Comparison of the nutritional indicators of critically Ill patients on extracorporeal membrane oxygen (ECMO) 重症患者体外膜氧(ECMO)营养指标的比较

Purpose: This study aimed at investigating the nutritional status, nutritional support, and nutritional indicators of critically ill patients on extracorporeal membrane oxygen (ECMO) in intensive care units (ICU). Methods: This descriptive study obtained data from the medical records of 37 patients who were treated using ECMO at a university hospital in Korea. The patients were admitted to the Korea University Anam Hospital ICU with acute or serious damage to vital organs from January 1, 2014 to June 30, 2016. Results: Although 32 patients (86.5%) were at a high risk of malnutrition, 26 patients (70.3%) were considered normal in terms of nutritional status by the nutritionist at the beginning of their ICU stay. However, after two weeks, nine patients had passed away and only one patient maintained normal status. Parenteral nutrition was started first but took 4.25 ± 6.95 days till initiation. Only eight patients (21.6%) were able to meet their requirement for both calories and protein. The group provided with adequate calorie and protein showed significantly longer use of the ECMO and respirator and longer ICU and total hospital stay than their counterparts. Normal levels of serum albumin and protein of the group at low-risk for malnutrition on the day of initiation of ECMO, which were significantly higher than the high-risk group, declined by the last day of ECMO leading to a lack of significant differences between the two groups. Conclusion: Considering that the nutritional indicators of patients deteriorated as the days on ECMO increased, more aggressive nutritional management to ensure adequate nutritional support should be emphasized from the beginning and throughout the ICU stay.

Anti-obesity effects of Tenebrio molitor larvae powder in high-fat diet-induced obese mice 黄粉虫幼虫粉对高脂饮食诱导肥胖小鼠的抗肥胖作用

Purpose: Obesity is a serious public health issue for the modern society and is considered a chronic health hazard. There are many surgical and pharmacological approaches to treat obesity. However, various potentially hazardous side effects remain the biggest challenge. Therefore, diets based on foods derived from natural products have gained increasing attention compared to anti-obesity drugs. Recently, research on edible insects as a food source has been a topic of considerable interest in the scientific communities. This study examined the anti-obesity effects of ingesting an edible insect by feeding a high-fat diet (HFD)-induced obese mouse models with a diet containing Tenebrio molitor larvae powder (TMLP). Methods: Six-week-old female C57BL/6J mice were divided into 4 groups according to treatment: 100% normal diet (ND), 100% HFD (HFD), HFD 99% + TMLP 1% (TMLP), and HFD 97% + TMLP 3% (TMLP 3%). TMLP was added to the HFD for 6 weeks for the latter two groups. Results: Compared to the HFD group, mice in the TMLP group showed weight loss, and micro-computed tomographic imaging revealed that the volume of the adipose tissue in the abdominal area also showed significant reduction. After an autopsy, the fat weight was found to be significantly reduced in the TMLP group compared to the HFD group. In addition, the degree of fat cell deposition in the liver tissue and the size of the adipocytes significantly decreased in the TMLP group. Reverse transcription polymerase chain reaction analysis for the mRNA expression of adipogenesis-related genes namely CCAAT-enhancer-binding proteins (C/EBP- β , C/EBP- δ ), and fatty acid-binding protein 4 (FABP4) showed that the expression levels of these genes were significantly reduced in the TMLP group compared to the HFD group. Serum leptin level also decreased significantly in the TMLP group in the comparison with the HFD group. In addition, total cholesterol, triglyceride, and glucose levels in mouse serum also decreased in the TMLP group. Taken together, our results showed that TMLP effectively inhibited adipocyte growth and reduced body weight in obese mice.

Association between depression and eating behavior factors in Korean adults: the Korea National Health and Nutrition Examination Survey 2018 韩国成年人抑郁与饮食行为因素之间的关系:2018年韩国国家健康与营养检查调查

Purpose: This study aimed to examine the association between depression and eating behavior factors in Korean adults. Methods: Study subjects were selected (n = 5,103) from the participants of the 7th Korea National Health and Nutrition Examination Survey 2018 and divided into depression (men, 59; women, 162) and normal groups (men, 2,083; women, 2,799). Subjects with a Patient Health Questionnaire 9 score ≥ 10 (out of 27 points) were defined as having depression. Results: A higher prevalence of depression was observed in both men and women who were unemployed (p < 0.001, p = 0.008), had lower income (both p < 0.001), poorer subjective health (both p < 0.001), and poor food safety (both p < 0.001). The prevalence of depression was higher in women with lower education levels (p = 0.008), who were unmarried (p = 0.010), smokers (p < 0.001), and in a one-person household (p = 0.001). Obese men showed a higher prevalence of depression (p = 0.009). Men who were eating alone or skipping lunch had a high prevalence of depression (p = 0.009), while women who were eating breakfast (p = 0.012), lunch (p = 0.001), and dinner (p = 0.010) alone had a high prevalence of depression. The relationship analysis between men and women according to dietary habits using logistic regression showed that, in women, after variable adjustment, skipping lunch (odds ratio [OR], 2.677; 95% confidence interval [CI], 1.090–6.574), meal frequency of 2 times per day (OR, 1.658; 95% CI, 1.084–2.536), and lunch frequency of 3–4 times per week (OR, 3.143; 95% CI, 1.725–5.728) were significantly associated with a higher prevalence of depression. Conclusion: Depression in women was not only affected by more sociodemographic variables but also associated with decreased frequency of lunch and dinner, especially with skipping lunch.

