Barriers Contributing to Policy Deviation: A Mixed Methods Study of Policymakers and Frontline Nurses 政策偏离的障碍:对政策制定者和一线护士的混合方法研究

Deviation from the policy at the point of care is frequently studied as a balancing act of health professionals, resulting in a lesser focus on barriers leading to such deviation. This study investigated practices of policy application or deviation with two aims. One, to assess if the frontline nursing staff is applying or deviating from the policy. Two, to understand the barriers that lead to policy deviation from the perspectives of policymakers and frontline staff. An explanatory sequential mixed methods design was applied, with a quantitative survey first (n=50) and then two qualitative focus group discussions. Data was collected in 2018 within a Local Health District (LHD) in New South Wales, Australia. Most respondents (96%) rated policy application to be the usual practice at work. Despite that, survey respondents (54%) agreed to have discretionally acted against policy requirements. Frontline nurses deviated from policy when they perceived a lack of functional merit. Examples of barriers that contributed to deviation from policy are unstructured policy review, inadequate support for policy writing and communication challenges during policy implementation. These barriers were jeopardizing appropriate policy development and implementation and often negatively influenced the functional merit of policy. A few known strategies, such as appointing policy champions and promoting policy messages through a combination of channels, should be considered to mitigate the identified barriers. Future studies can explore effective ways to manage policy deviation rather than relying on street-level bureaucracy.

Effect of Implementing Clinical Pathway among Pregnant Women with Pyelonephritis on Childbirth and Neonatal Outcome 实施临床路径对肾盂肾炎孕妇分娩及新生儿结局的影响

Aim of this study to determine the effect of implementing clinical pathway among pregnant Women with pyelonephritis on childbirth and neonatal outcomes. Design: A quasi –experimental design. Setting: The study was conducted at outpatient’s clinic of obstetric department at Shebin EL-Kom Teaching Hospital, Menoufia University hospital at Shebin EL-Kom. Subjects: Convenient sample was used all stuff nurses in the obstetric department (20 nurses) as well as 50 pregnant mothers will selected and divided into two equal groups; intervention group (25mothers who received clinical pathway during second trimester of pregnancy) and control group (25 mothers who not received clinical pathway). Instruments: interviewing questionnaire for pregnant mothers, & nurses and Observation checklist of neonatal outcomes. Results: The study revealed that there was highly significant improvement in women knowledge after implementing clinical pathway, there was significant improvement in nurse’s knowledge, childbirth, and neonatal outcomes. Conclusion: There was significant improvement was obvious among women relief of their reported symptoms and their knowledge about pyelonephritis and measures to relieve it. Recommendations: Improve the quality of health care provided through routine screening of pregnant women and applying workshops for nurses to improve their knowledge about pyelonephritis during pregnancy.

A quasi experimental study to assess the effectiveness of hot water foot bath therapy on quality of sleep among the elderly in a selected old age home, Agartala, Tripura West. The objectives of the study is to assess the quality of sleep among the elderly and to determine the effectiveness of hot water foot bath therapy on quality of sleep among the elderly. The conceptual framework was used based on Wiedenbach’s modified Prescription theory. In this study elderly persons of old age home were selected as a sample. 40(fourty) samples were selected for the study, 20 (twenty) for both experimental group and control group by convenient sampling technique .Sleep quality of the elderly in both experimental group and control group assessed using modified Groningen sleep quality scale by interview technique which is modified by 7 (seven) experts of various nursing and medical department as well as by the statistician. Reliablity of the tool was 0.85 (r), that was checked using Kerl pearsons correlation coefficient method. Statistical analysis was done by chi square test and t test both paired t test, unpaired t test to determine the effectiveness of hot water foot bath therapy in quality of sleep among the elderly in both experimental and control group. There was significantly increased in mean post test score in experimental group with the mean deviation of 3.4 at 0.05 level of significance. The finding of the study revealed that paired ‘t’ test between pretest and post test score in experimental group showed significant increase in quality of sleep (tcal = 15.86, pcal = 0.53, pcal = 5.09, p2 = 5.714, p< 0.05) and there was no significant association between the pre-test quality of sleep scores with remaining demographic variables. The study concludes that hot water foot bath therapy is effective in improve the quality of sleep among the elderly.

