Familial Aggregation Pattern of Non-Syndromic Combined Aplasia of Maxillary Lateral Incisors and Third Molars 上颌侧切牙和第三磨牙非综合征型合并发育不全的家族聚集模式

Congenitally missing teeth, the most common dental anomaly, refers to teeth whose germ did not develop sufficiently to allow the differentiation of the dental tissues. This study aimed to determine the importance of genetic investigations on oral health-related quality of life in children with congenitally missing lateral incisors. We are presenting a genetic study performed by the A.S. Division of Medical Genetics, in Bucharest, Romania, in the case of a patient (16-year-old boy) presenting bilateral maxillary lateral incisor agenesis in the permanent dentition, a hereditary, non-syndromic familial hypodontia in three successive generations, which suggest an autosomal dominant inheritance, combined with aplasia of maxillary third molars. The oral visual examination and the radiological exam of the patient were completed with the oral photographic examination, the family historyinvestigation, the pedigree, and the analysis of the studied family’s genealogical tree, after acquiring the signed informed consent of the patient. The genetic investigations of the congenitally missing lateral incisors, allow the calculation of the anomaly recurrence risk amongst offspring, and the direct and specialized monitoring of affected families, in order to limit the potential complications.

Cross-Sectional Analytical Study to Determine Knowledge and Attitude of Dental Students about COVID-19 Disease and Its Precautionary Measures 牙科学生对COVID-19疾病及其预防措施的知识和态度的横断面分析研究

Objective: To determine knowledge and attitude of dental and medical students about COVID – 19 disease and its precautionary measures Methodology: The study was conducted after approval from the ethical review committee of the Foundation University College of Dentistry, Islamabad from June 2021 to September 2021. Inclusion criteria adhered to the recruitment of fourth and final-year students of dentistry. Informed written consent was taken after explaining the study protocol. The format of the questionnaire was based on previously published studies, literature, and guidelines provided by Centers for Disease Control. The questionnaire was validated by a Pilot Study and the opinion of dental health experts on the subject. Methodology constituted of a paper-based and web-based questionnaire with closed-ended questions and feedback were received from (60%) participants. Variables for analysis included demographic profile, next section comprised of questions regarding COVID – 19 disease knowledge, its precautionary measures, and attitude towards the disease. Moreover, participant's awareness in case of suspected infection was also analyzed to gauge the level of preparedness. There was a total of 256 participants enrolled in this study via consecutive non-probability sampling methodology. Results: There was a total of 256 participants enrolled in this study. Out of 256, 154 (60.2%) were male and 102 (39.8%) were females. 110(42.8%) believed that COVID-19 symptoms resolve without treatment, 256(100%) believed that disease spread can be prevented with education. 222(86.6%) endorsed a preference to avoid or work with the COVID-19 Patient. As per opinion regarding social distancing, wearing masks and washing hands before initiation of treatment, 246(95.1%) believed it to be mandatory for disease transmission reduction whereas 38(14.9%) considered it unnecessary. Conclusion: Dental students demonstrated ample awareness and knowledge regarding COVID-19 disease presentation, preventive measures, and strategy in case of suspected infection with a motivated attitude to spread awareness among the population.

Efficacy of Chlorhexidine Intracanal Medicament on Periodontal Healing in Concomitant Endodontic-Periodontal Disease with Communication: A Randomized Clinical Trial 氯己定管内治疗伴发性牙髓-牙周病牙周愈合的疗效:一项随机临床试验

Background: Chlorhexidine has been extensively used in various periodontal and endodontic infections. However its use in combined endo perio lesion has not been defined clearly. The aim of this study was to evaluate the efficacy and synergistic effect of chlorhexidine intra canal medicament with combination of comprehensive periodontal therapy in endodontic periodontal lesions. Materials and Methods: The patients were randomly assigned group 1 and group 2. Both the groups were treated by phase 1 and phase 2 periodontal treatments while group 1patients received conventional root canal treatment and group 2 patients were treated by chlorhexidine intracanal medicament for 6 months. The primary outcome measures are probing pocket depth, relative attachment level and bleeding on probing. Results: Both groups showed improvement on primary outcome measures. However, group 2 showed significant more reduction in probing pocket depth (PPD) and bleeding on probing (BOP) and gain in relative attachment level (RAL) after complete 6 months treatment with chlorhexidine. Postoperative measurements from group1 and group 2 showed reductions in mean PPD of 2.63±0.98 and 3.57±1.13 mm, mean BOP reduction of 76.78±31.41and 87.54±22.35 and mean RAL gains of 2.42±1.27 and 3.42±0.94 mm, respectively. Conclusion: 2% CHX gel used as an intracanal medicament along with periodontal treatment demonstrated more favourable treatment response in terms of periodontal healing.

