An extended conceptual model of consent for information systems 信息系统同意的扩展概念模型

With the digitisation of off-line processes in e-commerce and moves towards the digitisation of consent processes in health care, the need for effective on-line consent management has grown considerably. Yet, contemporary constructs in information systems are unreportedly incomplete. For instance, the focus in contemporary literature is on the flawed notion that consent decision-making is synonymous with access control decision-making. Therefore, more elaborate conceptualisations are required, which promote the development of more effective consent management systems. To this end, in this paper, a novel conceptual model is proposed (extending Friedman et al.'s models) which describes in more detail key aspects of the idea of consent as widely recognised in health care. To help others assess or apply the model, the theory-based rationales underpinning the model are documented. It is argued that while the model is necessarily incomplete, it is also useful, insofar as it improves thinking about consent management, and supports research about, and the design and analysis of, more effective consent management systems.

Automatic no-reference quality assessment for retinal fundus images using vessel segmentation 基于血管分割的视网膜眼底图像自动无参考质量评估

Fundus imaging is the most commonly used modality to collect information about the human eye background. Objective and quantitative assessment of quality for the acquired images is essential for manual, computer-aided and fully automatic diagnosis. In this paper, we present a no-reference quality metric to quantify image noise and blur and its application to fundus image quality assessment. The proposed metric takes the vessel tree visible on the retina as guidance to determine an image quality score. In our experiments, the performance of this approach is demonstrated by correlation analysis with the established full-reference metrics peak-signal-to-noise ratio (PSNR) and structural similarity (SSIM). We found a Spearman rank correlation for PSNR and SSIM of 0.89 and 0.91. For real data, our metric correlates reasonable to a human observer, indicating high agreement to human visual perception.

An automatic clinical document importance estimator for an existing electronic patient record — Architecture and implementation 现有电子病历的自动临床文件重要性估计器。体系结构和实现

The goal of the OPTIM project is to optimize the graphical user interface of an electronic health record (EHR) by predicting clinical documents' relevance and provide a ranked list of relevant documents for the given user at a certain time. This paper describes the architecture of the relevance assignment and ranking prototype and some implementation issues. The prototype's design is based on two components: OPTIM Core, with logical representation, estimation server's integration and the webservice layer, and the OPTIM WebUI, with the user interface for presenting the results. The prototype was tested in integration with an EHR using a simulated environment. The results were encouraging but yet they revealed a certain lack of security (confidentiality). It has now the capacity of rating 10 documents per second. Nonetheless, the integration of features such as rating clinical relevance based on mathematical models can be included in existing EHR potentially improving their usability.

Using a clinical document importance estimator to optimize an agent-based clinical report retrieval system 使用临床文件重要性估计器优化基于agent的临床报告检索系统

The OPTIM project aims to optimize the graphical user interface of an electronic health record (EHR) by predicting clinical documents' relevance and provide a ranked list of relevant documents for the given user at a certain time. In this paper we will describe part of the system architecture pertaining to report retrieval and relevance assignment, focusing on the optimization of an agent based report retrieval system (MAID) using the webservice layer of the OPTIM project. The prototype of MAID using the optimized retrieval was tested in a simulated environment. In the executed simulations the classifier was able to rate 10 reports per second. Including a report rating in the EHR interface based on clinical relevance calculated by mathematical models can potentially improve the usability of the EHR.

Automatic Mapping tool of local laboratory terminologies to LOINC 本地实验室术语自动映射工具到LOINC

Clinical concepts and terminologies usually differ from one software system to another, making the exchange of data between them a very arduous and difficult task. It mainly involves finding the correspondences between the local identifiers of the concepts to be exchanged. In this paper, we describe a software tool called SAM (Automatic Mapping System), a tool for terminological normalization of laboratory tests to LOINC. SAM helps technicians in this task through simplification of the terminological mapping process. It uses approximate string matching techniques along with a thorough mining of previous mappings and LOINC parts.

