A clinical and radiographic evaluation of immediately loaded fixed full-arch prosthesis supported by implants placed in extraction sockets and healed ridges using All-on-4/All-on-6 protocol: A 2-year follow-up. 采用All-on-4/All-on-6方案立即加载固定全弓假体的临床和影像学评估,该假体由放置在拔牙槽内的种植体和愈合的脊支撑:2年随访。

Aim: The aim of this study was to evaluate immediately loaded implants, both clinically and radiographically, in extraction sites (IPIL) and healed sites (HSIL), rehabilitated using All-on-4/All-on-6 protocol, over a time span of 2 years.

Setting and design: The study proceeded under following phases of patient selection, placement of implants followed by immediate loading and then statistically analyzing the collected data.

Materials and methods: A total of 15 patients were evaluated, in 2 years, for study purpose. Among 87 immediately loaded implants, 44 were IPIL and 43 were HSIL. The bone level was measured using Implant abutment junction as reference point, at 0, 6, 12, 18 and 24 months. Bone loss was calculated as difference of mean.

Statistically analysis: The collected data was statistically analyzed using independent t-test (P < 0.05).

Results: The average bone loss for 2 years was 1.39 mm and 1.17 mm, in IPIL and HSIL group respectively with 100% implant survival rate and no statistical difference (P < 0.05).

Conclusion: Full arch prostheses can be successfully delivered by immediate loading of implants regardless of placement (healed or extraction sites).

Evaluation of horizontal condylar guidance on a fully adjustable articulator at varying protrusive distances: An in vivo study. 评估水平髁引导的完全可调关节在不同的突出距离:一项体内研究。

Aim: This study aimed to assess the accuracy of HCG values obtained with records at various protrusive distances on a fully adjustable articulator.

Settings and design: Pantographs are used for condylar settings on a fully adjustable articulator but are complex and time-consuming. Protrusive interocclusal records can be used to set horizontal condylar guidance (HCG) in fully adjustable articulators; however, the ideal protrusive distance to do so remains unclear. This was an in vivo observational cross-sectional study.

Materials and methods: Twenty-five dentate subjects participated in this study. Customized guides with calibrations at 2, 4, 6, and 8 mm were used to create protrusive interocclusal records at each increment. The HCG values derived from these records were set on a fully adjustable articulator (Denar D5A, Whip Mix Corp.). For control, standardized digital lateral cephalograms of each subject were taken in edge-to-edge and maximal intercuspal positions. These cephalograms were superimposed using Dolphin Imaging software (Dolphin Imaging and Management Solutions), allowing the determination of the cephalometric HCG angle by measuring the intersection between a line passing from the Condylion and the Frankfort horizontal plane. The HCG values from the articulator were compared with the cephalometric HCG values.

Statistical analysis used: Statistical analysis was conducted using repeated measures ANOVA for intragroup comparison and one-way ANOVA with Bonferroni post hoc tests for intergroup comparisons. The Shapiro-Wilk test was conducted to assess normality.

Results: The mean HCG values obtained at 2, 4, 6, and 8 mm were 49.92°, 44.28°, 36.60°, and 26.60°, respectively, while the cephalometric HCG value was 44.51°. Analysis revealed no significant difference between the cephalometric HCG and the 4 mm record (P > 0.05), but significant differences were noted at 2, 6, and 8 mm (P < 0.05).

Conclusions: The HCG set on the Denar D5A articulator using a 4 mm protrusive interocclusal record closely matched the cephalometric HCG, suggesting that a 4 mm record may be optimal when using protrusive records for programming this articulator.

Purpose: Effect of an erosive challenge on the surface, optical, and mechanical properties of prostheses manufactured using different types of dental ceramics for computer-aided design/computer-aided manufacturing (CAD/CAM).

Materials and methods: Protocol followed PRISMA guidelines. A PICO question was formulated: "What is the effect of gastric juice on the surface, optical and mechanical properties of different types of dental ceramics made using the CAD/CAM system?." PubMed/MEDLINE, EMBASE, Web of Science, Scopus, Cochrane Library, and ProQuest was search to July 2024. The quality of the full-text articles was evaluated for Joanna Briggs Institute critical. The meta-analysis was the inverse variance method (P < 0.05).

