Public Perception And Social Media Image Of Feldsher in Moscow 公众对莫斯科费尔泽的看法和社交媒体形象

Background. Despite being an unnoticed profession, feldsher is often the first point of contact for patients seeking medical assistance. Thus, in order to minimize profession-related risks, it is crucial to study the image of a feldsher, as well as to understand the characteristics and trends of the feldsher image that have emerged in public opinion and the information space. Purpose. To identify how Muscovites perceive feldshers and to assess social media coverage of their work. Methods. The study used the data collected in telephone surveys conducted among a representative sample of 596 adult Moscow residents. The authors analyzed 1,057 images and conducted content analysis on a subsample of 169,975 social media posts. Results. Collective consciousness mainly perceives the feldsher as an “emergency care practitioner” and a “rural physician” (as a specialist of a rural health post). 21 % of respondents are also unclear about the distinctions between feldshers and doctors or nurses, as well as their place in the medical hierarchy. The results of a quantitative poll are consistent with the social media content analysis about the perception of feldsher's responsibilities. The majority of messages (61 %) are neutral with a slight prevalence of negative (25 %) messages over positive (14 %). Positive messages are more often associated with personal qualities, particularly those that were made evident in dire circumstances during the epidemic, and negative messages are primarily associated with systemic issues of the profession’s status. The color coding of messages is mostly neutral-negative with a predominance of dark, gray, and brown.

Aspects of Healthcare Staffing With Specialists in Healthcare Organization and Public Health in Moscow 莫斯科医疗组织和公共卫生专家的医疗人员配备问题

Introduction. How many public health and healthcare organization professionals a health facility has on staff has a significant impact on the integration of all functional units, the ability to successfully address pressing issues, and the quality and safety of medical care. Purpose. The goal was to conduct a comparative analysis of staffing dynamics in health facilities with public health specialists in Moscow and Russia from 2017 to 2021. Materials and methods. Statistical, logical, and analytical methods were used. Data on the number of public health specialists in Moscow and Russia, workforce density and quantity of health facilities were obtained from statistical data of the Central Research Institute of Organization and Informatization of Healthcare of the Ministry of Health of the Russian Federation from 2017 to 2021. The demand for chief physicians, deputy chief physicians, methodologists, and medical statisticians was identified based on job vacancies on job search websites (HeadHunter (hh.ru), SuperJob.ru, zarplata.ru, careerist.ru) at the beginning of 2023. Result. It was shown that there remained a negative trend in Moscow, Russia, between 2017 and 2021 in the number of heads of medical organizations and their deputies. Conversely, over the five-year period the number of methodologists have increased, and the number of medical statisticians remained virtually the same. There are open positions for experts in healthcare organization and public health in the Moscow healthcare system, despite the established network of academic and educational institutions for the training of medical specialists.

Syndromic Diagnostics of Respiratory Infections in Children As One of Antibiotic Management Tools in a Hospital 儿童呼吸道感染的综合诊断是医院抗生素管理工具之一

An observational, ambispective, non-interventional study was conducted to compare the effectiveness of introducing syndromic diagnostic according to the RP-2021 protocol on the basis of the pediatric departments of the Morozov Children's City Clinical Hospital. The study was carried out in two phases. The prospective phase (February–June 2022) was conducted using multiplex PCR tests (mPCR) – BioFire FilmArray respiratory panel – and routine diagnostic tests according to accepted standards for patient examination. The second phase of the study was a retrospective analysis of the comparison group’s (historical control group) primary medical records for standard test results from the same time frame, February to June 2021. Data on 40 patients on the historical control group were taken from the CIS EMIAS system. The mPCR results of the prospective group were available to the doctor several hours later, at the very beginning of the diagnostic algorithm. The length of the hospital stay in the prospective period following the introduction of mPCR testing was reduced by one day compared to the period before the introduction of this technology. The study showed that express methods of syndromic diagnostic testing allow to quickly establish the etiology of a respiratory infection for preterm infants, as well as children with bronchopulmonary dysplasia, bronchiolitis, comorbid patients, patients in the ICU with suspected influenza, whooping cough, and atypical pathogens, which in turn optimizes antibiotics prescription and hospitalization time. This study highlights the importance of rapid diagnosis of respiratory pathogens in pediatric patients under the age of five in order to optimize patient management when admitting them to the emergency department with a suspected acute respiratory infection. In the prospective observation group after the introduction of mPCR testing, patients were significantly less likely to receive antibiotics — 47.5% (n=19) compared to the historical control group of 72.5% (n=29). The study presents the advantages of the syndromic approach to the diagnosis of respiratory infections in children as one of the most effective tools for increasing the rationality of antibiotic use in a children's hospital.

