for Future Pandemics: Infection Prevention in Health Care Workers. 从2019冠状病毒病中吸取的教训:预防卫生保健工作者的感染。

Dear Editor, Since the beginning of the pandemic in late December 2019 in Wuhan, China, and the rapid spread of the coronavirus disease 2019 (COVID-19) virus, until May 31, 2022, there were 6.9 million reported deaths and 17.2 million estimated deaths from COVID-19.1 Due to the unique genetic structure of coronaviruses, as well as their ability to reproduce and easily spread, the emergence of COVID-19 has not been unexpected for virologists. Based on the information collected to-date on COVID-19, due to the complex structure of the virus, high risk of inter-human transmission, presence of asymptomatic or mildly-symptomatic carriers, and progression of the condition to respiratory distress and 5-10% mortality, the occurrence of COVID-19 pandemic can be defined as a kind of perfect storm.2 The COVID-19 pandemic, in addition to its negative economic, social, political implications, has posed many challenges for the healthcare sector: dramatic increase of the need for medical staff, the cost of personal prevention and protection equipment, cost of diagnosing and treating patients, need for more intensive care unit and ventilated beds, and in the mortality rate in the community.3 Nurses, as the largest group of medical staff, spend more time with patients than other health care workers (HCWs). For many emerging diseases, there is no standard treatment, and nurses have the key role in the supportive care of the patients infected with emerging diseas.2 In the critical situation of emerging diseases, all social organs, even close relatives and the patient’s family, distance themselves from the patient, and it is the duty of the medical staff to take care of the patient despite the dangers that exist for them. 50% of those who died in the severe acute respiratory syndrome (SARS) epidemic were HCWs who became infected in hospital through caring of the infected patients.4 The prevalence of hospitalization among HCWs was 15.1% and mortality rate was 1.5 %.5 According to the Executive Director of the International Council of Nurses (ICN), “The fact that the number of nurses who died during the epidemic is similar to the number of people who died during World War I Is a giver. We have been calling for standard and systematic data collection on infection and deaths of HCWs, but the fact that we do not yet have accurate statistics is a major scandal”6 as the actual statistics are about 60% higher than reported.7 It is estimated that the rate of virus transmission within hospitals from patients to HCWs is about 29%.8 According to the Deputy Minister of Nursing of the Ministry of Health of Iran, more than 200 000 nurses are working in wards where patients with coronavirus have been hospitalized; since the outbreak of corona in Iran, more than 125 000 nurses have been infected with COVID-19.9 The COVID-19 pandemic makes it even more important to talk about the safety of all HCWs. Frontline Nurses' Experiences of Patient Care in the COVID-19 Pandemic: A Phenomenological Study. COVID-19大流行中一线护士护理患者的经验:现象学研究

Introduction: The COVID-19 pandemic is now a major public health crisis in the world. Nurses as key members of professional are exposed the most challenges caused by COVID19. Knowledgeable nurses' experiences can provide appropriate solutions to increase the quality of care and improve the health of the society. This study aimed to explore the lived experiences of nurses' caring for patients with COVID-19. Methods: 12 nurses participated in this phenomenological study. We performed purposeful sampling and in-depth face-to-face and semi-structured individual interviews for collecting data. Qualitative data, was analyzed by the 6-step Van Manen hermeneutic phenomenology. Results: After data analysis, the mean (SD) age of study participants was 32.25 (5.62) years and their mean work experience was 9.75 (5.39) years. From the analysis of data obtained from interviews with nurses working in COVID-19 wards, 1050 primary codes, 17 subthemes and 5 themes were extracted. Main themes include: sincere service, patient oppression, emotional instability, suspension and relaxation. Conclusion: Analysis of data in this study suggested that the nurses who care patients in COVID-19 pandemic, faced many professional and psychological challenges. Healthcare managers should plan for psychological support services for the nurses.