Results: After applying the selection criteria, 15 in vitro studies published between 2014 and 2022, 777 specimens were included. Fourteen studies were included in meta-analysis, which no significant difference in the surface roughness after the erosive challenge was observed between the ceramic types (P = 0.59; MD: 0.14; 95% confidence interval [CI]: -0.36-0.64; I2 = 86%, P < 0.01), significantly less color change was observed without the erosive challenge (P < 0.05; MD: 1.82; 95% CI: 0.86-2.79; I2 = 81%, P < 0.01), and significantly lower microhardness was observed for the groups with the erosive challenge (P = 0.03; MD: -0.95; 95% CI: -1.82--0.08; I2 = 88%, P < 0.01).

Conclusions: The ceramics showed changes in roughness, color stability, microhardness and flexural strength, depending on the type of ceramic.

Aim: This study aimed to evaluate the clinical efficacy of bioactive glass as a desensitizer with dentin bonding agent in preventing the postvital tooth preparation sensitivity at three different time periods.

Settings and design: An Observational Study.

Materials and methods: 39 participants of age 25-30 years requiring vital tooth preparation were divided into Group A as control, distilled water as placebo; Group B test control, 7th generation dentin bonding agent; and Group C test, bioactive glass. Dentin hypersensitivity was evaluated on the day of preparation (T0), 7th day after preparation (T1), and 15th day (T2) at the final cementation procedure using the Schiff scale.

Statistical analysis used: IBM SPSS statistics software, One way analysis of variance (ANOVA), Post hoc Bonferroni.

Results: The descriptive statistics showed the mean and standard deviation at preapplication (T0) to be 2.23 ± 0.83, 2.53 ± 0.66, and 2.61 ± 0.65 for Group 1, Group 2, and Group 3, respectively, with no significant difference between the groups (P > 0.05). Intergroup analysis revealed there was a statistical difference between the 3 groups at T1 and T2, with post hoc Bonferroni depicting a lower mean for Group 3 of 1.53 ± 0.87 and 0.76 ± 1.09 at T1 and T2, respectively.

Conclusion: Bioactive glass has a significant reduction in the dentinal hypersensitivity on the vital prepared tooth compared to both negative control and dentin bonding agent.

Accurate impressions are an essential prerequisite for fabricating implant-retained maxillofacial prostheses. This article describes an impression technique for recording orbital defects by making increments for an implant-retained orbital prosthesis using an open-tray impression. This technique enables an accurate transfer of implant position and recording of the area of interest. A wide range of impression-transfers, including dental implant impression posts, can also be used.

Aim: The aim is to evaluate and compare stress distribution characteristics of ball, magnet, and positioned attachment systems in single and double implant-retained overdentures using the finite element method (FEM).

Setting and design: In vitro (in silico study) finite element analysis (FEA).

Materials and methods: A Styrofoam mandible with duplicated silicon mucosa was used to construct a mandibular complete denture. Implants of size 12 mm by 3.4 mm body diameter (Implantium, Dentium, Korea) were used along with ball O-ring, positioner, and magnet attachment system for constructing single and double implant overdenture. FEA models were processed defining geometry. Six models were formulated, namely, three models of central single implant and three for double implant overdenture using ball O-ring attachment, positioner attachment, and magnet attachment named B1, P1, M1, B2, P2, and M2, respectively. The FEM analysis was done using ANSYS workbench 16.0. A load of around 100N was applied bilaterally in the first molar regions, anteriorly between the two central incisors, and unilaterally on the right first molar region. Stresses were calculated according to the Von Mises stresses scale. Stress magnitude and patterns were analyzed and interpreted accordingly.

Statistical analysis used: Stress patterns and maximum principal stress were described.

Results: FEM analysis revealed that stress concentration in the B1 model was the highest. Anterior loading produced maximum stress followed by unilateral and lastly bilateral loading. In the bone, stress concentration was the highest in the crestal bone region (3-18 MPa). Mucosa showed the least amount of stress in all models (1.3-3 MPa). Stress patterns in dentures were symmetric with concentration on the mid-lingual and molar premolar area (3-18 MPa). Implants showed stress concentration near the collar and the internal connection region (7.5-24 MPa). Maximum stresses were around the head of prosthetic abutments (10.5-24 MPa). The stress concentration in the ball head was typically located in the abutment neck. In positioner attachment, stress was located in the inner open end area (11-20.1 MPa). For the keeper, stresses were at its base and stress patterns extended into the body (10.5-17.7 MPa). Less stress magnitude was seen for all models on bilateral loading (10.5-15 MPa).

Conclusion: Magnet and positioner attachments have a more favorable stress distribution in both single and double implant-retained overdentures. Bilateral loading conditions are more favorable for uniform stress patterns.