Incidence Among Infants in Moscow Including the COVID-19 Statistics 包括 COVID-19 统计数字在内的莫斯科婴儿发病率

The study presents the data on incidence among infants in Moscow. The author analyzed the indicators of overall incidence, the characteristics of disease distribution, and the level of follow-up care in children of the given age group. The data was obtained from the open sources of Federal State Statistics Service of the Russian Federation (Rosstat) and from the Federal Statistical Observation Form No. 12 “On the number of diseases registered in patients residing in the service area of a medical organization” from 2016 to 2022. The author evaluated the impact of COVID-19 pandemic on the pre-COVID incidence trends in general and by specific disease categories, as well as on the distribution of diseases common to infants. The results show a decrease in overall incidence and specifically of infectious diseases, respiratory diseases, injuries, poisonings, and other external causes in all age groups. While overall incidence was decreasing between 2020 and 2022, the number of cases requiring the follow-up care was constantly growing. The absence of herd immunity and newborn vaccination due to the age characteristics of the group has led to an explosion of COVID-19 cases in the last three years. The study also revealed the variation in indicators for certain diseases, which warrants further investigation of the dynamics in these nosological groups.

The Specifics of Well-Being, Activity and Mood as Components of the Emotional Health of Students 作为学生情感健康组成部分的幸福感、活动和情绪的具体内容

Background. The professional training of students has dramatically changed over the COVID-19 pandemic with new conditions of education: transition to online learning, partial isolation, and loss of some means of communication. In addition, there are challenges in recovering mental, psychological, physical, and physiological health of students in the post-COVID period. The result is that the emotions and volition of young students have significantly transformed and influenced well-being, activity, and mood as components of students’ emotional health. Purpose. To define the specifics of well-being, activity, and mood as components of the emotional health of young students in the COVID and post-COVID periods. Methods and materials. 645 major students from Moscow universities were included on a voluntary basis in a sample. The authors applied five original methods for identifying the interrelation between personal problems’ impact on health status, health status itself, health self-assessment, well-being, emotional health, and body intoxication. Results. In the COVID-19 period, there were observed a high correlation between the average level of activity and the average level of health status and body intoxication, a moderate correlation between the average level of activity and the below-average level of personal problems’ impact on health status, health self-assessment, well-being, mood and emotional health, a moderate correlation between the average level of health status and the below-average level of health self-assessment, mood and emotional health, and a moderate correlation between the average level of body intoxication and the below-average level of well-being, mood and emotional health. In the post-COVID period, there were observed a high correlation between the average level of well-being, activity and mood and the average level of personal problems’ impact on health status, health status, health self-assessment, emotional health and body intoxication.

In the COVID-19 period, there were observed a high correlation between the average level of activity and the average level of health status and body intoxication, a moderate correlation between the average level of activity and the below-average level of personal problems’ impact on health status, health self-assessment, well-being, mood and emotional health, a moderate correlation between the average level of health status and the below-average level of health self-assessment, mood and emotional health, and a moderate correlation between the average level of body intoxication and the below-average level of well-being, mood and emotional health. Tools for the Development of Regional Medical Tourism Programs: International Experience 区域医疗旅游项目发展的工具:国际经验

Background. Internationalization of global healthcare and, in particular, the development of international medical tourism, informs the creation of a strategy for the development of domestic medical tourism programs in regions, states, or provinces using various tools for each stage of market development. Purpose. This article aims to identify, classify, and analyze the development tools for regional medical tourism programs applied in the countries recognized as international destinations for medical tourism. Methods and materials. The study data are based on foreign and domestic scientific publications in the field of international medical tourism and the export of medical services. The authors analyzed publications on the development of regional medical tourism programs in the media and performed content analysis of scientific publications in peer-reviewed scientific journals. Results. The internationalization of global healthcare has a significant influence on cross-border private medical travel (medical tourism), thus encouraging the development of both national and regional healthcare. The existing global practice of 19 countries in regional medical tourism provides an extensive development toolkit that includes more than 70 tools focusing on various aspects of the elaboration and implementation of opportunities for regional medical tourism and the export of medical services. The tools can be implemented in one of the four stages of market development: introduction, growth, maturity, and decline. The first stage is characterized by the prevalence of tools related to analysis and the development of the market base. The growth stage focuses on market expansion. At the third stage, the first tools aimed at offering better price for medical services appear. The stage of market decline is focused on the expansion of the target consumers by increasing the financial accessibility of medical tours. At the same time, the third and fourth stages are characterized by a limited number of support tools, which can be explained by the relative immaturity of the global medical tourism market and the focus on programs designed for the international market rather than on increasing economic efficiency. Therefore, in the following years, the scientific and professional communities will focus on the development and implementation of tools that can increase the reflow of financial resources expended on market development. The results of this study can be useful to the subjects of the Russian Federation that are interested in the development of regional strategies in terms of the federal project on the development of the export of medical services as part of the “Healthcare” national project.

