Objective: To evaluate visual and refractive outcomes with plate-haptic toric intraocular lens implantation (ph-toric) after uncomplicated microincision cataract surgery (MICS) to correct moderate-to-severe astigmatism. Design: Prospective cross-sectional study. Methods: The study was conducted at a tertiary eye care center in New Delhi, India, on patients with visually-significant cataract and moderate-to-severe astigmatism (>1.00 diopters [D]). Preoperative parameters like visual-acuity, keratometry and astigmatism values and lens power calculation were noted. After MICS via 1.8 mm incision, a ph-toric IOL was implanted. The outcome measures noted were uncorrected and corrected distant visual-acuity (UDVA, CDVA), decrease in astigmatism and rotational stability. Follow-up was done on day 1, day 7, 1-month and 3-months postoperative. Results: This study involved 30 eyes of 30 patients. 27 patients (90%) gained UDVA of 6/ 9 or better. Out of these, 27 patients (90%) achieved CDVA of 6/ 6. Mean CDVA changed from 0.967 ± 0.101 postoperative to 0.176 ± 0.82 preoperatively (p<0.001). Design: Prospective, observational, longitudinal, descriptive, series of cases. Material and methods: Patients who underwent posterior vitrectomy and internal tamponade with silicone oil of 1000 centistokes, from April to October 2021, were considered. All subjects included in the study had a complete ophthalmic examination pre and postoperatively on first day, first week and 1 to 6 months, and were assessed the best corrected visual acuity, ocular tension (raised > 21 mmHg, hypotony < 5 mmHg), emulsification, keratopathy, cataracts and posterior pole. Results: 40 eyes of 40 patients were considered; twenty women and 20 men, with an average age of 63 years (range 46 to 77 years). Conclusions: Complications are brief, treated medically or surgically, with better prognosis.

Objective: The objective of this work was to present two unusual cases of central serous chorioretinopathy (CSC) and the chosen therapeutic method. Materials and methods: In this article, two cases of CSC in pregnant patients were described. Results: The first case was a 35-year-old patient in the 16th week of pregnancy and the second one was a 26-year-old patient in the 20th week of pregnancy. Due to the contraindications associated to pregnancy, the therapeutic method chosen was subthreshold micropulse laser photocoagulation. The functional and anatomical evolution was very good in both patients. Discussion: In both cases, treatment of the disease was preferred to prevent important photoreceptor losses. After the treatment, very good anatomical and functional results were obtained. Conclusions: The micropulse laser is an effective solution for treating CSC. It is the only safe therapeutic solution during pregnancy. CSC can be associated with pregnancy, without necessarily suggesting pre-eclampsia. Objective: To present an atypical case of tuberculous dacryocystitis. Method: An adult female presented with long standing epiphora with gradual swelling over lacrimal sac region. On syringing, water was felt in throat with no regurgitation. CT-DCG and CECT orbit were done subsequently and simultaneously. Ill-defined, enhancing soft tissue surrounding and involving the lacrimal sac wall was identified. The sac wall outline was seen distorted with contrast in NLD. The histopathology was suggestive of non-specific chronic inflammation. GeneXpert analysis was shown to be very low positive for M. tuberculosis. Montoux test was strongly positive (40 x 40 mm). ATT was started. Results: The swelling and watering subsided over the next few months. Conclusion: Tuberculosis should be considered in cases of chronic granulomatous dacryocystitis. CECT with CT-DCG is essential imaging. GeneXpert is a new and sensitive tool with considerable specificity in cases in which histopathology is not conclusive. ATT is curative and DCR is reserved for only unresolved NLDO with persistent epiphora.

Purpose: To evaluate the refractive results, visual outcomes and patients' satisfaction of a new extended depth-of-focus (EDOF) intraocular lens (IOL). Setting: Oculens Clinic, Cluj-Napoca, Romania. Design: Retrospective, single-center, observational study. Methods: 104 eyes of 52 patients undergoing cataract surgery with implantation of the Mini Well Ready EDOF-IOL (SIFI, Catania, Italy) were included in the study. Visual acuity at distance, intermediate and near was evaluated at 1, 6 and 12 months. Refractive results, contrast sensitivity, defocus curve and photic phenomena were also assessed. Results: The mean age of patients was 66.04±7.82. A significant reduction of the spherical equivalent and improvement of corrected distance visual acuity (CDVA) was observed (p<0.0001) after the surgery. All eyes obtained a CDVA of 0.5 or better at one month. All eyes achieved a corrected intermediate visual acuity (CIVA) of 0.5 or better and a corrected near visual acuity (CNVA) of 0.5 or better. At 12 months the spherical equivalent was within -0.5- +0.5 diopters (D). Conclusions: The EDOF-IOL offers good visual outcomes at distance, intermediate and near vision, providing an adequate contrast sensitivity and low rate of visual disturbances.

