Medication-Related Osteonecrosis of the Jaw: A Reflection of Current Preventative and Therapeutic Guidelines: A Review 药物相关性颌骨骨坏死:当前预防和治疗指南的反映:综述

Medication-related osteonecrosis of the jaw (MRONJ) is a serious side effect of bisphosphonates, denosumab, and antiangiogenic drugs. Although the literature is rich with research work discussing MRONJ, the pathogenesis of this devastating condition has not been adequately understood yet. Roles of infection, immunity and genetic predisposition have been extremely unclear. Moreover, many controversies have been governing risk assessment and management guidelines. In 2003, 36 cases of osteonecrosis of the jaw (ONJ) associated with the use of either pamidronate or zoledronate were reported. Furthermore, in 2014, the American Association of Oral and Maxillofacial Surgeons (AAOMS) suggested the term “medication-related osteonecrosis of the jaw” (MRONJ) to describe jaw osteonecrosis related to the use of medications. This review sheds the light on recent research work discussing the pathogenesis of MRONJ. Moreover, it suggests possible guidelines for risk assessment and management based on information gathered from different research papers found in the literature.

Sex and Stature Estimation from Odontometric Parameters of Kalabari Ethnic Group in Rivers State, Nigeria 从尼日利亚河流州卡拉巴里族群的牙齿测量参数估计性别和身高

The resistance of the teeth to human and taphonomic factors has made it a choice for the identification of persons. The aim of this study is to estimate sex and stature from odontometric parameters of Kalabari ethnic group of Rivers State in Nigeria. The cross-sectional descriptive study involved 100 volunteers between the ages of 15 to 30 years (50 males and 50 females) whose parents and four grandparents were from Kalabri ethnic group. The parameters measured were the crown height (CH), maximum mesiodistal width (MD), buccolingual dimension (BL) of the right maxillary and mandibular incisors, canines, and premolars. The heights (HT) of the volunteers were also measured. Discriminant function analysis was used to design a predictive model for sex and Multivariate Regression Analysis was used to design a predictive model for stature. The results showed that the mean values of all teeth variables were larger in males than in females except buccolingual dimensions of second maxillary incisor (BL2) and crown height of first maxillary premolar (CH4). Discriminant Function Analysis of all measured parameters showed the score that predicts male is 1.067 while -1.067 for female. Stepwise Discriminant Function Analysis showed maxillary canine to be the best predictor of sex followed by mandibular incisor. Predictive model for sex identification was derived. Multivariate regression analysis was used to derive equation for stature estimation. This study showed that the combination of the teeth could be used for sex and stature estimation.

Does Maxillary Expansion Lead to Root Resorption?-A Comprehensive Review 上颌扩张会导致牙根吸收吗?-全面回顾

The objective of this review article is to assess root resorption of conventional rapid maxillary expansion (RME) and mini screw assisted rapid palatal expansion (MARPE) with 2D and 3D radiographic imaging method. For this review article, the current literature was searched on the studies conducted on the effects of such expansion appliances on root resorption. Orthodontic patients with transverse maxillary defieiceny undergo maxillary expansion therapy for the correction of posterior crossbite. The evalution of root resorption in such patients can be done with either 2D radiographs such as perioapical radiograph, panoramic radiograph or 3D radiographs such as cone beam computed tomography. This review article will discuss the studies showing that conventional RME appliances can result in root resorption of molar teeth after the expansion procedure. In contrast, MARPE appliances have not shown significant root resorption after expansion procedures.