Priority Development Subject Areas of Global Medical Science 全球医学科学优先发展学科领域

Introduction. One of the tools that allows to assess the relevance of initiated or ongoing scientific studies is the scientometric analysis of their topics. The method is actively and widely used not only for rating models based on various bibliometric indicators and parameters, as well as for evaluating the publication activity of authors, organizations, scientific publications, cities, and countries. Russia and other countries have amassed extensive experience rating accomplishments in scientific and educational fields, specifically using numerous scientometric indicators to assess scientific effectiveness and productivity. The article compiles a scientometric rating of the subject areas covered by “Medicine” in order to identify the priority development directions of global medical science. Materials and methods. The study was conducted in the “Medicine” section using the SciVal analytical platform, which uses data from the international scientific citation database Scopus. Four scientometric indicators (scholarly output, citation count, citation per output and field-weighted citation impact) were analyzed for a three-year period from 2019 to 2021. Results and discussion. The paper presents a table method for determining the worldwide priority directions of scientific research in medicine. For each of the four scientometric indicators, the TOP-5 subject areas have been identified, the list of which is approximately the same in terms of scholarly output and citation count, but significantly differs in citation per output and field-weighted citation impact. Conclusion. The TOP-5 of four scientometric indicators for the period 2019–2021 includes twelve subject areas, such as cardiology and cardiovascular medicine, oncology, surgery, infectious diseases, epidemiology, medical microbiology, public health, etc. The data obtained during the study confirm the strong thematic correlation between publications in certain subject areas. The algorithm described in the paper can be further adopted on local, organizational, regional, and national levels, i. e. in organizations, cities, and countries. This methodology seems to be promising in identifying and analyzing priority areas of scientific research and determining the index of scientific specialization of certain groups of countries, for example, BRICS, G7, G20, EU, ASEAN, Shanghai Cooperation Organization, Eurasian Economic Union, etc.

Reasons for Using Electronic Nicotine Delivery Systems by Young People 青少年使用电子尼古丁传送系统的原因

Research shows that there is an increase in the number of young people who have never smoked before, but now use electronic nicotine delivery systems (ENDS). Therefore, it seems relevant to study the reasons for using electronic cigarettes. A theoretical analysis of the problem allowed us to describe the main reasons for using electronic systems. An empirical study involving students of the Medical Institute of the Patrice Lumumba Peoples' Friendship University revealed the reasons among future health workers. The main reason for starting to use ENDS among the respondents was stress. 58.3% of participants noted that it was the reason to start smoking e-cigarettes. Other reasons included smokers in the social circle; attempts to quit smoking regular cigarettes; a desire to copy adults and attempts to protect others from secondhand smoke exposure.

The article discusses potential applications of a risk-based approach to health facility management. The study analyzed various management models, as well as process-oriented and project-oriented management systems, the use of risk management, quality management standards, lean technologies (lean manufacturing), and scorecard interfaces (score ratings). The article presents the results of applying risk analysis, which was developed using balanced scorecard approach that describes activities of a health facility. It was found that the balanced scorecard approach and scoring calculations enable risk level assessment in a health facility to classify risks by work areas and take suitable measures to mitigate or prevent them.

Modern Models of Polyclinic Management: Literature Review 综合医院管理的现代模式:文献综述

Introduction. The system of primary health care (PHC) has drastically evolved in the last few years due to the novel challenges of the COVID-19 pandemic. The global community has developed new approaches for using modern technologies to quickly make decisions and promote primary prevention, new software tools and applications for patients, general practitioners, and doctors in training, and new roles for PHC team members to support general practitioners and deliver health care to a larger part of the population. The growing population’s needs should be met through the efficient organization of a primary care-oriented system. In order to have timely, high-quality medical care, national authorities and healthcare organizations providing primary care are engaged in PHC management. Materials and methods. The review was prepared using reporting guidelines for systematic reviews and meta-analyses (PRISMA). The search was conducted in the bibliographic databases eLibrary, PubMed, MEDLINE, Google Scholar for the following keywords: functional-target operating model, primary health care, management in primary health care, management of polyclinics in the countries of the world, lean polyclinic, and medical prevention. In addition, authors analyzed and structured the information on Russian and foreign regulations and guidelines from the official websites of WHO, the Ministry of Healthcare of the Russian Federation, etc. Results and discussion. The World Healthcare Organization developed the target operational framework for PHC, including the main characteristics of models for healthcare organization and primary care provision. The model of medical care organization is adapting to the changing goals and priorities of the global community in healthcare. Conclusion. The current tendencies in PHC organization are prevention, digitalization, resource optimization, teamwork, and the transfer of functional responsibilities to non-medical specialists. Each country chooses its own model, system, and program of PHC provision depending on the population’s needs for medical services. Many of the developed models have already proven their efficiency.

The World Healthcare Organization developed the target operational framework for PHC, including the main characteristics of models for healthcare organization and primary care provision. The model of medical care organization is adapting to the changing goals and priorities of the global community in healthcare. \u0000Conclusion. The current tendencies in PHC organization are prevention, digitalization, resource optimization, teamwork, and the transfer of functional responsibilities to non-medical specialists. \u0000Each country chooses its own model, system, and program of PHC provision depending on the population’s needs for medical services.
