Psychological Status of Clinical Laboratory Staff During the COVID-19 Outbreak. COVID-19 爆发期间临床实验室工作人员的心理状况。

Background: In early December 2019, during the outbreak of the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), which was first detected in Wuhan, COVID-19 was suspected, detected, and confirmed in an increasing number of patients every day. The clinical laboratory staff have always played an important role in the laboratory diagnosis of patients. Currently, there are many research studies on the mental health of the first-line doctors or nurses managing the COVID-19 outbreak, both domestically and overseas, but data of the mental health and associated factors among the clinical laboratory staff who handle the blood or biological samples of confirmed cases and are consequently exposed to COVID-19 are limited.

Methods: This cross-sectional survey-based study was performed via an online survey in a single designated hospital from April 20 to April 23, 2020 in Yiwu,China. The online survey included questions on sociodemographic and clinical variables. Totally, 45 clinical laboratory staff and 20 nonmedical health workers participated. Mental health variables were assessed via 4 Chinese versions of validated measurement tools : Zung's Self-rating Depression Scale (SDS), Zung's Self-rating Anxiety Scale (SAS), the Pittsburgh Sleep Quality Index (PSQI), and the Eysenck Personality Questionnaire (EPQ).

Results: Significant differences were observed in the SDS and SAS scores, between the clinical laboratory staff and the nonmedical health workers (P < .001, P < .003, respectively). The scores for exposure risk and neuroticism of participants were the main factors influencing both the SDS scores of the clinical laboratory staff (P = .002, P = .005, respectively), and also their SAS scores (P = .003 P = .006, respectively).

Conclusions: The results showed that a significant proportion of clinical laboratory staff experienced anxiety and depression symptoms. Their scores for mental health problems, exposure risk, and neuroticism were associated with severe symptoms of depression and anxiety. Therefore, the high-risk group of the clinical laboratory staff and those individuals with higher neuroticism scores may need special attention.

Introduction: Pre-eclampsia is one of the major causes of maternal and neonatal morbidity and mortality in the world. The complexity of its etiopathogenesis involves, among other things, age, primi-gravidity, obesity, lack of sensitization to the partner's sperm. Our objective was to describe the socio-clinical profile of the preeclampsia pregnant woman received in consultation in our department and to investigate its influence on certain coagulation parameters (prothrombin level, activated partial thromboplastin time). Methodology: We carried out an analytical cross-sectional study from 01st November 2018 to 31st May 2019, in the gynecology and obstetrics department of the Laquintinie Hospital in Douala. We recruited 150 pregnant women including 50 preeclampsia matched to 100 non-preeclampsia all with a gestational age greater than 20 weeks of amenorrhea. The variables of interest were age, pregnancy, parity, gestational age, marital status and body mass index, Prothrombin Level (PL), and Activated Cephalin Time (ACT). Statistical tests were considered significant for a p-value <0.05. Results: The majority age group in both groups was that of 25-30 years with a mean age of 27.80 ± 5.80 in the pregnant preeclampsia; the latter was predominantly pauci-gravid, nulliparous, and grade 1 obese with a gestational age predominantly ≥ 37 weeks of amenorrhea. In linear regression analysis and unvaried regression analysis, we did not find any socio-clinical factors exposing to coagulation disorders in preeclampsia. Conclusion: Our preliminary study reveals the non-existence of socio-clinical factors exposing to coagulation disorders in preeclampsia in our setting.

Cyclosporine is more selective in its suppression of the immune system than previously available drugs. Although the exact mechanism of action is not yet known, it probably acts by interfering with lymphokine secretion. It primarily inhibits T-lymphocyte responses, although some B cell responses are also affected. Neutrophils and platelet formation are spared. Its efficacy in prolonging the survival of transplanted organs in experimental animals has been a stimulus to the study of graft-host interactions and to the growth of clinical transplantation. In clinical renal transplantations, many, but not all, centers have observed an improvement in graft survival. Nephrotoxicity is common--the average creatinine level of a stable patient on cyclosporine is typically a third again higher than in a patient on conventional therapy. Results of studies up to three years have not shown a further deterioration in renal function, but longer term data are unavailable. Because patients on cyclosporine often have less severe rejection and require lower dosages of corticosteroids, progress in cardiac and hepatic transplantation is being made. The role of blood level monitoring in the avoidance of nephrotoxicity and other side effects remains unsettled. Only further experience with measuring concentrations of the drug in the blood will reveal if it is useful. Numerous drug interactions are important, particularly with aminoglycosides and amphotericin.
