Definition of pelvic venous disease and the new SVP classification 盆腔静脉疾病的定义和新的SVP分类

Chronic pelvic pain (CPP) is often diagnosed by gynecologists. In recent years, the realization that venous insufficiency in the pelvic or retroperitoneal area can also cause CPP has led to the involvement of vascular physicians in the diagnosis and treatment. Differentiating pelvic venous disease (PeVD) from gynecological, internal or even orthopedic complaints poses a challenge. A comprehensive study of the patient"s medical history and a detailed description of symptoms are of paramount importance. The symptoms of PeVD depend on the veins involved. Whereas pelvic pain is mainly an expression of an insufficiency of the ovarian vein and/or the internal iliac vein, left flank pain and hematuria are caused by an obstruction of the renal vein. Symptoms may also be associated with atypical varicose veins in the vulva, perineum, or legs. Anatomical gaps, pelvic escape points, allow them to communicate with the branches of the insufficient internal iliac veins and with the ovarian. As an effective tool for reporting PeVD patient populations the symptoms-varicespathophysiology (SVP) classification was published in 2021. It is thought to be the initial point of both the treatment standard and identifying homogenous patient populations for clinical trials.

The role of fetuin-A in vascular aging in the rat aorta 胎蛋白a在大鼠主动脉血管老化中的作用

Objectives: In this study, we aimed to investigate the relationship between vascular system changes due to the increased inf lammatory response and oxidative stress with advanced age and fetuin-A levels in aortic tissues of a naturally aged rat model. Materials and methods: A total of 16 Wistar albino rats were equally divided into two groups as the young group and the elderly group. The thoracic aorta was excised. The effect of aging on the aorta, proliferation, oxidative stress, and inf lammation markers were evaluated using the real-time polymerase chain reaction method. Histological examination was performed to confirm the findings related to aging, and serum sampling was performed to determine fetuin-A levels. Results: Interleukin-6 levels were lower in the elderly group (1.007 vs. 0.099-fold decrease, respectively; p=0.000). There was a moderate, positive correlation between fetuin-A and interleukin-6 levels (r=0.56; p=0.03). There was no significant difference in the antioxidant capacity as assessed by superoxide dismutase-1, while the oxidative stress markers were significantly higher in elderly rats (inducible nitric oxide synthase: 1 vs. 812.3-fold increase, respectively; p=0.006; endothelial nitric oxide synthase: 0.98 vs. 3.65-fold increase, respectively; p=0.001). There was a moderate, negative correlation between fetuin-A and endothelial nitric oxide synthase (r=-0.56; p=0.024). Conclusion: Vascular senescence causes cell damage through inf lammatory responses. Fetuin-A may indicate early vascular damage.

Mid- and long-term outcomes of extra-anatomic bypass procedures 解剖外旁路手术的中期和长期结果

Objectives: In this study, we report our midand long-term results of extra-anatomic bypass surgery. Patients and methods: Between January 2005 and December 2011, a total of 65 patients (54 males, 11 females; mean age: 54.3±4.2 years; range, 20 to 82 years) with extra-anatomic bypass surgery with complete follow-up data were retrospectively analyzed. Extra-anatomic bypass surgery was performed to the lower extremity in 36 (55.3%) and the upper extremity in 29 (44.7%) patients. Additional distal bypass surgery was done in 18 of 36 patients with lower extremity disease. Results: The median follow-up time was 3.6 years (range, 3 months to 6 years). There was no early mortality, while there were five reoperations due to graft thrombosis and two amputations due to insufficient arterial f low in the postoperative period. There were two reoperations due to graft thrombosis and graft infection as late morbidity. In the long-term, the mortality rate was 6% (n=4) related to the malignancy in two patients, pulmonary embolism in one patient, and aortoduodenal fistula in one patient. Conclusion: An extra-anatomic bypass procedure is a surgical approach that can still be performed with low mortality and morbidity rates and acceptable patency rates in high-risk patients.

Endovascular treatment versus femoropopliteal bypass surgery for TASC II type C lesions of the superficial femoral artery 股浅动脉TASCⅱ型C型病变的血管内治疗与股腘动脉搭桥手术比较

Objectives: In this study, we aimed to compare the outcomes of endovascular treatment (ET) and femoropopliteal artery bypass (FPB) in patients with Trans-Atlantic Inter-Society Consensus (TASC) II type C femoropopliteal lesions. Patients and methods: A total of 149 patients with symptomatic TASC II type C femoropopliteal lesions who underwent invasive treatment between January 2012 and January 2017 were retrospectively analyzed. The patients were divided into two groups as the ET group (n=46; 34 males, 12 females; mean age: 64.3±10.3 years) and the FPB group (n=103; 82 males, 21 females; mean age: 62.9±8.2 years). Primary and secondary patency rates at 6, 12, and 24 months were evaluated. Results: The primary success rates for ET and FPB were 100%. Primary patency at 6, 12, and 24 months were 93.5%, 89.0%, 69.5%, respectively for ET and 86.4%, 81.5%, 72.8%, respectively for FPB (p>0.05). Secondary patency rates at 6, 12, and 24 months were 97.8%, 93.5%, %84.8, respectively for ET and 96.1%, 90.3%, 79.6%, respectively for FPB group (p=0.41). The length of hospital stay was significantly longer in the FPB group (p<0.001). The cost of treatment was significantly higher in the FBP group (p=0.02). Conclusion: In TASC II type C patients, ET is a safe therapeutic option with lower in-hospital stay and treatment expenses and similar primary and secondary patency rates to FPB.

Femoral nerve block versus spinal anesthesia in the treatment of saphenous vein ablation 股神经阻滞与脊髓麻醉治疗隐静脉消融的比较

Turk J Vasc Surg 2022;31(1):7-14 ABSTRACT Objectives: This study aims to compare the effects of spinal anesthesia and femoral nerve block and to determine the optimal anesthesia method for embolization of vena saphena magna. Patients and methods: Between December 2019 and March 2021, a total of 160 patients (89 males, 71 females; mean age: 44.4 years; range, 18 to 69 years) who were diagnosed with varicose veins and hospitalized for surgery in our cardiovascular surgery clinic were included. The patients were divided into two groups. The first group (Group S, n=80) underwent spinal anesthesia and the second group (Group F, n=80) underwent femoral nerve block for the surgical operation. Results: The mean arterial pressures (MAPs) were signif icantly lower in Group S compared to Group F. The mean time until surgery was earlier in Group S (4.57±1.0 min vs. 9.9±3.4 min, respectively; p<0.01). The mean duration of motor block was longer in Group S (3.4±1.1 h vs. 2.7±0.8 h, respectively; p<0.05). The mean duration of mobilization was statistically signif icantly shorter in Group F (6.1±1.7 h vs. 5.2±1.4, respectively; p<0.05). The development of urinary retention was statistically signif icantly higher in Group S (p<0.05). The mean patient satisfaction score after discharge was higher in Group F (3.5±0.5 vs. 2.9±0.8, respectively; p<0.05). The incidence of postoperative shivering was statistically signif icantly higher in Group S (p<0.05). Conclusion: Femoral nerve block, which is used for intraoperative anesthesia during the procedure of endovenous laser ablation, can be preferred as an alternative method to spinal anesthesia.
