A Case Report on Parkinson’s Disease Soyangin Patient with Abdominal Pain and Constipation Jinyoung Kwak*, Yoon-Ha Kim*, Jung-Hwan Park*, Seo-jin Hong†, Taek-Won Ahn* *Dept. of Sasang Constitutional Medicine, College of Oriental Medicine, Daejeon University †Cheonan Oriental Hospital of Daejeon University Objectives The purpose of this case was to report significant improvement of abdominal pain, constipation and a slight improvement of tremor after treatment by Sasang Constitutional Medicine to the patient with Parkinson’s disease. Methods The patient was diagnosed as Soyangin according to the result of Sasang constitutional diagnosis, and treated by Sasang constitutional medications and acupuncture. The progress was evaluated with the Global Assessment Scale (GAS). Results The abdominal pain was reduced from Numeric Rating Scale (NRS)9 to NRS1 and headache was reduced from NRS5 to NRS3. The tremor was reduced to GAS 10. Unified Parkinson Disease Rating Scale (UPDRS) was reduced from 40 to 27. The difference of heat distribution between left and right side was reduced and the abdomen was more warmer on Digital Infrared Thermal Imaging (DITI). Conclusions Sasang constitutional medicine can be a solution for the non motor symptoms caused by Parkinson’s disease.

The purpose of this study was to validate the biopsychological structure of SPQ in middle school students. Methods 681 Korean high school students (356 boys and 325 girls) completed SPQ, Junior version of Temperament and Character Inventory (JTCI), and height and weight measures. The correlation between SPQ and JTCI subscales were examined, and the differences of SPQ and JTCI subscales, Body Mass Index (BMI) and Pondera Index (PI) among high (30%), middle (40%), and low (30%) SPQ total score groups were investigated with Analysis of Variance (ANOVA). The profile analysis was also performed to compare JTCI subscale profiles of three SPQ total score groups. Results The SPQ score was significantly (p<0.001) correlated positively with JTCI Novelty-Seeking and negatively with JTCI Harm-Avoidance. The JTCI Novelty-Seeking score of high SPQ group was significantly (p<0.001) higher than that of low SPQ group, and the JTCI Harm-Avoidance score of low SPQ group was significantly higher than that of high SPQ group. The JTCI subscale profiles for three SPQ groups were significantly different for boys and girls. Significant correlations or differences for the PI and BMI among SPQ groups were not found. Conclusions This study presented that the biopsychological structure of SPQ is robust as shown in adults. The SPQ would be a useful clinical measures of Sasang typology in pediatric patients.

The Sasang Constitutional Approach for Health Preservation of Cancer Survivors Objectives There is growing interest in the management of cancer survivors due to the increase in long-term survivors. But no literature research focusing on the management of cancer survivors has been done in Sasang Constitutional Medicine despite the superiority in comprehensive management of patients. Therefore, we planned to look into the perspective of health preservation in Sasang Constitutional Medicine. Methods We investigated the Donguisusebowon-Sasangchobongwon( 東醫壽世保元 四象草本卷 ) . We classified the contents which might be applicable to cancer patients. After classifying, we compared the contents with clinical experience and health preservation methods of Western Medicine. there is a difference in the prognosis and management. Cancer patients can be regarded as being in the stages of Noeok( 牢獄 ) and Wiegyeong( 危傾 ). In these stages, patient’s health preservation is important. In order to achieve long-term survival and to prevent disease progression, it is important to maintain requisite energy( 保命之主 ). The attitudes that patient must have are simplicity( 簡約 ), attentiveness( 勤幹 ), vigilance( 警戒 ), knowledgeability( 聞見 ) for keeping one’s own health and fulfilling the social obligations of human relations. The concept of health preservation in Sasang Constitutional Medicine covers the wide range of health. But detailed methods for patients to understand and practice are lacking. Moreover, it is hard for cancer patients to access the information. Therefore, further researches should be done to make the concept more specific and easy to access for cancer patients.

