Interleukin 1β Up-Regulates mRNA Expression of Inducible Nitric Oxide Synthase in 3T3-L1 Preadipocytes: Role of JAKs/STATs, PKCs, and Src 白细胞介素1β上调3T3-L1前脂肪细胞诱导型一氧化氮合酶mRNA表达:JAKs/STATs、PKCs和Src的作用

Recent evidence suggests obesity as a low or systemic chronic inflammation. (Pre) adipocytes in the adipose tissue (AT) express and secrete a variety of cytokines and adipokines. Inducible nitric oxide synthase (iNOS) is an inflammatory enzyme involved in the production of NO. Until now, the inducer(s) of iNOS expression in (pre)adipocytes remains unclear. In this study, we investigated the effects of proinflammatory cytokines [interleukin-1β (IL-1β), IL-10, IL-12, interferon-γ (IFN-γ)], adipokines [retinol-binding protein 4 (RBP4), adiponectin, leptin, and resistin], and lipopolysaccharide (LPS), a bacterial cell wall component, on the expression of iNOS in 3T3-L1 preadipocytes. Notably, treatment with IL-1β at 20 ng/mL for 4 h markedly increased iNOS mRNA expression in 3T3-L1 preadipocytes, but that with IL10 (10 ng/mL), IL-12 (5 ng/mL), IFN-γ (10 ng/mL), RBP4 (5 ug/mL), adiponectin (100 ng/mL), leptin (100 ng/mL), and resistin (100 ng/mL), and LPS (1 ug/mL) for 4 h had little or no effect on it. Results of dose-response and time-course experiments confirmed the ability of IL-1β at 20 ng/mL for 4 h to maximally induce iNOS mRNA expression in 3T3-L1 preadipocytes. Importantly, pharmacological inhibition studies demonstrated that treatment with AG490 [an inhibitor of Janus-activated kinases (JAKs) and signal transducer and activator of transcription proteins (STATs)], GO6976 (an inhibitor of PKCs), or PP1 (an Src kinase inhibitor) suppressed IL-1βinduced iNOS mRNA expression in 3T3-L1 preadipocytes, pointing out the involvement of JAKs/STATs, PKCs, and Src in the process. This work advocates that IL-1β is a major and strong inducer of iNOS expression in 3T3-L1 preadipocytes.

Treatment of Low Back Pain and Sciatica by Local Anesthetic Injection through Epidural Catheter in Pregnant Woman 经硬膜外导管注射局麻药治疗孕妇腰痛及坐骨神经痛

Lower back pain in pregnant women is common and 30% of them require bed rest as a result [1]. Meanwhile, sciatica associated intractable pain occurs in approximately 1:10,000 pregnant women [1]. The occurrence of lower back pain and its severity are affected by hormone changes, weight gain, shift in center of gravity leading to disc bulging or herniation and relaxin levels, which contribute to the sliding mobility of the sacroiliac joints, widening of the pubic symphysis, and laxity of the posterior longitudinal ligament [2]. Conservative therapy, such as stretching, acupuncture, and core training, should be tried as first line interventions and analgesics, such as nonsteroidal anti-inflammatory drugs, may be recommended following the risk classification guidelines for drug use in pregnancy. In non-pregnant patients, disc length is a predictor for surgical treatment or nerve block success [3]. However, there are no definitive guidelines for nerve block and drug combinations in pregnant women with sciatica or lower back pain. This case report aimed to investigate the efficacy of epidural catheter insertion and intermittent local analgesic administration in a pregnant woman, presenting with severe, acute lower back pain and sciatica.

Understandings and Prospects of Laboratory Diagnosis of SARS-CoV-2 SARS-CoV-2实验室诊断的认识与展望

In 2019, a novel betacoronavirus, Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2), was detected in the Chinese city of Wuhan. SARS-CoV-2 caused a severe form of pneumonias which is closely related to SARS-CoV-1. Detection of viral RNA by real-time reverse transcription polymerase reaction (real-time RT-PCR) is crucial for the management of the patients and prevention of the SARS-CoV-2. Until now real-time RT-PCR methods have been used as a successful diagnostic tool in this epidemic. Furthrmore, more rapid and reliable diagnostic tool might be needed for the next epidemic.

Analysis of Job Interest and Job Value in the Freshmen of a Medical School 某医学院新生工作兴趣与工作价值分析

This study was conducted to study the job interest and job value of medical freshmen. The job interest test and job value test by the Korea Employment Information Service of the Ministry of Employment and Labor were conducted on 149 medical freshmen enrolled in a medical school at 2019 and 2020. Among six types of job interest, investigative type was most suitable for the medical freshmen, followed by realistic type, enterprising type, social type. Among 13 types of job value, job stability, body and mind space, recognition, and financial compensation were higher values than the others in order. We expect that this result will be helpful to establish the career identity of medical students, and to construct the system of curriculum or counseling programs for career developing of them.

Rocuronium Induced Masseter Muscle Rigidity during Intubation in Patients with Dental Surgery 罗库溴铵对牙科手术患者插管时咬肌强直的影响

Masseter muscle rigidity (MMR), known as the ‘jaw of steel’, sometimes makes laryngoscope unusable due to the stiffness of the jaw. MMR during general anesthesia was previously considered as an early episode of malignant hyperthermia (MH). Generally, MMR occurs in the pediatric patients after induction with succinylcholine and volatile agents. However, MMR following the administration of non-depolarizing muscle relaxants is uncommon, although described in some reports. We report a case of 62-year-old male patient which developed MMR following intravenous administration of rocuronium during induction of anesthesia. Our patient was not found to be vulnerable to MH. Although fiberoptic nasotracheal intubation had been successfully performed in our case, an unanticipated difficult airway due to MMR can be experienced at any time. Therefore, we should be aware that, although rare, MMR can occur after using non-depolarizing muscle relaxant. Additionally, in case of any incident of MMR, anesthesiologists need to recognize the management of a difficult airway and to maintain adequate oxygenation in a variety of methods according to a difficult airway algorithm.