Purpose: This study examined general characteristics, health status, accessibility to medical services, health-related quality of life, dietary behavior, and energy and nutrient intakes of the elderly at different levels of food security utilizing data from the 7th Korea National Health and Nutrition Examination Survey (2016–2018). Methods: The elderly subjects (1,721 males and 2,271 females) were divided into 3 groups (secure, mildly insecure, moderately/severely insecure) according to their food security levels. Health and nutrient status was determined using energy intake, nutrient density, the prevalence of insufficient nutrient intake, dietary behavior, and health status. Results: The elderly with food insecurities had a lower self-evaluated health status and a higher prevalence of physician-diagnosed chronic diseases such as arthritis, osteoarthritis, rheumatoid arthritis, osteoporosis for males, and hypertension, stroke, arthritis, and osteoarthritis for females. The associated financial burden was the major reason for not accessing medical services in the food insecure group. Agrifood consumer competency index and food consumption behaviors based on the 2019 Consumption Behaviors Survey for Food 基于2019年食品消费行为调查的农产品消费者能力指数与食品消费行为

Purpose: This study investigated the food consumption behaviors in Korean adults, according to the agrifood consumer competency index (ACCI). Methods: Data obtained from the 2019 Consumption Behaviors Survey for Food were analyzed. A total of 6,176 adults (2,783 males, 3,393 females) aged ≥ 19 years, were included in the study. Based on the score of agrifood consumer competency index, the subjects were classified into three groups. The dietary habits, eating-out and food-delivery/take-out behaviors, opinion of food labeling, and concerns for domestic products were compared among the 3 groups. Results: The ACCI scores of the male and female subjects were 63.6 and 64.8, respectively. Subjects of both genders in the highest tertile of the ACCI were more likely to have a higher education level and higher health concerns, as compared to subjects in the lowest tertile (p < 0.05). Male subjects having highest tertile of the ACCI reported significantly more exercise and alcohol consumption, as compared to subjects in the lowest tertile (p < 0.05). A higher score of the ACCI also portrayed a higher satisfaction in own diet and greater checking of the food label. Moreover, subjects with a higher score of the ACCI showed greater satisfaction and reliability in the food label, as well as increased concerns for domestic agrifoods, local foods, and eco-friendly foods. Subjects in the lowest tertile of the ACCI acquired their dietary information from acquaintances, whereas subjects in the highest tertile of the ACCI learnt the information from food labels themselves. A higher score of the ACCI also portrayed a higher satisfaction in own diet and greater checking of the food label. Moreover, subjects with a higher score of the ACCI showed greater satisfaction and reliability in the food label, as well as increased concerns for domestic agrifoods, local foods, and eco-friendly foods. Subjects in the lowest tertile of the ACCI acquired their dietary information from acquaintances, whereas subjects in the highest tertile of the ACCI learnt the information from food labels themselves. Development of physical activity classification table for Koreans: using the Compendium of physical activities in the United States 韩国人体育活动分类表的编制:以美国体育活动纲要为例

To set the estimated energy requirement (EER) in Dietary Reference Intakes for Koreans (KDRI), we need the coefficient by physical activity stage, as determined by the physical activity level(PAL). Thus, there has been demand for a tool to calculate PAL based on the physical activity diary. This study was undertaken to develop a physical activity (PA) classification table for Koreans, using the 2011 Compendium of physical activities in the United States. The PA classification table for Koreans contains 262 codes, and values of the metabolic equivalent of task (MET) for specific activities. Of these, 243 PAs which do not have Korean specific data or information, were selected from the 2011 Compendium of PAs that originated in the United States; another 19 PAs were selected from the previous research data of Koreans. The PA classification table is codified to facilitate the selection of energy values corresponding to each PA. The code for each PA consists of a single letter alphabet (activity category) and four numeric codes that display the activity type (2 digit number), activity intensity (1 digit number), and specific activities (1 digit number). In addition, the intensity (sedentary behavior, low, middle and high) of specific PA and its rate of energy expenditure in MET are presented together. The activity categories are divided into 4 areas: Daily Activity (A), Movement (B), Occupation (C), and Exercise and Sports (D). The developed PA classification table can be applied to quantify the energy cost of PA for adults in research or practice, and to assess energy expenditure and physical activity levels based on self-reported PA.

