Non-Surgical Management of Iatrogenic Perforation in Mandibular Molar Using Mineral Trioxide Aggregate Followed by Composite Onlay: A Case Report
Henytaria Fajrianti, H. K. A. Nugroho, Yulita Kristanti, D. N. Ratih
One of the most common iatrogenic during endodontic is a perforation, especially furcation perforation on the molar. Perforation creates an artificial communication between a root canal and periodontal tissue, which could lead to pulpal inflammation extends to the periodontium. Management of perforation is a challenging task for clinicians’ inconsideration of poor accessibility and visibility. This case report aims to present a case of furcal perforation treatment by a non-surgical approach. A 24-year-old patient was referred to the Conservative Dental Clinic Prof. Soedomo Dental Hospital to continue root canal treatment on a mandibular first molar (36). The patient had been treated by two dentists before. Clinical examination showed a black area on the floor of the pulp chamber. The radiographic examination showed a little radiolucent area in the furcal region of tooth 36. The palpation test did not reveal any tenderness with no mobility but had percussion pain. An artificial wall was built on the distal wall. Root canal treatment was done by a rotary file with the crown down technique on three canals. Furcation perforation was treated under endodontic microscopic. Mineral trioxide aggregate (MTA) was placed on the perforation by using the MTA carrier. After MTA has set, bulk-fill and fiber-reinforced composite were placed on building up the cavity. Final restoration made by direct-indirect technique with onlay composite, Conclusion, non-surgical approach of perforation treatment could be done by using MTA under good magnification to achieve proper material placement on the perforation. MTA has a good sealability to repair perforation, and this case could be done successfully.

Asthma treatment generally aims to control asthma recurrence so that patients can lead an everyday life with few or no symptoms. In addition, low levels of asthma control are associated with a decreased meaning of quality of life. Therefore, this research aims to determine the study of asthma therapy in terms of the rationality of drug use and to determine the relationship between the control level of asthma and the quality of life of asthma patients at Rumah Sakit Khusus Paru ‘Respira’ Yogyakarta (RSKP Respira Yogyakarta), a lung hospital in Yogyakarta. This observational analytic research used a cross-sectional approach with 30 patients as the respondents. Patient data were collected from November 2019 to February 2020 using Asthma Control Test (ACT) questionnaire to measure the level of asthma control and Mini Asthma Quality of Life Questionnaire (MINI AQLQ) to measure asthma patients' quality of life. Analysis of the relationship between the patients’ control level of asthma and the patients’ quality of life used the Chi-Square test. The results showed the rational use of medicine in asthma outpatients at RSKP Respira Yogyakarta was that 30 patients (100%) had the right indication, 27 patients (90%) received the right drug, 28 patients (93,33%) were the right patients, 30 patients (100%) had the right dose, and no side effects were found (100%). Overall, there were 83.33% of patients who met the requirements for the rationality of drug use. A p-value of <0.05 on the Chi-Square test indicated that there was a relationship between the level of asthma control and the quality of life of asthma outpatients at RSKP Respira Yogyakarta.

Inflammation is an important biological process for eradicating pathogens and maintaining tissue homeostasis. Cyclooxygenase 1 (COX-1) is one of the pharmacological targets for anti-inflammatory drugs. The purposes of this study was to predict the affinity of 23 compounds contained in Hemigraphis alternata leaves in inhibiting COX-1. The compounds of Hemigraphis alternata leaves were screened for its affinity towards COX-1 using docking software, DOCK 6.9, with aspirin as the reference. Based on the Grid score, the greatest binding affinity is referred to 3,7,11,15-tetramethyl-2-hexadecen-1-ol. It is better than affinity of aspirin to COX-1. The study showed that Hemigraphis alternata leaves contain potential active components that could be developed as COX-1-inhibitor.

According to Indonesia Basic Health Research 2018, the prevalence of mental emotional disorders which is indicated by depression and anxiety symptoms reached 9.8% of the total households in Indonesia. Screening of mental emotional disorders in a population at risk needs to be done. Therefore, people with mental emotional disorders can be handled promptly and appropriately. The purpose of this study was to determine the prevalence of mental emotional disorders in risk community through screening in the community as well as to find the related factors. This research was a descriptive-analytic study with a cross-sectional method which was conducted from January to April 2019. The subjects of this study were 104 people taken from 4 NCD-integrated health care post (Posbindu PTM), in Kasihan 2 Community Health Centre, Bantul District, Special Region of Yogyakarta, Indonesia. Inclusion criteria were 18-59 years old, cooperative, and willing to participate in the study. Exclusion criteria included having a serious physical illness, disability, comorbidity with substance abuse, or other serious mental disorders. The screening applied a selfreporting questionnaire (SRQ) 20. The data was processed using a computer program, SPSS. Descriptive data were presented categorically using the distribution frequency of subjects' characteristics and percentages of mental emotional disorders. Bivariate analysis using the chi-square test was used to evaluate the correlation among characteristic factors of the subjects and mental emotional disorders. Most subjects were in the age of 41-50, female, unemployed, and high school graduate. A total of 29 people (27.9%) of the subjects had mental emotional disorders, 45 people (43.3%) were easily feeling tired, and 35 people (33.7%) feel worried, tense, and anxious. There was one person who had a suicide idea. The factor associated with mental emotional disorders was the level of education (pvalue = 0.05). Mental emotional disorders were experienced by 27.9% of the subjects in this study with the most symptom was easily feeling tired. Screening of mental health disorders should be performed periodically in the community and primary care.

Background: Inflammation plays an important role in the pathogenesis of COVID-19 and can be an important factor in the prognosis. The innate immune response to a respiratory infection is characterised by an influx of neutrophils to the lungs, particularly the alveoli. A sustained release of anti-inflammatory cytokines may lead to widespread apoptosis of lymphocytes, resulting in lymphopenia. Hs-CRP is an indicator of COVID-19 inflammation. The aim of this study was to analyse Neutrophilto-Lymphocyte Ratio (NLR) and Hs-CRP in order to predict mortality in COVID-19 patients. Research methods: This research was conducted at Moewardi Hospital in July 2020. The subjects were COVID-19 patients. NLR is neutrophils divided by lymphocytes using a haematology analyser. The examination of Hs-CRP was conducted utilizing the ELISA method. Statistical test with an independent T-test and cox proportional hazard analysis with p <0.05 was also used. Results: The results showed that Hs-CRP and NLR increased in COVID-19 patients who later died and obtained lower levels of surviving (p = 0.001 and p = 0.11). Hs-CRP increased the risk of death for COVID-19 patients (p = 0.18;HR = 1.036) and NLR also increased the risk of death for COVID-19 patients (p = 0.08;HR = 1.34) Conclusion: Neutrophil-to-Lymphocyte Ratio and Hs-CRP predicted mortality in COVID-19 patients

Drug-drug interactions increase the effectiveness of therapy but can also have undesirable effects on toxicity. A pharmacist has the responsibility to prevent the undesirable effect of drug-drug interactions. The purpose of this study was to evaluate the potential drug-drug interactions in hypercholesterolemia patients at Teaching Hospital Surabaya. This study was conducted with a prospective method for a month. We obtained 54 patients and identified 45 patients that have potential drug-drug interactions. The total potential drug-drug interactions were 101. The severity of potential drug-drug interactions is serious (24,7%) and moderate (75,3%). Based on the mechanism action of drug interactions are pharmacodynamics (59,4%), pharmacokinetics (32,7%), and unknown (7,9%). The role of the pharmacist in providing safe, effective and rational drug therapy can be done by either identifying drug interactions, prevent and minimize the negative effect of drug interactions.