In this study, we report significant improvements of anorexia, dyspepsia, fatigue and weight loss after treatment with Hyangsayangyi-tang, Insambaekhaogwanjung-tang and acupuncture. Methods The patient was identified as Greater-Yin accompanied Jaundice pattern and treated with Hyangsayangyi-tang, Insambaekhaogwanjung-tang and acupuncture. The Global Assessment Scale (GAS) was used to assess the improvements of anorexia, dyspepsia, fatigue and pelvic pain after the treatment. And the liver function test was performed to measure the overall function of the liver. Result and Conclusion The symptoms of anorexia, dyspepsia, fatigue and pelvic pain were improved from GAS 100 to GAS 0-10 for four months. And The liver function test showed improved the response to treatment. The levels of AST (aspartate aminotransferase)/ALT (alanine aminotransferase) decreased from 43/42 to 29/14 IU/L. The level of total bilirubin was decreased from 2.7 to 1.9 mg/dL. Furthermore total weight gain was 8 kg from starting weight of 36.5 kg for four months. In conclusion, this study shows that Sasang constitutional medicine can be effective and safe treatment for the Soeumin patient with jaundice.

Methods SDQ_S was developed through discussions of three sasang constitutional medical specialist. We recruited 210 subjects aged from 20 to 70, and conducted the evaluation of SDQ_S and sasang constitutional diagnosis. For the reliability test, internal consistency was examined by calculating the cronbach’s α coefficient on 88 items excepted measurements of trunk, and independence and agreement was examined by chi-square test and McNemar test for 84 subjects which retested SDQ_S. For the validity test, discriminant functions was derived from the sample group(105 subjects), and they were applied to the test group(105 subjects). We identified the hit-ratio of SDQ_S in the test group. Results and Conclusions In the reliability test, the cronbach’s α coefficient was 0.664 and 87 items(98.9%) have dependent relationships between 1st test and 2nd test. And 39 items(44.3%) have high agreements(0.8≤McNemar), 26 items(29.5%) have common agreements(0.4≤McNemar<0.8). In the validity test, four discriminant variables(Soeum Index, Taeeum Index, Torso Ratio I, Taeyang Index) were selected, and two discriminant functions were derived. Total hit-ratio was 74.2%, and hit-ratio of Taeeumin(76.7%) was higher than that of Soeumin(75.9%) and Soyangin(69.7%). SDQ_S has good reliability and validity for sasang constitutional diagnosis.

Comparison of Effects of Yangkyuksanhwa-tang, Palmulgunja-tang and Cheongpyesagan-tang on the Rat Hyperthyroidism Induced by Levothyroxine Objectives This study‘s object was to observe the comparative effects of Yangkyuksanhwa-tang , Palmulgunja-tang and Cheongpyesagan-tang on the chronic LT4(levothyroxine) induced hyperthyroidism in rats. Methods Six groups, each of 8 rats in group, were used in this study. Saline and distilled water treated rats are intact control group. Hyperthyroidism was induced by daily subcutaneous LT4 300 μ g/kg treatment for 27 days(LT4 control). Since 12th LT4 treatment PTU(propylthiouracil) 10 mg/kg was intraperitoneal injected(PTU group) and aqueous extracts of Yangkyuksanhwa-tang , Palmulgunja-tang and Cheongpyesagan-tang (YS, PG and CS) 500 mg/kg were orally administrated(YS, PG, CS group), once a day for 15 days. The differences in the body, thyroid gland and epididymal fat pad weights, serum T3(tri-iodothyronine), T4(thyroxine), TSH(thyroid-stimulating hormone), thyroid gland and epididymal fat pad histopathology, liver weight, AST(asparte aminotransferase), ALT(alanine aminotransferase) concentrations, hepatic lipid peroxidation, GSH(glutathione), SOD(superoxide dismutase), CAT(catalase) activities, liver histopathology were observed to evaluate effects on hyperthyroidism, liver damages and antioxidant effects. suggest YS, and favorably control the LT4 induced hyperthyroidism and related liver in through modulation of the

Methods Six groups, each of 8 rats in group, were used in this study. Saline and distilled water treated rats are intact control group. Hyperthyroidism was induced by daily subcutaneous LT4 300 μ g/kg treatment for 27 days(LT4 control). Since 12th LT4 treatment PTU(propylthiouracil) 10 mg/kg was intraperitoneal injected(PTU group) and aqueous extracts of Yangkyuksanhwa-tang , Palmulgunja-tang and Cheongpyesagan-tang (YS, PG and CS) 500 mg/kg were orally administrated(YS, PG, CS group), once a day for 15 days. A Case report of Soyangin patient University Objectives The purpose of this case study is to report significant improvement of headache, constipation and a slight improvement of tremor after treatment with Hyeongbangjihwhang-tang in a Soyangin patient with Parkinson's disease. This Soyangin patient is treated by Hyeongbangjihwhang-tang and acupuncture therapy. The progress was evaluated using the Unified Parkinson Disease Rating Scale before and after treatment. Secondary results assessments include Global Assessment Scale, change of patient's omniloquilism, Xerostomia and Cold hands & Feet Syndrome. conclusion, study shows that Hyeongbangjihwhang-tang can be effective for Soyangin patient with early-stage Parkinson’'s disease. Overall, Sasang medicine can be a solution of the symptoms caused by Parkinson's disease. Through Sasang medicine we can contribute Parkinson's disease Patient's quality of life get better.