The Effect of Volatile Oils on De-Bonding of Polycarbonate Bracket Reinforced with Ceramic Filler: A Quantitative Study (Part 1) 挥发油对陶瓷填料增强聚碳酸酯支架脱粘影响的定量研究(一)

Introduction: The introduction of esthetic brackets is a new clinical approach in the field of orthodontics. Two main types of esthetic brackets have been developed and became widely available; the polycarbonate brackets, and the ceramic brackets. Both types have faced two significant de-bonding problems: (1) enamel and bracket fracture. The two problems have been well documented and continue to be of concern to clinicians. Aim: The aim of this in vitro study was to evaluate quantitatively the effect of three volatile oils and their combinations on the de-bonding of ceramic brackets. Material and Method: Polycarbonate brackets reinforced with ceramic filler were bonded to one hundred and eighty (180) sound extracted human premolars using Transbond XT light curing adhesive. The teeth were then distributed into the following groups according to the type of volatile oil and period of application: clove oil (5 & 30 minutes), peppermint oil (5 & 30 minutes), Black seed volatile oil (5 & 30 minutes), and mixture of peppermint + Black seed volatile oils (5 & 30 minutes) in addition to control group. The brackets were then de-bonded using the Instron Universal Testing Machine and the de-bonding force was recorded for each type of the volatile oils in different times of application. Result: The result revealed that the Black seed volatile oil gave the lowest de-bonding force in 5 minutes and 30 minutes of application whereas the mixture of the Black seed oil + peppermint oils gave the highest force in both time of application. Further, the peppermint oil application for 5-minute was found to be in the second rank whereas the Clove oil was in the third rank. Furthermore, in the 30 minutes of application the Clove oil was in the second rank whereas the peppermint was found to be in the third rank and the combination of Black seed oil + peppermint oil shows the highest debonding force. Conclusion: Despite the limitation of the present study result, an assumption has been drawn from the results obtained that (1) the 5- minutes of black seed or peppermint volatile oils application can be considered the best debonding agent compared to the other volatile oils; (2) the clove oil can be used after de-bonding to facilitate the final cleansing-up of enamel surface. To confirm the latter assumption, further investigations are needed.

Maxillary Complete Denture Buttressed with Metal Denture Base and Amalgam Stops Opposing Deranged Occlusal Plane: A Case Report 金属义齿基托与银汞合金止点对牙合平面的上颌全口义齿1例报告

Fracture of a single maxillary complete denture is a common occurrence in patients with opposing retained natural mandibular teeth. Factors like flexural fatigue caused by cyclic deformation, as well as those that aggravate the deformation of the denture base or change its load, have all been linked to the midline fracture. This case report presents oral rehabilitation of a completely edentulous maxillary arch opposing natural mandibular dentition using metal denture base and reinforcing acrylic teeth using amalgam stops along with establishing plane of occlusion with the help of a customized template to improve the longevity of the prosthesis. Single complete denture opposing natural dentition is a challenging job for the prosthodontist due to the biomechanical differences in the supporting tissues. As a prosthodontist, we must provide the patient a stable and functional prosthesis.

Natal Teeth - Case Series and Management 出生牙齿-病例系列和管理

First year of child development is marked by many changes of which eruption of first tooth into the oral cavity is considered an important milestone. It has shown to follow a chronology and has been established in the literature corresponding to the date when tooth erupts into the oral cavity. Depending on hereditary, endocrine and environmental factors they may be subjected to variations. However, at times, chronology of tooth eruption suffers a more significant alteration in terms of onset, with the teeth being present at birth or arise during the first month of life poses greater concern due to feeding difficulties along with societal unpleasant reaction towards it. Objective of this paper is to present case series of natal and neonatal teeth with considerations in the management.

However, at times, chronology of tooth eruption suffers a more significant alteration in terms of onset, with the teeth being present at birth or arise during the first month of life poses greater concern due to feeding difficulties along with societal unpleasant reaction towards it. Retention of Central Incisors Caused by Four Supernumerary, Molariform Teeth in A Non-Syndromic Patient 1例非综合征患者多出4颗臼齿导致中门牙潴留

The presence of supernumerary teeth is one of the most common factors that can alter the normal eruption pattern. We present here a rare phenomenon of four, molariform mesiodentes in a systemically healthy, non-syndromic patient, with non-contributory family history, causing delayed eruption of permanent, upper, central incisors. Multiple supernumerary teeth can be an important component of distinctive disorders and a valuable clue for their early diagnosis. However, this case highlights the importance of non-hereditary factors in the pathogenesis of multiple supernumerary teeth in a non-syndromic patient.