Qualitative data, was analyzed by the 6-step Van Manen hermeneutic phenomenology. <b>Results:</b> After data analysis, the mean (SD) age of study participants was 32.25 (5.62) years and their mean work experience was 9.75 (5.39) years. From the analysis of data obtained from interviews with nurses working in COVID-19 wards, 1050 primary codes, 17 subthemes and 5 themes were extracted. Main themes include: sincere service, patient oppression, emotional instability, suspension and relaxation. <b>Conclusion:</b> Analysis of data in this study suggested that the nurses who care patients in COVID-19 pandemic, faced many professional and psychological challenges. The Nurses' Perception of Expressive Touch: An Integrative Review. 护士对表达性触摸的感知:一项综合评价。

Introduction: Nurses use physical touch to interact with patients and address their needs. Human touch benefits social development, stress/anxiety reduction, and rapport building. Touch has been a part of nursing care for centuries, however nurses' perceptions of expressive touch are not easily ascertained from the literature. Literature currently offers one review on the perceptions of various medical professionals regarding touch of all kinds. No reviews specific to the nurses' perception of expressive touch exist. This integrative review provides a greater understanding of nurses' perceptions of expressive touch. Methods: Using Cooper's steps for integrative review, CINAHL, Medline, Academic Search Premier, and Complimentary Index were searched from 2000-2022; using search term expressive touch or caring touch or compassionate touch and nurse/nurses/nursing and perception or perspective. Results: Of 283 articles identified, 22 articles remained for full-text review after duplicate removal and application of inclusion/exclusion criteria. Five topics on nurses' perceptions of expressive touch were identified: Comfort with touch and job satisfaction, expressive touch as an essential part of nursing practice, expressive touch as a form of compassion and/or communication, the impact of expressive touch on the humanization of patients in the nurses' perception, and nurse discomfort with expressive touch. Conclusion: This integrative review provides findings that assist in understanding nurses' perceptions of expressive touch. Further research should examine the impact of gender, education, and experiences on nurses' perceptions as they use expressive touch in nursing practice.

Poor Care: A Walker and Avant Concept Analysis. 不良护理:沃克和先锋概念分析。

Introduction: Care is the fundamental fact of nursing. In recent years, poor care is discussed frequently in articles and newspapers, however health care providers do not have a common understanding of this concept. Therefore the purpose of this paper was to clarify the concept of poor care in nursing and highlight the importance of tackling this issue. Methods: The concept was analyzed using the 8-step Walker and Avant’s method. The SCOPUS, PubMed, ISI, and Embase databases were searched with the keywords "poor care" and "poor nursing care" in the titles and abstracts of articles. Of 550 sources found in the initial survey, 32 articles were finally included in the study. Results: Poor care attributes include poor evaluation, inadequate or inappropriate patient management, and delay, treating, and referring patients to other departments. These attributes are caused by antecedents of nursing workload, the complexity of patient conditions, inappropriate interactions, insufficient workforce, and educational and organizational factors. Conclusion: Poor care is a general concept that includes undesirable and unacceptable standards for receiving or providing clinical and interpersonal health services. Poor care includes eliminating planned activities or performing unplanned activities that are experienced by the patient, health care workers, or caregivers.

Lived Experiences of the Patients with COVID-19: A Hermeneutic Phenomenology. COVID-19患者的生活体验:解释学现象学。

Introduction: The COVID-19 pandemic is a considerable challenge for infected patients who require more attention for recovery. Therefore, this study aimed to explore the lived experience of patients with COVID-19. Methods: This qualitative, hermeneutic phenomenological study was conducted in the southeast of Iran from March to May 2020. The data were purposefully collected from 16 patients with COVID-19. Data were collected using in-depth semi-structured telephone interviews and analyzed using the Van Manen method. Results: The lived experiences of patients were categorized into five themes, including incredible clinical symptoms of COVID-19, horror and stigma, bad memories of hospitalization, trust in God and hope as recovery agents from COVID-19, and reborn after recovery from COVID-19. Conclusion: Patients with COVID-19 have experienced variety of physical and psychosocial challenges. Health care providers have to supply appropriate strategies to fulfill the infected patients needs in such a crisis.