Introduction of New Tooth Notation Systems in Comparison with Currently In-Use Systems 介绍新的牙齿符号系统,与目前使用的系统进行比较

Dental charting, referral notes, and dental financial claims are integral parts of dental practices for their smooth and effective usage. Currently, dental charting is proceeded by one of the three commonly used tooth notation systems, The Zsigmondy-Palmer, Federation Dentaire International, and American Dental Association. Although these systems have been used and adopted internationally, in practice, there is much confusion in referring to a tooth, which leads to mismanagement and eventually leads to confusion affecting the clinician-patient relationship. Hence, a growing need for a new system to make dental charting simple, secure, and void of confusion is always felt. In this experiment, we evaluated currently in use tooth notation systems and introduced three tooth numbering systems named based on the place of origin as the Herat Dentistry Faculty 1, Herat Dentistry Faculty 2, and Herat Dentistry Faculty 3 tooth notation systems. A questionnaire containing 17 questions was designed, a meeting was held with the participants where both systems were introduced, and the questionnaire was explained to them. The questionnaire was filled by a population of 481, among which 213 were randomly selected, and data were entered into SPSS. The results proved that the three newly suggested systems are more efficient in terms of learning and entering into patient files compared to traditionally in-use systems.

Effect of Lateral Cyclic Loading on Screw Loosening in Morse Taper Implant-Straight Abutment Connection 横向循环荷载对莫氏锥度种植体-直基台连接螺钉松动的影响

Background: Screw loosening is a common problem that can lead to mechanical and biological failure of dental implants. This study aimed to assess the effect of lateral cyclic loading on screw loosening (detorque value) in Morse taper implant-straight abutment connection. Methods: This in vitro, experimental study evaluated 12 dental implants with Morse taper implant-abutment connection. Dental implant-abutment assemblies were mounted in transparent acrylic resin blocks with 45-degree angle relative to the acrylic surface. The abutment screw was torqued to 30 N/cm by a digital torque-meter and retorqued after 10 minutes. The assemblies were then randomly divided into two groups (n=6). In the test group, lateral cyclic loads with 75 N magnitude and 1 Hz frequency were applied at 45-degree angle relative to the longitudinal axis of the fixture. After 500,000 cycles, the detorque value was measured at the implant-abutment interface by a digital torque-meter. The assemblies in the control group did not undergo cyclic loading. The detorque values were compared between the two groups using the Student t-test (alpha=0.05). Results: The mean detorque value was 23.17±1.33 N/cm in the control and 21.3±3.38 N/cm in the test group. According to the Student t-test, this difference was not significant (P=0.4). Conclusion: According to the present results, application of lateral cyclic loads had no significant effect on screw loosening (detorque value) of Morse taper implant-straight abutment connection but insignificantly increased the torque loss.

In the test group, lateral cyclic loads with 75 N magnitude and 1 Hz frequency were applied at 45-degree angle relative to the longitudinal axis of the fixture. After 500,000 cycles, the detorque value was measured at the implant-abutment interface by a digital torque-meter. The assemblies in the control group did not undergo cyclic loading. The detorque values were compared between the two groups using the Student t-test (alpha=0.05). \u0000Results: The mean detorque value was 23.17±1.33 N/cm in the control and 21.3±3.38 N/cm in the test group. Oral Health Policy in Latin America: Challenges for Better Implementation 拉丁美洲口腔卫生政策:更好实施的挑战

Background: Oral health is considered an important public policy. This study aimed to understand which barriers and facilitator occurs during the implementation of oral health programs/policies. Methods: A qualitative study was conducted within personal interviews with decision-makers, at the political, technical, and academic levels. Data were transcribed and analyzed using Maxqda®. Results: The formation of dentists with an emphasis on the public service proved to be a critical knot, also the population´s lack of knowledge about their rights was mentioned. In Chile and Colombia, one of the main facilitators was the previous existence of oral health programs. As barrier, the lack of systematization of epidemiological data. In Brazil, the main facilitators were many dentists available and the political will of politicians to include oral health as public. Managers unprepared to work in public service was one of the main barriers. Conclusions: During the development and implementation of public policies several factors are encountered, it can be concluded that human resources are fundamental elements for the functioning of public service, besides the interest of the managers and politicians for the inclusion of this theme in the political agenda of the country.