Objective: The aim of this report is to present the diagnostic and therapeutic approach in a case of long standing peripheral ulcerative keratitis of a patient with already decreased visual acuity due to preexistent optic nerve partial atrophy. Case presentation: A 58-year-old male patient presented to our clinic with unilateral pain, photophobia, tearing and decreased visual acuity in the right eye. The symptomatology began 4 months prior to the presentation, after trauma with a vegetative corneal foreign body, for which he received treatment in another Ophthalmology Service. After every specific cause of peripheral ulcerative keratitis was excluded, using extensive laboratory testing, the diagnosis of unilateral Mooren's ulcer in the right eye was established and the topical steroid treatment was initiated. A very good response to the treatment was observed, with complete resolution of the affliction. Discussions: Mooren's ulcer represents an idiopathic form of peripheral ulcerative keratitis. The diagnostic approach in this type of cases consists in the exclusion of infectious etiology (to safely use topical steroids) and of specific autoimmune etiologies. As the evolution of this pathology is severe with frequent complications, the treatment must be initiated as soon as possible. Conclusions: Despite being a rare diagnosis, Mooren's ulcer must be considered in cases in which clinical and paraclinical presentation is highly suggestive. A rapid therapeutic approach can offer good results despite the usual severe evolution of this disease.

Objective: To report a case of a non-arteritic anterior ischemic optic neuropathy (NAAION) in a patient treated with Sumatriptan. Materials and methods: NAAION represents a severe affection that frequently determines irreversible visual acuity damage. The exact cause is yet to be identified, but is usually connected to the systemic status of the patient. We presented the case of a 53-year-old female patient who complained of visual acuity loss in her right eye, associated with inferior visual field (VF) damage. Patient history revealed migraine attacks, raised arterial blood pressure (BP), mitral valve insufficiency and dyslipidemia. Systemic treatment included Sumatriptan for migraine attacks and Bisoprolol for arterial hypertension. Results: A complete ophthalmologic examination was performed, including a visual field examination and optic coherence tomography. Interdisciplinary consults, along with inflammatory biomarkers, brain scan and cardiovascular Doppler echography were used to establish the final diagnosis. Considering the patient's history, systemic medication, clinical picture, paraclinical findings and interdisciplinary check-ups, NAAION was established as a diagnosis. Discussion: NAAION occurs more frequently after the age of 50 years old and may be associated with systemic factors such as nocturnal hypotension, diabetes, atherosclerosis, sleep apnea. In the present case, the association of medically induced nocturnal hypotension and vasoconstriction led to optic nerve ischemia. Conclusions: In a patient with multiple pathology, we must consider the systemic therapy when performing any clinical examination. {"title":"The pathophysiology of Sumatriptan induced non-arteritic anterior ischemic optic neuropathy.","authors":"Dana Dăscălescu, Cătălina Corbu, Livia Șram, Valeria Coviltir, Mihaela Constantin, Miruna Burcel, Maria Marinescu, Andrei Comber, Vasile Potop","doi":"10.22336/rjo.2022.62","DOIUrl":"10.22336/rjo.2022.62","url":null,"abstract":"<p><p><b>Objective:</b> To report a case of a non-arteritic anterior ischemic optic neuropathy (NAAION) in a patient treated with Sumatriptan. <b>Materials and methods:</b> NAAION represents a severe affection that frequently determines irreversible visual acuity damage. The exact cause is yet to be identified, but is usually connected to the systemic status of the patient. We presented the case of a 53-year-old female patient who complained of visual acuity loss in her right eye, associated with inferior visual field (VF) damage. Patient history revealed migraine attacks, raised arterial blood pressure (BP), mitral valve insufficiency and dyslipidemia. Systemic treatment included Sumatriptan for migraine attacks and Bisoprolol for arterial hypertension. <b>Results:</b> A complete ophthalmologic examination was performed, including a visual field examination and optic coherence tomography. Interdisciplinary consults, along with inflammatory biomarkers, brain scan and cardiovascular Doppler echography were used to establish the final diagnosis. Considering the patient's history, systemic medication, clinical picture, paraclinical findings and interdisciplinary check-ups, NAAION was established as a diagnosis. <b>Discussion:</b> NAAION occurs more frequently after the age of 50 years old and may be associated with systemic factors such as nocturnal hypotension, diabetes, atherosclerosis, sleep apnea. Purpose: The purpose of this study was to compare the results of 4 different autorefractometer devices with the results of retinoscopy in children. Methods: A total of 120 eyes of 60 patients aged between 6 and 18, who applied to Afyonkarahisar Health Sciences University unit of Pediatric Ophthalmology, were included in the study. Refraction with Plusoptix A09 (photoscreener) without cycloplegia was the first to be measured. Spherical and cylindrical values were recorded. Then, half an hour after the patients were instilled 2 drops of cyclopentolate with an interval of 5 minutes, dilated retinoscopy was performed, and spherical and cylindrical values were recorded. Autorefractometer measurements with cycloplegia were performed with Canon RK-F1 autorefractometer, Nidek Tonoref III and Retinomax K-Plus 3, and spherical and cylindrical values were recorded. Results: The mean age of the patients was 11.02 ± 2.1. The mean spherical equivalents were Canon RKF1 autorefractometer +0.045 ± 2.49, Nidek TonoRef III +0.023 ± 2.48, Retinomax K-Plus 3 +0.078 ± 1.42, Plusoptix A09 -0.119 ± 2.20, retinoscopy +0.124 ± 2.65. Moreover, the mean cylindrical values were Canon RK-F1 autorefractometer -0.893 ± 0.69, Nidek TonoRef III -0.927 ± 0.72, Retinomax K-Plus 3 -0.888 ± 0.73, Plusoptix A09 -0.883 ± 0.719, retinoscopy -0.923 ± 0.71. The statistical values compared with retinoscopy; Canon RKF1 spherical equivalent (p=0.376), cylindrical (p=0.515), Nidek TonoRef III spherical equivalent (p=0.485), cylindrical (p=0.198), Retinomax K-Plus 3 spherical equivalent (p=0.141), cylindrical (p=0.058), Plusoptix A09 spherical equivalent (p=0.085) and cylindrical (p=0.086) values were not different. Conclusions: In spherical and cylindrical refractive error detection, all 4 devices showed reasonable and consistent results compared to retinoscopy.