The Effect of Thai Massage on the Severity of Pain in Patients with Unstable Angina: A Randomized Controlled Clinical Trial. 泰式按摩对不稳定型心绞痛患者疼痛程度的影响:一项随机对照临床试验。

Introduction: Chest pain is an unpleasant and the most common symptom in patients suffering from unstable angina. This study was implemented to investigate the effect of Thai massage on severity of pain in patients with unstable angina. Methods: This study was a randomized controlled clinical trial that carried out on 70 patients with unstable angina who had been hospitalized in hospital affiliated to Ahvaz Jundishapur University of Medical sciences, Ahvaz, Iran. The participants were allocated randomly to either the intervention (n=35) or control group (n=35) between August 1, 2016 and April 30, 2017. The patients in the intervention group received routine care beside and Thai massage with the duration of 30 minutes in two consecutive days. The patients in the control group received routine care. The intensity of pain was assessed by using the numeric rating scale, five minutes before and immediately, 15, 30 and 60 minutes after the intervention. The independent t-test and repeated measures analysis of variance (ANOVA) were used to analysis data through SPSS 13. Results: After the intervention, the severity of pain was significantly diminished by about four measurement points (P=0.001) with the linear pattern of pain reduction. Nonetheless, in the control group, no statistically significant difference in the intensity of pain was found. Conclusion: This study showed that Thai massage as non-invasive approach, concomitant with standard nursing care, has the potential to decrease chest pain. Nurses should be taught about pain relief methods for improving the quality of patient care.

The Parturient Women's Privacy Preservation in the Delivery Rooms: A Qualitative Study. 产房中产妇隐私保护的定性研究

Introduction: Parturient women's privacy preservation and respectful maternity care (RMC) in delivery room is an important principle in the high quality of midwifery care to achieve maternal satisfaction and positive childbirth experience. Hence, it is essential to make natural vaginal delivery (NVD) a positive experience and increase the mothers' satisfaction. This study aimed to investigate the privacy preservation of parturient women's in the delivery room. Methods: Using conventional content analysis, this qualitative study was conducted from June 2018 to December 2020 at two hospitals and three health centers in Shahroud, Iran. Purposeful sampling was employed and it was continued till data saturation through in-depth interviews with 37 participants. Results: The results of interviews with 21 women with NVD experience and 16 maternity health service providers resulted in the extraction of four themes including physical, spiritual-mental, informational, and social privacy. Conclusion: Various mechanisms were found to promote the privacy and satisfaction of parturient women in the delivery room. They included the necessity continuous education, monitoring about mother's privacy preservation and intervention to improve effective communication skills among staff in delivery rooms.

{"title":"The Parturient Women's Privacy Preservation in the Delivery Rooms: A Qualitative Study.","authors":"Farzaneh Valizadeh,&nbsp;Farahnaz Heshmat,&nbsp;Zahra Motaghi","doi":"10.34172/jcs.2023.30401","DOIUrl":"https://doi.org/10.34172/jcs.2023.30401","url":null,"abstract":"<p><p><b>Introduction:</b> Parturient women's privacy preservation and respectful maternity care (RMC) in delivery room is an important principle in the high quality of midwifery care to achieve maternal satisfaction and positive childbirth experience. Hence, it is essential to make natural vaginal delivery (NVD) a positive experience and increase the mothers' satisfaction. This study aimed to investigate the privacy preservation of parturient women's in the delivery room. <b>Methods:</b> Using conventional content analysis, this qualitative study was conducted from June 2018 to December 2020 at two hospitals and three health centers in Shahroud, Iran. Purposeful sampling was employed and it was continued till data saturation through in-depth interviews with 37 participants. <b>Results:</b> The results of interviews with 21 women with NVD experience and 16 maternity health service providers resulted in the extraction of four themes including physical, spiritual-mental, informational, and social privacy. <b>Conclusion:</b> Various mechanisms were found to promote the privacy and satisfaction of parturient women in the delivery room.