Effects of Bionator Devices on Orofacial Muscle Strength in the Treatment of Class II Malocclusion in Developmental Phase: Systematic Review 发育期II类错颌畸形治疗中Bionator装置对口面肌力的影响:系统综述

Background: Class II malocclusion is a common condition. The most common clinical presentation is a mandibular deficiency. Treatment with functional devices is an option that can be used for the treatment of malocclusion. In an era of evolving treatments, Bionator is still widely used in clinical practice. Objective: The purpose of this systematic review was to analyze the effectiveness of the treatment of class II malocclusion with the Bionator device in the growth and development phase. Methods: Data collection was carried out by searching literature on article search sites, namely Pubmed, Cochrane, Wiley, Google Scholar and Science Direct which were published from 2011 to 2021. The search was carried out from March to April 2021. Data search was carried out systematically using the bionator keyword. malocclusion class II, orofacial muscles, growth and development. Results: After eliminating published articles, the title and abstract of each article in the analysis amounted to 132 articles, 106 articles were excluded. The full text articles from the exclusion results were re-analyzed and 16 articles were excluded. The text of the article is complete and meets the eligibility of as many as 10 articles. Conclusion: From all the studies reviewed, the Bionator device can improve class II malocclusion in children.

Background: In general, patients with Class II malocclusion are often associated with a narrow transverse dimension. The shape of the narrowed jaw space can cause various problems such as crowding, jaw mismatch, protrusion of incisors. Given this, the use of Rapid Maxillary Expansion or Rapid Palatal Expansion is a treatment option that has been the choice for decades. Objective: To systematically review the benefits of using RPE in the improvement of malocclusion in child development. This device is used to correct maxillary constriction and posterior crossbite. Methods: From four database sources PubMed, GoogleScholar, Wiley and Cochrane from 2016 to 2021. Thedatabase search process used the keywords "Rapid Palatal Expansion" "Rapid Maxillary Expansion" "Class II", from full journals published in less than 5years. Results: After eliminating duplicated articles, the abstract title of each article was analyzed from 136 articles which were excluded a total of 123 articles. Thetext of the article is complete and meets the eligibility as many as 13 articles. Conclusion: Rapid Palatal Expansion is effective in correcting class II malocclusion in child development.

Knowledge & Practice of Radiation Exposure Protection Protocols by Dentists Performing Endodontic Treatment in Pakistan 巴基斯坦牙医进行牙髓治疗的辐射暴露防护协议的知识和实践

Dental radiography is an essential diagnostic tool in endodontics and one of the indispensable indicators of success. However, the potentially harmful effects of radiation cannot be overlooked. The aim of this survey was to assess knowledge, attitude and practice of radiation hazards and exposure protection techniques of dentists in Pakistan. A cross-sectional study was conducted using an online survey questionnaire. The questionnaire was distributed among all regions of Pakistan through social media and WhatsApp after carrying out the reliability analysis. Statistical analysis was performed using SPSS 20.0. Question wise analysis using frequencies and percentages was done. Association between the level of awareness with qualification and workplace setting was carried out using chi-square test and logistic regression analysis. The minimum required sample size calculated for this study was 335. A total of 341 dentists [performing endodontic treatment] participated and submitted the form from all regions of Pakistan. 65% of the dentists were aware with the ALARA principle. 40% were aware with the ICRP and NCRP guidelines. Only 27% were using lead apron and 71.8% had never used thyroid collar on the patient. Furthermore, a positive correlation was seen between the designation and awareness level and practice [p=0.01]. Similarly, a significant correlation p=0.01 was seen between awareness level/practice and workplace setting. The results of the present study can help design continual education programs at regular intervals at institutional and national level for strict observance of radiation protection guidelines with more emphasis on general practitioners and government hospitals.