Thermal Inactivation of Myrosinase from White Mustard Seeds 白芥菜籽黑芥子酶的热失活研究

Myrosinases (thioglucosidases) catalyze the hydrolysis of a class of compounds called glucosinolates, of which the aglycones show various biological functions. It is often necessary to minimize the loss of myrosinase activity during thermal processing of cruciferous vegetables. Myrosinase was isolated from a popular spice, white mustard (Sinapis alba), and its thermal inactivation kinetics was investigated. The enzyme was extracted from white mustard seeds and purified by a sequential processes of ammonium sulfate fractionation, Concanavalin A-Sepharose column chromatography, and gel permeation chromatography. At least three isozymes were revealed by Concanavalin A-Sepharose column chromatography. The purity of the major myrosinase was examined by native polyacrylamide gel electrophoresis and on-gel activity staining with methyl red. The molecular weight of the major enzyme was estimated to be 171 kDa. When the consecutive step model was used for the thermal inactivation of the major myrosinase, its inactivation energy was 44.388 kJ/mol for the early stage of destruction and 32.019 kJ/mol for the late stage of destruction. When the distinct two enzymes model was used, the inactivation energy was 77.772 kJ/mol for the labile enzyme and 95.145 kJ/mol for the stable enzyme. The thermal inactivation energies lie within energy range causing nutrient destruction on heating.

Purpose: This study was undertaken to evaluate the health, nutritional status and metabolic syndrome risk according to the dietary pattern of adult single-person households, using information obtained from the Korea National Health and Nutrition Examination Survey (KNHANES). Methods: Data were collected from the 2013–2016 KNHANES, of adults aged 19–64 years, belonging to single-person households. Based on cluster analysis, the dietary patterns of subjects were classified into three groups. The dietary behavior factors, health-related factors, nutritional status, and prevalence of metabolic syndrome obtained from KNHANES questionnaires were compared according to the individual dietary pattern. The nutrient intake data of the subjects were calculated using the semi-food frequency questionnaire. Moreover, blood and physical measurement data of the subjects were analyzed to obtain the prevalence of metabolic syndromes. Results: The major dietary intakes of subjects were classified as ‘Rice and kimchi’, ‘Mixed’, and ‘Milk·dairy products and fruits’ patterns. Characteristics of subjects based on their dietary pattern, gender, age, and education level were significantly different. The ‘Milk and fruits’ pattern showed low frequency of skipping breakfast and eating out, and had higher intake of dietary supplements. The estimation of cholesterol intake in elderly: reliability and validity of short, Semi-Quantitative Food Frequency Questionnaire (SQ-FFQ) 老年人胆固醇摄入量估算:短时间半定量食物频率问卷的信度和效度

Purpose: High intake of cholesterol leads to cardiovascular disruption. Estimating the actual intake of cholesterol can be beneficial for nutrition intervention. This research aimed to develop Semi-Quantitative Food Frequency Questionnaire (SQ-FFQ) to estimate cholesterol intake and analyze its reliability and validity. Methods: SQ-FFQ was developed by sorting high cholesterol food items in Indonesian food database and food items' availability. A total of 30 older adults were randomly chosen from Public Health Center in Jagir District, Surabaya, Indonesia to test its validity. Reliability test was done by measuring the same developed SQ-FFQ in one-month period, while validity test was done by comparing SQ-FFQ results with 6-days food record. Statistical analysis used for reliability test was paired t-test, the Intra-class Correlation Coefficient (ICC), and Cronbach's α to measure the internal consistency. Meanwhile, validity of developed SQ-FFQ was analyzed using paired t-test and Bland-Altman. Results: Reliability of 2 administered SQ-FFQs showed a good agreement based on paired t-test analysis (p = 0.200), ICC (0.609), and Cronbach's α (0.757). Strong agreement was found in most of food items, but agreements for egg yolk and fried duck were poor. Significant difference was found between those food items (p = 0.001 vs. p < 0.001, respectively) with mean difference were −25.3 mg and 46.2 mg. Validity of developed SQFFQ2 compared to 6-days food diary records also found a strong agreement based on paired t-test and the Bland-Altman analysis. Conclusion: This baseline research provides a reasonably valid and repeatable measure of cholesterol intake estimation that can be widely used in nutrition and public health study, especially in Indonesia. No study has been conducted in Indonesia on the development of tools to estimate the cholesterol intake.

Statistical analysis used for reliability test was paired t-test, the Intra-class Correlation Coefficient (ICC), and Cronbach's α to measure the internal consistency. Meanwhile, validity of developed SQ-FFQ was analyzed using paired t-test and Bland-Altman. Results: Reliability of 2 administered SQ-FFQs showed a good agreement based on paired t-test analysis (p = 0.200), ICC (0.609), and Cronbach's α (0.757). Strong agreement was found in most of food items, but agreements for egg yolk and fried duck were poor. Significant difference was found between those food items (p = 0.001 vs. p < 0.001, respectively) with mean difference were −25.3 mg and 46.2 mg. Validity of developed SQFFQ2 compared to 6-days food diary records also found a strong agreement based on paired t-test and the Bland-Altman analysis. Conclusion: This baseline research provides a reasonably valid and repeatable measure of cholesterol intake estimation that can be widely used in nutrition and public health study, especially in Indonesia.
