Gingival Augmentation with an Alternative Gingival Grafting Technique: A Case Report 一种替代牙龈移植技术的牙龈增强术:1例报告

The significance of keratinized gingiva around teeth and dental implants has long been challenged. However, certain clinical circumstances require gingival augmentation. In this case report patient having Miller's Class III gingival recession along with absence of attached gingiva in relation to malposed mandibular right central incisor was treated with free gingival graft procured from buccal gingiva. The aim was to augment keratinized tissue and attached gingiva in relation to mandibular central incisor along with esthetics. As free gingival graft procured from palate has its disadvantage of “tire-patch appearance” so graft was procured from buccal gingiva in relation to maxillary premolar area. This case was followed up till 6 months. The patient also underwent fixed orthodontic treatment, initiated 3 months after surgery. During orthodontic treatment periodontal parameters remained stable.

The utilization of computer-aided design and production has become increasingly prevalent in the dental industry, as in many other fields. Additive manufacturing, a computer-aided production method, has brought about numerous advantages such as fast and precise production, design freedom, and cost and time savings. It is anticipated that 3D printers will become the primary method for digital processing in dentistry in the future. The objective of this article is to offer a comprehensive introduction to the present-day manufacturing methods for fixed prosthetic rehabilitation, focusing on the 3D printing manufacturing and photopolymer resin materials employed in these technologies.

Evaluation of the Buccal Cortical Bone Thickness in Class II Malocclusion for Mini-Implant Insertion: a CBCT Study 评估II类错牙合植入微型种植体时颊皮质骨厚度:一项CBCT研究

Background: This study aimed to evaluate the buccal cortical bone thickness of the Iraqi population for mini-implant insertion using cone-beam computed tomography (CBCT). Methods: This cross-sectional study was conducted on maxillary and mandibular CBCT scans of 40 patients (14-25 years). One examiner measured skeletal parameters at 4, 6 and 8 mm apical to the cementoenamel junction (CEJ). Results: the largest buccal cortical bone thickness in the maxilla was between the first and second molar (1.36mm for males, 1.24mm for females) and in the mandible was between the first and second molar (2.17mm for males, 2.37 for female). Conclusions: The buccal cortical bone thickness varies in different individuals. In the mandible, the buccal cortical bone thickness was increased as we move from the anterior to the posterior area. The buccal Cortical bone thickness is thicker in the mandible compared to the maxilla.

Management of an Oral Fibroma by Scalpel Excision in a Patient on Anticoagulant Therapy: A Case Report 经抗凝治疗的口腔纤维瘤患者的手术治疗:1例报告

A traumatic or irritational fibroma is the most prevalent submucosal reactive lesion in the oral cavity. It forms as a result of irritation or trauma and is composed of fibrous or connective tissue. Although electrical or laser surgery is beneficial for the excision of oral fibroma in patients taking anticoagulant, but these devices may not be available in all clinical settings. This case report aims to demonstrate scalpel excision as an effective and convenient surgical treatment for the management of the fibroma in patient having anticoagulant drug instead of using electrical, laser, or cryosurgery. A female patient, 70 years old, reported to the department with a chief complaint of painless swelling in the right lower posterior tooth region for the past 2 years. On examination, the lesion was found to be a well circumscribed, smooth, tender, firm, and lobulated pink swelling 1x0.8x0.5 cm in diameter positioned on the buccal mucosa of the right molar region. Surgical therapy was carried out by scalpel excision, followed by medication for the management. In compared to electrical, laser, and cryosurgery, scalpel excision was a simple and effective approach to successfully remove the fibroma, and complete healing of the surgical site was observed after 2 months.

Giant cell Fibroma of Tongue: Entity with unique histopathological features 舌巨细胞纤维瘤:具有独特组织病理特征的实体

Giant Cell Fibroma can present as a nodular growth in the oral cavity, representing approximately 2% to 5% of all oral fibrous proliferations. They can be best distinguished from other fibrous lesions by their typical histopathological features of presence of stellate shape fibroblasts. Here, we report a case of a 65-year-old female with a mass on the tongue and discuss its clinical and histopathological differential diagnosis.