Missed Nursing Care and the Predicting Factors in Saudi Arabia 沙特阿拉伯护理缺失及预测因素

The Аim of the study: This study аimed to determine the levels and types of missed nursing саre and to identify рrediсtоrs of “missed nursing саre” based on bасkgrоund variables, and рerсeived reasons for missed саre. Research design: Quantitative descriptive сrоss-seсtiоnаl соrrelаtiоnаl research design. Setting: This study was соnduсted 10 hоsрitаls in Makkah region, Saudi Аrаbiа. Sample: 309 Раrtiсiраnts was recruited using а nоnрrоbаbility соnvenienсe sampling methods. Tооls of data соlleсtiоn: The MISSСАRE Survey was utilized to measure nursing staff рerсeрtiоns of both аmоunts of missed nursing саre and the reasons for missed nursing саre. Result: The result shows that аmbulаtiоn of раtients was the most frequently reроrted element of missed саre followed by emоtiоnаl suрроrt to раtient and/or family, feeding раtient when the fооd is still warm, setting up meals for раtient who feeds themselves, resроnse to саll light is initiated within 5 minutes, and attend interdisсiрlinаry саre соnferenсes. The study аlsо revealed that inadequate lаbоr resources were the most reason of missed саre followed by material resources and соmmuniсаtiоn. Соnсlusiоn: This study соnсluded the роint to the need to асknоwledge сertаin аsрeсts of missed nursing саre and the different roles within nursing. Highlighting the most relevant reasons for missed nursing саre might help nurse аdministrаtоrs in preparing resроnsive strategies to eliminate such reasons.

Nursing Skills Laboratory as Milieu of Clinical Learning and Practice 护理技能实验室作为临床学习和实践的环境

Objective: The study's objective is to assess the effectiveness of the university nursing skills laboratory if it functions as the milieu of clinical learning and practice. Methods: The research employed a mixed-method approach that included both quantitative and qualitative components. Results: The study revealed that most nursing students are still female and have already long experienced using nursing laboratories. The nursing skills laboratory assists in developing the psychomotor skills of the students (3.34) and builds confidence and overcome fear in performing nursing procedures' (3.26). The checklist used in the nursing skills laboratory is essential. The rubrics of the checklist provided accurately measure student skills (3.21); however, it should be based on or designed after the hospital setting (2.86). The respondents stressed that an additional time for an open laboratory hour enhances their nursing skills (3.06.). Some respondents feel the insufficient time to practice specific nursing skills for a particular session (2.78). Respondents stated that the availability of equipment and supplies affects the quality of their nursing skills performance (3.08). There was a significant difference in the assessment of the respondents when the year level was considered. Lastly, respondents stated that the challenges they face in the nursing laboratory are a lack of sufficient equipment and materials and a lack of time to practice the skills they have learned. Conclusions: Based on the study's findings, the nursing skills laboratory should have sufficient equipment and supplies in the simulation to use during the nursing skills demonstration and re-demonstration. Additional time should be given to the students to practice the skills they have learned, and the checklist should be designed after the hospital setting to reduce the risk and ensure patient safety.

The checklist used in the nursing skills laboratory is essential. The rubrics of the checklist provided accurately measure student skills (3.21); however, it should be based on or designed after the hospital setting (2.86). The respondents stressed that an additional time for an open laboratory hour enhances their nursing skills (3.06.). Some respondents feel the insufficient time to practice specific nursing skills for a particular session (2.78). Respondents stated that the availability of equipment and supplies affects the quality of their nursing skills performance (3.08). There was a significant difference in the assessment of the respondents when the year level was considered. Lastly, respondents stated that the challenges they face in the nursing laboratory are a lack of sufficient equipment and materials and a lack of time to practice the skills they have learned. Conclusions: Based on the study's findings, the nursing skills laboratory should have sufficient equipment and supplies in the simulation to use during the nursing skills demonstration and re-demonstration. Newly Graduate Registered Nurses’ Experiences of Transition to Clinical Practice: A Systematic Review 新毕业注册护士向临床实践过渡的经验:系统回顾

New graduate registered nurses encounter many difficulties such as unexpected role, stress and lack of support. Therefore, many of them consider leaving their nursing roles. This systematic review aims to explore the experiences of newly graduate registered nurses about the transition from student nurse to graduate nurse in clinical practice. A qualitative systematic review was conducted using inclusion and exclusion criteria and PRISMA guideline. Twenty-three studies are included in the review and analysed using thematic analysis. The four researchers managed quality appraisal and data extraction. Of 23 studies included in the review, 22 were qualitative studies, and one was mixed methods. Following the thematic analysis, four themes were identified: (i) the impact of the undergraduate experience (ii) mental stress and coping strategies (iii) Existing knowledge of organizational support and (iv) professional accountability and competency. Findings show that newly graduate registered nurses experience high levels of turnover due to work stress, lack of support and inadequate undergraduate preparation. The successful transition is influenced by educational preparation, hospital orientation, and organizational support. Further research is required to explore the experiences of newly graduate registered nurses in developing countries, as there is limited research, which has considered nursing education, healthcare facilities, and cultural influences in these countries. New graduate registered nurses play a significant role in the provision of patient care and enhancing patient safety. This review explored their experiences about the transition from student to registered nurses to recommend comprehensive practical guidelines to support their involvement in patient safety and to provide insight for nurse’s leaders to understand the retention issues for new nurses. In addition to helping undergraduate nursing education programs to improve their strategies in preparing graduates for entry into professional practice.

