Glycemic control of diabetes dialysis patients and ‘burnt-out diabetes’ 糖尿病透析患者的血糖控制与“燃烧型糖尿病”

「血液透析患者の糖尿病治療ガイド2012」に基づき,わが国ではグリコアルブミン(glycated albumin:GA)を用いて糖尿病透析患者の血糖コントロールを評価している.心血管イベントの既往歴のない症例では暫定的な目標値としてGA値20.0%未満が推奨された.心血管イベントの既往歴を有する症例や低血糖傾向のある症例では24.0%未満が提案されている.近年,わが国から透析患者のGA値と生命予後に関する臨床成績もいくつか報告されてきた.また,透析導入に伴い,血糖値やHbA1c値が正常化し,血糖降下薬が不要となる現象があり,“Burnt-out diabetes”と呼称されている.糖尿病透析患者においてはBurnt-out diabetes,とくにHbA1c<5.0%と低値であることが予後不良であると報告されているが,その頻度や予後はHbA1cのみならずGAと組み合わせて評価することが重要である.
根据《血液透析患者糖尿病治疗指南2012》,我国使用糖白蛋白(glycated albumin, GA)评估糖尿病透析患者的血糖控制情况。对于既往没有心血管事件的病例,建议GA值小于20.0%作为暂定目标值。在有心血管事件病史的病例和低血糖倾向的病例中建议的比例小于24.0%。近年来,我国也报告了一些有关透析患者GA值和生命预后的临床成绩。另外,随着导入透析,血糖值和糖化血红蛋白值趋于正常,存在不需要降糖药的现象,被称为“burint -out diabetes”。在糖尿病透析患者中,Burnt-out diabetes,特别是糖化血红蛋白< 5.0%的低值被报告为预后不良,其频度和预后不仅要结合HbA1c,还要结合GA进行评估。
