Post-iodination trends in thyroid pathology; 14-year experience at a single center in a goitre endemic area 碘化后甲状腺病理趋势;在甲状腺肿流行地区的一个中心工作了14年

Introduction: Goitre is endemic in many parts of Sri Lanka. Universal iodination of salt was commenced in Sri Lanka in 1995 as a measure to reduce the prevalence. This study was designed to identify the pattern of thyroid pathology reported during a period of 14 years from 2002 to 2015 in a Pathology Unit serving the population in a goitre endemic area in Sri Lanka.Methods: This descriptive cross-sectional study included all thyroidectomy specimens histologically assessed at our unit from 2002 to 2015. Data on age, gender, clinical presentation, type of resection, and histopathological diagnosis were retrieved from the records. The trend of change in thyroid pathology over time was analysed by comparing 2002-2010 cohort (615specimens) with the 2011-2015 cohort (995 specimens).Results: This study included 1610 thyroidectomy specimens. Features of colloid goitre were present in 71.6% (1152/1610). Prevalence of chronic autoimmune thyroiditis and thyroid cancer was 41% (660/1610) and 15.3% (246/1610) respectively. There was no significant change in the gender and the age at presentation over the years. However, an upward trend in the prevalence of both focal and diffuse chronic autoimmune thyroiditis (p<0.001), hyperplastic nodules (p=0.008), papillary microcarcinoma (p<0.001), papillary carcinoma and colloid nodular goitre (p=0.004) was evident.Conclusions: Parallel increase in the prevalence of chronic autoimmune thyroiditis and papillary carcinoma together with the upward trend in the prevalence of colloid nodular goiter was evident.

Health-related quality of life and its predicting factors among male oral cancer survivors in Sri Lanka: A cross sectional study 斯里兰卡男性口腔癌幸存者的健康相关生活质量及其预测因素:一项横断面研究

Introduction: Health-related quality of life (QoL) and its predicting factors for oral cancer survivors are varied and little evidence is available in Sri Lanka. This descriptive cross-sectional study aimed to assess the QoL and its predicting factors of male oral cancer survivors.Methods: The study was conducted at Apeksha Hospital (National Cancer Institute, Sri Lanka) from June to December 2018 for a period of six months. QoL of 295 oral cancer survivors was assessed using the WHOQoL-BREF questionnaire. The range of QoL score is from 0 to 100 and higher the score, higher the level of QoL Socio-demographic and disease-specific characteristics were considered as predicting factors for QoL and Multiple linear regression was performed to determine the predicting factors. The significant level was kept as p<0.05.Results: The mean score of QoL was averaged among all domains of WHOQoL-BREF questionnaire, which was depicted as 51.9±15, 52.3±19, 49.5±22 and 59.2±16 in Physical, Psychological, Social Relationship and Environment domains respectively. The number of years from diagnosis, metastasis, cancer stage and cancer site were the predictors of the physical health domain while employment status, metastasis, cancer stage and duration of betel chewing for the psychological domain (p<0.05). Age, education status, monthly income, metastasis, and duration of alcohol consumption were identified as the significant predictors of social relationships while employment, monthly income, metastasis, and duration of betel chewing were predictors of the environment domain (p<0.05).Conclusions: The study revealed that QoL of all domains were around average, but each QoL domain has different predicting factors. Therefore, consideration of these predicting factors are immensely helpful to enhance the QoL of these survivors.

Knowledge, attitude and practices on insulin therapy among patients with type 1 diabetes 1型糖尿病患者胰岛素治疗的知识、态度与实践

Introduction: The incidence of type 1 diabetes mellitus (T1DM) is increasing worldwide. Daily administration of multi-dose or single-dose Insulin therapy is the cornerstone in the treatment of T1DM. The recent studies highlighted the gap in knowledge, attitude, and practice (KAP) towards insulin therapy for the optimal control of hyperglycemia T1DM. The study aimed to assess knowledge, attitudes, and practices regarding insulin therapy among T1DM patients attending the diabetic clinic, Teaching Hospital Karapitiya (THK).Methods: A cross-sectional descriptive study was conducted using an interviewer-administered questionnaire to elicit socio-demographic information and KAP towards insulin therapy. A total of 62 attending the diabetic clinic at THK were recruited.Results: The study population consisted of 91.9% early young adults (ages 18-25 years) while the remainder represented late young adults (ages 26 - 30 years). A good knowledge, attitude and practice were seen in 61.3%, 62.9% and 38.1% of the patients respectively. Age, level of education, duration of DM, regular clinic visits and favourable attitudes were associated with good knowledge whereas level of education, duration of DM, regular clinic visits and good knowledge were associated with favourable attitudes. Duration of DM and regular clinic visits were the only factors associated with good practices. Higher level of knowledge or higher level of education were not associated with good practices towards insulin therapy (p>0.05).Conclusions & recommendations: Though majority had good knowledge and attitudes, practices were unsatisfactory in the majority of them. Self-management education and training giving special attention to psychosocial aspects of living to improve self-care behaviour are recommended to address the gap between knowledge and practices.