Factors that Influence the Clinical Longevity of Ceramic Unit Restoration: A Scoping Review 影响陶瓷单元修复临床寿命的因素:范围综述

Statement of Problem: Ceramic restorations can be essential in dental rehabilitation aesthetic and functional, but its longevity and treatment plan need to be good settled down. Different material options, manufacturing techniques and protocols are available. It is important to map the literature to know the state of the art about of clinical failures related to these fixed prostheses. Purpose: Evaluate the rate of survival of single restorations (crowns and ceramic veneers) as well as identify the types of clinical complications reported. Material and methods: A scoping review was conducted from the MEDLINE/Pubmed, Embase, Scopus, Web of Science and Cochrane Library. A search of all published literature until June 2021 was based on the following keywords: ('dental crown' OR 'ceramic veneer' OR 'porcelain veneer' OR 'dental veneer' OR 'dental porcelain') AND ('survival rate' OR 'ceramic restoration failure' OR 'prosthesis complication' OR complication). Results: The most reported biological complications were need for treatment endodontic and detachment. The complications and technical failures involved, mainly, fractures and cracks in ceramics. There were also cases of pigmentation marginal. Conclusion: The survival rate of single ceramic restorations is high. The main factors influencing restoration failures are bruxism, operator inexperience and inadequate cementation.

Impact of Surface Treatments on the Roughness Surface and Fracture Pattern of Zirconia Ceramic Restorations 表面处理对氧化锆陶瓷修复体表面粗糙度和断裂模式的影响

Purpose: The aim of this study was to assess the surface roughness, and fracture pattern of zirconia ceramic restorations after the application of different surface treatments. Material and method: We conducted a systematic literature search on the electronic databases PubMed and Science direct using 12 key words via Boolean equations. We identified articles that met the eligibility criteria by checking the title and abstract. A full-text review was used to select articles based on the CASP grid. (The Critical Skills Appraisal Programme) Results: 13 articles were selected after reading the full text. We noted a predominance of mixed failure in our study. Conclusion: Sandblasting before sintering is a recommended method to increase surface roughness and improve the bond strength of the cosmetic ceramic. The mixed fracture mode is the most frequent mode in our study. Further clinical studies with standardization of protocols should be conducted.

A Methodological Assessment of Weaknesses in Systematic Reviews on Pulp Capping 牙髓封盖系统综述中缺陷的方法学评价

Introduction: Clinical decision making in dentistry is directly impacted by the systematic reviews available. The increasing number of systematic reviews along with their direct impact in clinical practice has emerged a need to assess their quality. Pulp capping is an important alternative to the more invasive interventions such as endodontic treatment and aims to preserve pulp tissue. The aim of this study is to methodologically assess the weaknesses of systematic reviews on pulp capping and provide recommendations on how to improve them. Methods: An electronic literature search was conducted in Pubmed, Cochrane Library and Web of Science up to January 2022. Systematic reviews with or without meta-analysis including randomized and non-randomized studies on pulp capping were retrieved. A methodological assessment of their quality was performed using AMSTAR 2. Results: A total of 203 publications were identified and reviewed for eligibility. Twenty-seven fulfilled the inclusion criteria. The reviews were assessed using AMSTAR 2 by two independent reviewers. The results were analyzed, and weaknesses were noted. Conclusion: The study suggests an inconsistency in methods and structure in systematic reviews on pulp capping. Readers of the reviews should make use of AMSTAR 2 in order to evaluate their quality. Suggestions and weaknesses pinpointed can aid future systematic reviews to be more comprehensive with a more unified methodology.

During root development, if tooth is subjected to trauma or caries, it results in tooth with immature and open root apices. The primary factor affecting the success of endodontics is achievement of perfect apical seal. Apexification is done to achieve an apical barrier against which the obturating material can be condensed. Various treatment modalities are used to treat immature open root apices. In the following case reports, apexification using different treatment modalities and both traditional and newer biomaterials have been explained.