Aim: The aim of this study was to investigate and compare the antimicrobial effects of an 810-nanometer diode laser, utilizing or not utilizing toluidine blue as a photosensitizer, in the management of peri-implant mucositis.

Settings and design: The present study was carried out in 30 implant sites in 15 patients with peri-implant mucositis with a specific inclusion and exclusion criteria. 15 sites were treated utilizing a diode laser (control group) and 15 with photodynamic therapy (test group) in a split-mouth format.

Materials and methods: Samples were taken from the gingival sulcus with the help of plastic curettes from control and test sites both at baseline and at 3 months for microbiological analysis.

Statistical analysis used: Shapiro-Wilk Test was used to check deviation from normality. Wilcoxon signed-rank test was used to analyse the two dependent groups.

Results: Intragroup comparison was found to be statistically significant when compared at baseline and at 3 months in the photodynamic (P = 0.001) and diode laser groups (P = 0.001), respectively. No statistically significant reduction in bacterial count was found at baseline (P = 0.1) and at 3 months (P = 0.5) when the diode laser group and photodynamic group were compared with each other (intergroup).

Conclusion: Within the limitations of the study, it can be concluded that there is a definitive reduction in pathogenic bacteria with both interventions and PDT offers clinically visible benefits in the treatment of peri-implant mucositis.

Introduction: Accurate implant impressions are crucial for successful prosthetic rehabilitation. Digital impressions using intraoral scanners (IOS) have emerged as an alternative to conventional techniques. Various factors influence the accuracy of digital implant impressions, including scan body design, scanning protocol, and splinting techniques.

Aim and objective: To evaluate the difference between splinted and nonsplinted scan bodies in the accuracy of digital impressions for single or multiple implants by measuring distance and angular deviations between superimposed digital impressions.

Materials and methods: PRISMA guidelines were followed in this systematic review. Electronic databases were searched for relevant studies published up to January 2024. Inclusion criteria encompassed clinical trials, in vivo and in vitro studies on scan bodies in partially or fully edentulous arches. Two reviewers independently assessed abstracts, titles and full texts. Data extraction included distance deviation, angular deviation, trueness, and precision measurements.

Discussion: Most studies found that scan body splinting improved the accuracy of digital implant impressions, particularly for complete-arch cases. Splinting techniques varied, including light-cured resin, pattern resin, dental floss, and custom-made splints. Factors such as the inter-implant distance, number of implants and choice of IOS also influenced accuracy. However, some studies reported no significant improvement or even negative effects of splinting on accuracy.

Conclusion: Splinting scan bodies generally improves the accuracy of digital implant impressions, especially for complete-arch cases. It enhances the stitching process in digital workflows and provides more stable reference points. However, the effectiveness may vary depending on the specific clinical situation, scanning protocol, and IOS used. Further research is needed to establish standardized protocols and evaluate long-term clinical outcomes of digitally fabricated implant restorations based on splinted scan body impressions.

Hence, this study aims to compare the short-term efficacy of occlusal splint therapy and the synergistic effect of therapeutic exercise with occlusal splint therapy for 3 weeks in individuals with TMD.

Settings and design: in-vivo observational pilot study.

Materials and methods: Fourteen participants were grouped based on the type of treatment: Group A, assigned with combination therapy, stabilization splint with therapeutic exercises, and Group B, with only stabilization splint therapy. Surface electromyography (EMG) was taken to evaluate the masseter and temporalis muscle activity at baseline and 2nd week. The patient's perception of TMD was evaluated using the Fonseca questionnaire at baseline, 48 h, 14th, and 21st days.

Statistical analysis: Chi-square statistical analysis was applied to the quantitative data obtained from the Fonseca questionnaire and EMG analysis, with P < 0.05 as significant.

Results: Fonseca questionnaire revealed a significant difference between the two groups at 48 h (P < 0.05). EMG outcome showed no significant difference between the groups at baseline and 14th day (P > 0.05). However, the mean muscle activity recorded at the masseter in Group B was increased on 14th day (56.5 ± 3 μV) and an isotonic muscle pattern was observed in Group A.

Conclusion: Combination therapy, which included stabilization splint and home care exercises for moderate to severe TMD patients, led to earlier curative changes in the temporomandibular joint and improved functional jaw movements, along with a reduction in pain. An isotonic muscle activity pattern was also observed, confirming the effect of combination therapy.