Introduction: Leptospirosis is a neglected and re-emerging zoonotic disease that may cause ocular involvement, uveitis being the main complication of the systemic disease. Aim: The purpose of this review was to raise awareness and update on uveitis caused by leptospirosis, which is a challenging pathology because it can mimic other types of uveitis. Materials and methods: An article review, was conducted by searching PubMed (MEDLINE), Scielo, Cochrane Library databases using the following MeSH and DeCS terms: "leptospirosis", "uveitis", "ocular", "eye" and "human". The inclusion criteria were articles between 2000 and 2021 in English, Spanish, French or Portuguese. Results and Discussion: A total of 49 articles were obtained with the mentioned inclusion criteria, and additionally 5 articles before the year 2000, which were considered due to their relevance and scarcity of articles on the pathology. Afterwards, a description of the disease was made. Conclusion: This literature review was steered to raise awareness and to apprise physicians and ophthalmologists about a pathology that is becoming increasingly relevant, but underdiagnosed, even in developed countries. Objective: To study the demographic and clinical characteristics of childhood and adult onset vernal keratoconjunctivitis in a tertiary care center during Covid pandemic. Methods: A prospective, hospital-based study including a total of 135 patients with Vernal Keratoconjunctivitis (VKC) studied from June 2021 to June 2022 in a tertiary care center in India. Results: Patients presented were in the age range of 1½ to 30 years old. Adult onset VKC included 10.4% of the total patients. The disease was found to be more common in males, with a male to female ratio of 2.5:1. Limbal VKC was the most common presentation found in 61.5% of the patients followed by palpebral type in 28.9% and mixed type in 10.4% of the patients. Severity wise moderate intermittent form of the disease was found to be the most common in 49.6% of the patients. Steroid induced glaucoma was found to be the most common complication in 8.89% of the patients, requiring regular monitoring of intraocular pressure and change in treatment regime, followed by shield ulcer in 2.96% and limbal stem cell deficiency and Keratoconus in 1.48%. Conclusion: This study represents the nature of Vernal Keratoconjunctivitis and its presentation in North India, showing regional variations in the presentation of the disease due to different environmental conditions and the variations in aggression in management protocol. It also emphasizes the regular monitoring of intraocular pressure and the supervision on rising Adult onset VKC.