Automatic nerve tracking in confocal images of corneal subbasal epithelium 角膜基底下上皮共聚焦图像中的自动神经跟踪

The analysis of nerve structures in the cornea subbasal epithelium is of relevant clinical interest, as it provides information related to changes caused by surgical interventions, transplantation or diseases (i.e. diabetic neuropathy). We addressed the problem of recognizing and tracing corneal nerves in confocal microscopy images with a novel method based on a sparse tracking scheme. After a set of seed points is identified all over the image, nerves are traced by connecting seeds by means of minimum cost paths, whose weights have been estimated considering several directional measures. The performance of the method was assessed on a dataset of 30 images, from both normal and pathological subjects, whose nerves were traced manually to provide a ground truth reference. An average sensitivity of 0.85 and false detection rate of 0.05 were obtained, with an average running time of 25 seconds per image.

Modeling and architecture design of an MDPnP acute care monitoring system MDPnP急症监护监测系统的建模与体系结构设计

Medical Device Plug-and-Play (MDPnP) permits the mitigation of preventable medical errors. However, MDPnP results in a dynamic environment, which presents great challenges to traditional software architectures. This paper addresses these challenges by describing an abstract representation of dynamic clinical environment and a modular architecture is utilized to support safe system reconfiguration.

A scalable ontology reasoner via incremental materialization 通过增量物化实现可扩展的本体推理器

Ontology based knowledge management systems have a lot of potential: their applicability ranges from artificial intelligence, e.g., for knowledge representation and natural language processing, to information integration and retrieval systems, requirements analysis, and, most lately, to semantic web applications and workflow management systems. However the huge complexity of reasoning for ontologies with large TBoxes and/or ABoxes is often a barrier to their applicability in real-world settings especially those which are time sensitive. Materialization is a promising solution for scalable reasoning over ontologies with large ABoxes as it derives the implicit knowledge of an ontology and makes it available in a relational database. Although materialization can reduce the query answering time of an ontology, it has limitations in applications which require frequent update to the knowledge base. To overcome this problem, we developed a tool for incremental materialization which identifies the fragment of the ontology that needs to be updated due to the ABox or TBox change, thereby reducing the complexity of the exhaustive forward chaining required.

Autopilot spatially-adaptive active contour parameterization for medical image segmentation 用于医学图像分割的自动驾驶空间自适应主动轮廓参数化

In this work, a novel framework for automated, spatially-adaptive adjustment of active contour regularization and data fidelity parameters is proposed and applied for medical image segmentation. The proposed framework is tailored upon the isomorphism observed between these parameters and the eigenvalues of diffusion tensors. Since such eigenvalues reflect the diffusivity of edge regions, we embed this information in regularization and data fidelity parameters by means of entropy-based, spatially-adaptive `heatmaps'. The latter are able to repel an active contour from randomly directed edge regions and guide it towards structured ones. Experiments are conducted on endoscopic as well as mammographic images. The segmentation results demonstrate that the proposed framework bypasses iterations dedicated to false local minima associated with noise, artifacts and inhomogeneities, speeding up contour convergence, whereas it maintains a high segmentation quality.

Community pharmacies and eHealth services: Barriers and opportunities for real Primary Healthcare integration 社区药房和电子卫生服务:初级卫生保健真正整合的障碍和机会

Background: Primary Healthcare (PHC) reforms are presently addressing chronic-disease management. It is of importance to science and society to understand how eHealth services could be developed and used to address the challenges of health services integration like community pharmacies within PHC. Method: Focusing on the two initial steps of Design Science Research Methodology, a mixed methods approach was used with an online survey to collect data on use of Information Technologies (IT) in community pharmacy, followed by an exploratory observational time and business processes study, using the shadowing method. Results: The current state of IT usage in Portuguese community pharmacies is mainly focused on medicine dispensing. Pharmacists spend 50% of their time in patients' interactions, 38% on administrative tasks. Patterns of used time reflect a need to internal reorganization of pharmacies in order to enable eHealth pharmaceutical services provision.
