Dr. Jong Hoon Kim 金钟勋博士

Dr. Jong Hoon Kim M.D. Ph.D. is an Assistant Professor in the Department of Dermatology and Cutaneous Biology Research Institute at Yonsei University in South Korea. He completed his Ph.D. and postdoctoral projects between 2013 and 2016, which focused on the study of T cell immunology. His initial research aimed to dissect the mechanism of T-cell activation in psoriasis. Through his work, he found that TCR-mediated activation of PD-1+IL-17A+ T cells contributes to disease activity in murine psoriasis, independent of IL-23-mediated activation (J Allergy Clin Immunol. 2016). This study provided insights into the mechanism of generating PD-1+ resident memory Tc17 cells observed in human psoriatic skin. He also conducted a study on human T cells by collecting liver perfusate during liver transplantation, where he identified specific features of liverresident CD69+CD103-CD8+ T cells under healthy and diseased conditions (J Hepatol. 2020).

A Case of Recessive Epidermolysis Bullosa with Long-term Survival 隐性大疱性表皮松解症长期存活1例

劣性栄養障害型表皮水疱症はⅦ型コラーゲンの遺伝子変異により発症し,生後から四肢体幹に水疱やびらんが出現し,瘢痕形成を繰り返す。予後因子として青年期以降から出現する有棘細胞癌や,慢性炎症による腎不全,食道びらんや狭窄による摂食障害,びらん部からの二次感染などが問題となり,全身状態の悪化を起こしやすい。症例は 82 歳,女性。幼少期より四肢のびらんを繰り返し,摂食障害が起こることも度々みられたが現在まで通院加療を行っている,当院最高齢の患者である。本症例は表皮水疱症の診断基準が確立する前から当院への通院を開始したが,現在までに良好な経過を維持できている理由として毎日の自己処置が行えていたこと,摂食不良時には経腸栄養剤を積極的に用いていたことや,小児期から体への負担の少ない生活スタイルだったことが挙げられた。しかし長年経過良好に過ごしていたが,歩行時痛から ADL が低下した際には急激な潰瘍の悪化,蜂窩織炎を引き起こしたことから,今後は高齢になっていく患者の生活状態に合わせ,早期に介護や訪問看護を導入するなどの対応も重要とされた。
劣性营养障碍型表皮水疱症Ⅶ型胶原蛋白的基因变异,导致发病,从出生开始四肢体干细胞出现水疱和糜烂,重复的疤痕形成。作为预后因素,从青年期以后开始出现带刺细胞癌、慢性炎症引起的肾功能不全、食道糜烂和狭窄引起的进食障碍、糜烂部的二次感染等成为问题,容易引起全身状态恶化。病例82岁,女性。从幼年时期开始四肢反复糜烂,也多次出现进食障碍,到目前为止一直在医院接受治疗,是该院年龄最大的患者。本病例在表皮大疱症的诊断基准确立之前就开始到本院就医,到目前为止病情维持良好的原因是每天进行了自我处理,进食不良时积极使用经肠营养剂;这是因为,从孩提时期开始就过着对身体负担较轻的生活。虽然多年来病情进展良好,但由于走路时疼痛到ADL下降时溃疡急剧恶化,引起蜂窝织炎,因此今后为了适应老年患者的生活状况,早期引入护理和上门护理等应对措施也很重要。

症例1: 75歳,男性。腰痛症に対してカルバマゼピン投与開始約3週間後より右大腿に紅斑が出現し,全身に拡大。発熱や口腔粘膜のびらんを伴い.左右鼠径リンパ節の腫脹もみられた。症例2: 53歳,男性。脳梗塞後遺症にてカルマバゼピン内服開始約5ヵ月後より顔面,陰嚢に紅斑が生じ,全身に拡大。発熱や全身倦怠感を伴い,両側の頚部リンパ節および鼠径リンパ節も触知した。2例ともヒトヘルペスウイルス6(似下HHV-6) 抗体価の上昇が認められた。
病例1:75岁,男。针对腰痛症,开始使用卡巴马西平约3周后,右大腿出现红斑,并扩散至全身。伴有发热和口腔粘膜糜烂。左右腹股沟淋巴结也出现了肿胀。病例2:53岁,男。因脑梗塞后遗症,口服卡马伐西平约5个月后,面部和阴囊出现红斑,并扩散到全身。伴有发热和全身倦怠感,两侧颈部淋巴结和腹股沟淋巴结也有触痛。两例病例均发现人类疱疹病毒6(类似HHV-6)抗体价值上升。