A Case Study of a Taeeumin Patient with Advanced Parkinson’s Disease Having Orthostatic Hypotension Mi-Suk Lee*, Yu-Gyeong Park*, Na-Young Bae† *Dept. of Sasang Constitutional Medicine, Pusan National University Korean Medicine Hospital †Dept. of Sasang Constitutional Medicine, Div. of Clinical Medicine, School of Korean Medicine, Pusan National University Objectives This case study is about a Taeeumin patient with advanced Parkinson’s disease identified as Dry-heat (Joyeol) pattern. In this study, we report significant improvement of non-motor and motor symptoms after treatment with Cheongsimyeonja-tang. Methods The patient was identified as Taeeumin Dry-heat (Joyeol) pattern and treated with Cheongsimyeonja-tang and acupuncture. The Unified Parkinson Disease Rating Scale (UPDRS) was used to assess the overall functions of the patient. And the global assessment scale (GAS) was used to assess the improvements of dizziness, bradykinesia and tremor after the treatment. Result and Conclusion The UPDRS total score decreased from 138 points to 86 points after 5 weeks treatment. And symptoms of dizziness caused by hypotension, bradykinesia, and tremor showed significant improvement in GAS after the treatment. Furthermore, sleep disturbance and constipation were reported to be improved after the treatment. In conclusion, this study shows that Sasang constitutional medicine can be effective treatment for Taeeumin patient with advanced Parkinson’s disease having orthostatic hypotension.

A case Report of Tinnitus after Subarachnoid Hemorrhage(SAH) Operation Diagnosed as Soyangin Hyunggyeok yeoljeung Tae-Hyung Lim*, Hye-Sun Park Dept. of Sasang Constitutional Medicine, Graduate School of Korean Medicine, Dongshin University Objectives This case study reports patient diagnosed as Soyangin Hyunggyeok yeoljeung with tinnitus, headache and sleep discomfort after Subarachnoid Hemorrhage(SAH) had treated with HyungBangSabaek-San. Methods The patient suffering from tinnitus, sleep discomfort had taken HyungBangSabaek-San from the first to the end three times a day. We measure tinnitus by Tinnitus Handicap Inventory(THI) and observe symptoms change using Visual Anlaogue Scale(VAS). Results The symptoms had been steadily decreased and quality of life had increased also. Headache and constipation had been improved in a week and tinnitus and chest discomfort had decreased next week. At last, patient complained symptoms slightly. Conclusions HyungBangSabaek-San a is treatment of Soyangin Hyunggyeok yeoljeung and may be considered as a treatment of tinnitus.

The objective of this study is to develop a taeeumin animal-experimental model induced lung fibrosis with Bleomycin and evaluate the effect on obesity in this animal-experimental model. The subjects were divided into 3 groups : normal group, high fat diet(HFD) control group, and HFD group administered with bleomycin(n=10 per group). To develop taeeumin animal-experimental model with reduced respiratory metabolism, 8-week-old C57BL/6 mice were administered with 0.03ml solution of bleomycin 1U/ml dissolved in distilled water, intratracheal(IT), once. Then, the HFD control group and the experimental group were fed with high fat diet for 6 weeks. Airway hyperresponsiveness(AHR) to methacholine was measured at the 1st and 3rd week after bleomycin was administered. Food intake and body weight were measured at regular time weekly. After the final experiment, blood was gathered by cardiac puncture for bloodchemical examination and organs(liver, fatty tissue) were remoed, weighted, and mRNA was analyzed. the gene expression increased and energy metabolizing gene mRNA of decreased, it was found that Taeeuim animal-experimental model is susceptible to metabolic syndrome and cardiovascular diseases.
