Summary of the annual meeting of the Okinawa Dialysis and Transplant Association (ODTA40) 冲绳透析与移植协会年会纪要

第1回沖縄県人工透析研究会は1983年に開催,2023年で第4回大会を迎えた.2020年2月に始まった新型コロナ感染症の蔓延により2020年は休会を余儀なくされたが,2021年より種々難題を抱えながら再開している.当初は一般演題,特別講演のみであったが透析施設数が増加するにつれて現況報告,腎移植・CAPD分科会報告が加わっている.大会長の提案でワークショップ,シンポジウム,特別企画も加わっている.一般演題は総数で計1,635題:医師492題(30.1%),技師415題(25.4%)および看護師728題(44.5%)である.2008年に発足した沖縄県透析医会とも協力してさまざまな調査や行政との連携も進めている.危機管理(台風,地震,水不足,停電,COVID-19)およびスタッフ不足・教育は喫緊の課題である.これまでの研究会プログラムを振り返り今後の研究会や透析医療についても考察する.
第1届冲绳县人工透析研究会于1983年召开,2023年将迎来第4届大会。由于新型冠状传染病从2020年2月开始蔓延,该组织不得不于2020年休会,但从2021年起,该组织将带着种种难题重新开始。最初只有一般演讲和特别演讲,但随着透析设施数量的增加,增加了现状报告和肾移植、CAPD分会报告。在大会长的提议下,还增加了研讨会、研讨会、特别企划。一般演讲题目总数为1,635题:医生492题(30.1%),技师415题(25.4%)以及护士728题(44.5%)。与2008年成立的冲绳县透析医会合作,推进各种各样的调查和行政协作。危机管理(台风,地震,缺水,停电,COVID-19)以及员工不足和教育是紧迫的课题。回顾迄今为止的研究会项目,对今后的研究会和透析医疗也进行考察。

標準的治療が無効であった複数の嫌気性菌によるPD関連腹膜炎の1例 一例PD相关腹膜炎,由多种厌氧菌引起,标准治疗无效。

66歳女性.Alport症候群を原疾患に入院3年前に腹膜透析(peritoneal dialysis:PD)を導入.入院当日,排液混濁および発熱を主訴に外来受診.好中球優位の排液細胞数増多を認めPD関連腹膜炎の診断にてセファゾリン(CEZ)+セフタジジム(CAZ)の腹腔内投与を開始.3日目,排液細胞数は減少せず排液混濁も改善は認めなかった.4日目よりメロペネムの腹腔内投与に変更.当初排液培養は陰性に思われたが,増菌によりBacteroides ovatusおよびEggerthella lentaが検出された.以後排液細胞数は減少に転じたが11日目に再燃.治療抵抗性のPD関連腹膜炎として14日目にカテーテル抜去,PD離脱に至った.内因性PD関連腹膜炎の原因となる腸内細菌には,第一選択となるCEZ+CAZに抵抗性のものも少なくない.中でも稀な起炎菌による腹膜炎を経験したので,文献的考察を交えて報告する.
66岁女性。因Alport症候群为原病,入院3年前导入腹膜透析(peritoneal dialysis, PD)。住院当日,以排液混浊及发热为主要症状门诊就诊。发现中性粒细胞占优势的排液细胞数增多,诊断为PD相关腹膜炎,开始腹腔内给予头孢唑林(CEZ) +头孢噻嗪(CAZ)。第3天,排液细胞数没有减少,排液混浊情况也没有改善。从第四天开始改为腹腔内给药。当初认为排液培养呈阴性,但通过增菌检测出Bacteroides ovatus及Eggerthella lenta。此后排液细胞数开始减少,但在第11天再次出现。作为治疗抵抗性PD相关腹膜炎,在第14天拔除导管,导致PD脱离。导致内源性PD相关腹膜炎的肠道细菌中,也有不少对第一选择CEZ + CAZ具有抵抗性。其中经历了罕见的由起炎菌引起的腹膜炎,所以结合文献考察作报告。