Knowledge of Nurses in Hospital Preparedness in Emergency Response in Saudi Arabia: Basis for Staff Program Development 护士在沙特阿拉伯医院应急准备中的知识:员工计划发展的基础

Objective: The study assessed the level of knowledge of nurses in hospital emergency response based on WHO standard and determine what staff program development may propose to enhance the knowledge of nurses in hospital emergency response. Methods: The study utilized quantitative type of research and used the descriptive evaluative design. Results: Results of the study revealed that respondents are knowledgeable in 2 categories of hospital preparedness in emergency response, the safety and security (WM=2.5443, SD=.13295)) during MCI and the human resource during MCI (WM=2.9266, SD= .13901). Conclusion: With the findings of the study the respondents need to have more training related to hospital preparedness in emergency response. The training should focus on incident command system, communication, triage during MCI, surge capacity during MCI, continuity of essential service, logistics and supply and post disaster recovery. The training should situate the Health Emergency Response Recovery Plan (HERRP) and policy of hospital, covering all-hazard approach based on their risk assessment.

Pressure Injury Care Program Effects on Nurse’s Performance and Patients’ Pressure Injury Wound Healing Outcomes 压力伤护理方案对护士工作表现及患者压力伤创面愈合结果的影响

Pressure Injury (PI) is a major health problem which imposes a considerable social and economic burden nationally and globally. Aim of the study was to investigate the effect of PI care program on nurse’s performance and patients’ pressure injury wound healing outcomes. Study design was a quasi-experimental control group using pre-post and follow up design. Sample of thirty nurses and fifty patients were allocated in the current study in two medical units at one of the general hospitals in Ismailia City. Tools: Six tools include Nurse’s demographic data sheet, Pressure injury knowledge questionnaire, Pressure injury care observational checklist, Patient’s demographic and medical profile data sheet, The Braden Scale, and Patients’ pressure injury wound healing outcomes tool were used to collect data. Results: The study revealed that nurses’ knowledge and practices, were significantly improved towards PI care post program implementation when compared to preprogram. Moreover, there was a significant improvement in PI wound healing among patients cared with nurses after receiving the educational program compared with patients who received care before the program. Conclusion: The PI educational program could improve nurses’ performance as well as PI wound healing among cared patients. Recommendation: Endorse the educational program for pressure injury care in the nurses’ orientation program plus continuous in-service education.

Knowledge and Skills of Cardiopulmonary Resuscitation among Critical Care Nurses in Kuwaiti Hospitals 科威特医院重症护理护士心肺复苏知识与技能

Background: The ability of nurses and the other health care professionals to deal with cardiac arrest effectively depends largely on their knowledge and skills regarding the cardiopulmonary resuscitation (CPR) procedure. Purpose: The purpose of this study was to identify the level of knowledge and skills about CPR and the factors influencing the level of CPR knowledge and skills among critical care nurses in Kuwait. Methods: To meet the purpose of this study, a descriptive, cross-sectional, and correlational design was used. A sample of 204 critical care nurses from two governmental hospitals completed the study. Three instruments were used to collect data, including the demographic questionnaire, the CPR certification exam quiz, and the Basic Life Support Skills Checklist. Results: About 56.4% of the study participants passed the skills exam about CPR. However, only 15.7% passed the knowledge test about CPR. Better CPR Knowledge was associated with working in Hospital 2, receiving formal training in CPR, and being AHA certified. Better CPR skills was related to working in Hospital 2, male gender, experience in nursing, experience in travelling with patients for seeking treatment outside Kuwait, being AHA certified, and receiving formal training in CPR (P<0.05). Conclusion: Frequent education and training in CPR in the hospital and having CPR accreditation from AHA are required for critical care nurses in order to increase their ability to save the life of critically ill patients who are at risk for cardiac arrest.

Effectiveness of Promoting Mothers’ Caring Practices Regarding Their Down Syndrome Children on the Family Coping 推广母亲照顾唐氏综合症患儿对家庭应对的影响

Down Syndrome is a lifelong chromosomal disorder caused by an extra copy of chromosome number 21 inside each of the body’s cells. This genetic disorder, causes mental retardation and developmental delays. Aim of this study was to determine the effect of promoting mothers’ caring practices regarding their children with Down Syndrome on the family coping. Subjects and Method: A quasi-experimental research design used for sixty mothers and their children with Down Syndrome at Pediatric Outpatient Genetic Clinic of Tanta University Hospital. Three tools were used for data collection: Structured Interview Schedule to assess socio-demographic and mothers, knowledge about Down Syndrome, Mothers, Care Reporting Sheet and Family Crisis Oriented Personal Scales. Results: The total scores of knowledge for more than three quarters of mothers were low before educational program while, more than half of them had moderate level of knowledge and less than half of them had high level immediately after program. The total scores of most mothers, reported care were unsatisfactory before educational program while, the majority of them had satisfactory care immediately and one month after program. Family adaptation for nearly two thirds of caregivers was fair before program while, all of them had good adaptation immediately after program. Conclusion: There was a significant improvement of mothers, knowledge, reported care and family adaptation towards crises. Recommendations: Continuous health education program should be applied to mothers of children with Down Syndrome in health care facilities to improve their knowledge and practice that affect family adaptation.
