Developing competency among interns in palliative care of critically ill patients 培养实习生在危重病人姑息治疗方面的能力

Background: Palliative care has become increasingly important in the last decades with a rise in needy patients and subsequent shortage of health care professionals serving palliative care, making it a global public health concern. Patients and methods: Educational interventional study conducted in the Department of Anaesthesi-ology of a tertiary care teaching institute. Pre-and post-tests by standardized validated multiple choice questions for assessment of knowledge and awareness were conducted before and after the session. Interactive didactic lectures integrated with faculty narrative from the Department of Palliative Care, demonstration among small groups, and live demonstration on patients under the aegis of the Anaes-thesia Department were given after the pre-test. A feedback questionnaire in the form of a Likert scale for assessment of students’ satisfaction and attitude was done at end of a session. Results: Fifty (50) interns participated in the study. Out of 50, 28 (56%) were male and 22 (44%) were female interns. The mean pre-test score was 8.82 ± 2.13 (range 4–12) out of a total of 20. The mean post-test score was 14.44 ± 1.72 (range 11–17). The pre-and post-test results difference was significant (p < 0.0001). The percentage gain in knowledge and awareness was 63.95%. Conclusions: Most professionals will need basic skills of various management modalities in supportive therapy in near future to fully fill the demand for palliative care which is going to be doubled within the next few decades so the need for conducting more such sessions regularly amongst young budding doctors including interns at a very early stage to develop competency for palliative care was observed. Impact of persistent coronavirus infection on the treatment of an immunocompromised oncological patient 持续冠状病毒感染对免疫功能低下肿瘤患者治疗的影响

The emergence of the coronavirus pandemic in 2020 has challenged many aspects of the management of clinical care. It has negatively impacted the already overwhelmed healthcare system in Poland, leading to further limitation of access to specialist care, delay of treatment and even failure to initiate it. Patients with severe, rapidly progressing diseases such as cancer, are among those most adversely affected. Immunocompromised patients are prone to persistent COVID-19 infection and re-test positively even when asymptomatic. In this case report, the authors present an immunocompromised patient with follicular lymphoma and active tuberculosis, who re-tested positive for SARS-CoV-2 in real-time polymerase chain reaction and rapid antigen tests twenty-two times over seventeen weeks of hospitalisation in the isolation ward in University Clinical Centre in Gdansk, Poland. The management of her oncological treatment was significantly disturbed by prolonged isolation and organisational issues arising from the coronavirus pandemic. Copyright © Via Medica.

Aspects of palliative medicine in intensive care units: A narrative review 重症监护室姑息医学的几个方面:叙述性综述

Background: The perspective of palliative care has changed dynamically over the years, and palliative medicine, which was originally concerned with patients with advanced oncology diseases, has become an interdisciplinary area. Many societies have published guidelines for the use of palliative medicine in intensive care units. Method(s): This article presents indications and methods for implementing the principles of palliative medicine in intensive care units. Particular attention is devoted to the consultations of palliative medicine for current medical trends - COVID-19 infection, oncological diseases, fragility syndrome, and end-stage circulatory failure. Conclusion(s): Elements of palliative medicine are necessary for everyday practices in the intensive care unit. The most important task in cooperation is to present classifications that can help in the objective identification of patients requiring palliative care. It seems that creating a checklist of the qualifications for a palliative medicine consultation can be the next step towards making decisions about this form of therapy. Copyright © Via Medica.
