Introduction: This research paper is a cross-sectional study that aims to estimate the prevalence of oral health problems among adult patients in palliative care, as well as to investigate the oral hygiene practices of palliative care patients in Brunei Darussalam. Patients and methods: In this study, the Oral Palliative Care and Assessment and Referral tool (OPCAR) along with a 15–item questionnaire were utilized as reliable tools for measurements. All palliative care patients who had been assessed in this study were invited to participate. A total of 90 palliative care patients were invited to participate and 73 patient data were used for the subsequent data analysis. Results: The three most common problems among in palliative care patients were saliva, lips, and teeth category. Fisher’s exact test identified there was a significantly higher teeth-related problems (teeth caries, fracture, broken root stumps) in non-denture wearing patients as compared with denture wearing patients. However, other associations between denture wearing to oral hygiene was of little significance. Conclusions: Using the normal approximation sample method, it was revealed that almost all the participants were eligible to be referred to the dental clinic for at least one oral problem.

Endocrine and metabolic disorders can occur in the form of paraneoplastic syndrome and as a consequence of oncological treatment. The most common disorders include hypercalcaemia, ectopic production of vasopressin in the course of cancer, ectopic Cushing’s syndrome and secondary osteoporosis. What should be taken into account during the diagnosis of endocrine disorders, is paraneoplastic syndrome that frequently precedes the occurrence of symptoms typical of a particular diagnosis, which could influence the proper orientation of the diagnostic process. During and after anticancer treatment, it is recommended that patients should be monitored for the possible development of acute or chronic metabolic and endocrine disorders which may pose a threat to their life or health.

The endocannabinoid system (ECS) regulates body’s homeostasis and performs vital physiological functions. The thorough understanding of the role of endocannabinoids and the composition of cannabis containing phytocannabinoids and terpenoids play an important role in symptom management. Clinical effects of medical cannabis depend among others on the activity of the ECS in a given patient, the proportion of phytocannabinoids, and the doses used. Cannabinoids are used to treat symptoms in patients with cancer and chronic non-malignant diseases. Cannabinoids are usually not drugs of first choice and can be added to basic pharmacotherapy, which often improves the effectiveness of symptom treatment. Due to limited scientific evidence, experimental studies on the role of the ECS and clinical trials on the use of cannabinoids in the treatment of symptoms are recommended. In this article the role of the ECS and the use of cannabinoids in palliative care patients has been discussed.

Background: The Edmonton Symptom Assessment System (ESAS) quantifying the most common symptoms rated in the numerical rating scale (NRS) for some respondents are cumbersome or incomprehensible. The study aimed to introduce, implement and validate psychometric properties of the ESAS (revisited) six-point Verbal Rating Scale — Polish version (ESAS-r 6VRS-PL) into a Polish hospice setting. Methods: In a cross-sectional, observational prospective psychometric validation study the adult advanced cancer patients admitted to a hospice were evaluated twice for test-retest reliability. Each patient’s evaluation encompassed the ESAS-r 6VRS-PL, which was compared on the first day with the validated Polish version of the European Organisation for Research and Treatment of Cancer QLQ-C15-PAL, considered the gold standard, to accomplish its external construct validity. Results: Of the 57 patients enrolled 48 were evaluated twice and 9 (15.5%) dropped out. All respondents were able to describe their symptoms in < 15 minutes independently without any need to clarify the meaning of the questions and only occasionally with assistance in reading the questions or answers. The high intraclass correlation coefficient of ESAS Global Distress Score was noted (0.825; 95%CI 0.723–0.888). Similar items of the ESAS-r 6VRS-PL and QLQ-C15-PAL showed moderate correlations (range 0.59–0.65; p < 0.001). The internal consistency of the ESAS-r 6VRS-PL measured using Cronbach’s alpha coefficient was fairly high, reaching 0.723 (item range 0.664–0.734). Conclusion: The ESAS-r 6VRS-PL is a simple, valid, reliable and feasible test which allows to estimate the QoL in advanced cancer patients.

Background. The introduction of new permissions, in the field of prescribing medicines and medical devices for nurses and midwives in Poland, took place at the beginning of 2014. Then a draft amendment to the Act on the profession of nurse and midwife was presented by the Ministry of Health. The aim of the study was to explore nurses opinions about the possibility of prescribing drugs and medical devices. Material and methods. The study was conducted in the period from January to February 2020 among 100 nurses related to palliative care from all over Poland. An own survey questionnaire consisting of 23 questions was used. Results. All participants responded to the questionnaire (response rate = 100%). More than half of the respondents (55%) write prescriptions in palliative nursing practice. The decisive majority of respondents (89%) confirmed that the obtained right to prescribe increases the prestige of the nursing profession and 83% of respondents believed that this right contributes to an improvement of the quality of medical services. A total of 57% of surveyed participants indicated the need to expand a list of drugs that could be prescribed by palliative care nurses. Conclusions. The decisive majority of the surveyed nurses expressed views that the obtained possibility to prescribe drugs increases the prestige of the nursing profession and that this right contributes to the improvement of the quality of medical services provided by nurses.

Introduction. To assess knowledge, experiences, perceptions and barriers of healthcare professionals regarding oral palliative care. Material and methods. The study involved 150 participants working at, a tertiary healthcare institute in India. Data collection tool was pretested and self-administered with sections on demographics, knowledge, attitude and practices, patient referral, perspectives, and barriers to oral palliative care. Results. Majority of participants 142 (94.6%) believed that palliative care patients need oral care, 85 (68.6%) participants had treated palliative care patients with dental problems. However, 60 (40%) had not received formal training for assessment and referral of patients with oral problems; 95 (63.4%) had never used tools to assess oral conditions of palliative care patients. According to 69 (46%), the best method to maintain oral hygiene is rinsing with saline and 81 (54%) expected physicians to be responsible for oral care of palliative care patients. Main challenges in providing oral care were lack of proper guidelines listed by 117 (78%) and lack of formal training indicated by 60 (40%) respondents. Conclusions. This study highlighted the need of effective assessment of the mouth, and appropriate oral care. Training of healthcare professionals, educating families and patients with oral palliative care are necessary to effectively manage oral symptoms. An appropriate patient follow-up and care delivery system should be structured at comprehensive cancer centres, which can improve quality of life and compliance of patients. There is a need for development of assessment tools and referral practices for providing relief, comfort and consolation to patients and families.

Introduction: The use of morphine is viewed as an integral part of pain management in cancer patients. The responsibility for pain treatment lies with physicians. Therefore, this study assessed the knowledge of clinical use of morphine in the management of pain in cancer patients among physicians working in Kebbi State, Nigeria. Material and method: A self-administered questionnaire was distributed to all the 110 participants in attendance at the Continuing Medical Education meeting organized by the Nigerian Medical Association, Kebbi State Branch. Data were analyzed using SPSS version 20. Result: A total of 95 physicians participated, including 12 consultants (12.6%), 38 registrars (40.0%), 35 medical officers (36.8%), and 7 house officers (7.4%). Only 30 respondents (31.6%) had ever received professional training on pain management or analgesic use. Overall, only 10 (10.5%) respondents had good knowledge of pain management in cancer patients and morphine use. Conclusion: Physicians knowledge of pain management in cancer patients and clinical use of morphine is poor. There is a need to improve active professional analgesic education programs.