Comparative Evaluation of Metformin Gel and Chlorhexidine Gel as Adjunct to Scaling and Root Planning in the Treatment of Chronic Periodontitis: A Clinical Study 二甲双胍凝胶与氯己定凝胶辅助刮治及牙根规划治疗慢性牙周炎的临床研究

Introduction: Pyorrhoea is an inflammatory gum disease in which destruction of tooth supporting tissues that results ultimate tooth mobility and exfoliation. In local drug delivery (LDD), which includes the delivery of therapeutic agents like Irrigating systems, fibers, gels, strips, films, microparticles, nanoparticles and low dose antimicrobial agents via systemic and local route as an adjunct to mechanical therapy which has beneficial for periodontal therapy. Aim: To evaluate and compare the efficacy of subgingivally delivered 1% Metformin gel and Chlorhexidine gel as an adjunct to scaling and root planing in the management of patients with chronic periodontitis. Materials and Method: Twenty systemically healthy patients with the age group of 20-60 years irrespective of gender suffering from generalized chronic periodontitis were selected, in which fourty sites were randomized at split mouth level and divided into two groups: (Group A -SRP+1% Metformin gel and Group B - SRP+ 0.2% Chlorhexidine gel). Parameters such as, Plaque index (PI), Modified gingival index (MGI), Probing pocket depth (PPD), and Relative clinical attachment level (R-CAL) were measured at baseline and after 1 month and 3 months. The significance of difference within and between the groups was evaluated with paired and unpaired t-tests. Result: In our study, the mean Plaque index (PI), Modified gingival index (MGI), Probing pocket depth (PPD), and Relative clinical attachment level (R-CAL) showed a statistically significant difference from baseline to 1 month and 3 months (p≤0.05) in both Group A and Group B. But, Mean PPD reduction and mean R-CAL gain was found to be greater in Group-A than Group B at 1 month and 3 months. Conclusion: Both Metformin and 0.2% Chlorhexidine gel can be considered as effective as a LDD agent adjunct to scaling and root planning (SRP) in the management of chronic periodontitis. But, 1% Metformin gel was more effective as a LDD adjunct to SRP in curative of chronic periodontitis patient than Chlorhexidine gel (Cevitec gel).

Management of Lip Sucking Habit Using Combination Therapy: A Case Report 综合治疗吸唇习惯1例

Oral habits are those that are repeatedly performed thereby causing deleterious effects on the teeth and the adjoining oral tissues. In addition to or as a substitute for thumb sucking, lip sucking or biting is often observed. An orthodontic lip bumper, designed to intercept dental and occlusal problems, is a fixed or removable functional appliance. In this case report, the habit was treated using fixed lip bumper, lip exercises and habitual therapy conducted by both parents and the child. Parental encouragement and constant reminders combined with the child’s co-operation in wearing the appliance and performing the advised lip exercises regularly are significant factors in the success of this treatment.

Mandibular 2nd molar uprighting by using TADs- A Case Report TADs矫治下颌第二磨牙1例

The purpose of this case study is to present the application of mini-screw implants (MIs) as an optional treatment in case of mandibular molar uprighting without loss of anchorage. This is a clinical case where a mesially tilted mandibular 2nd molar is described that has been uprighted by using a mini-implant as an anchorage and the site of insertion of temporary anchorage devices (TADs) is distal to the molar.

Concentration of sodium hypochlorite disinfection in regenerative endodontic procedure: A systematic review 次氯酸钠浓度消毒在再生牙髓治疗中的应用综述

Objectives: To evaluate whether low concentration sodium hypochlorite (NaOCl) disinfectant protocol can achieve a sufficient amount of disinfection to provide a conducive environment for clinical and radiographic success of regenerative endodontic procedure. Materials and methods: This systematic review includes necrotic immature permanent anterior teeth below 20 years of age using triple antibiotic paste as an intra-canal medicament and autologous platelet concentrates as scaffold. The interventional group was low concentration (≤2%) NaOCl and comparator group included high concentration (>2%) NaOCl as disinfectant. Cochrane risk of bias assessment 2 (RoB 2) tool was used for assessment of risk of bias. Included studies were evaluated for the absence of clinical signs and symptoms, increase in root length and apical closure on radiograph, and positive response to vitality testing. Results: After evaluation, seven studies were included. Clinical success rate was seen in 98.33% cases in low concentration (≤2%) and 95% cases in high concentration (>2%) NaOCl. The radiographic success rate was 86.66% in low concentration (≤2%) and 80% in high concentration (>2%) NaOCl disinfection. Positive response for vitality testing was seen in 46.66% cases in low concentration (≤2%) and 38.70% in high concentration NaOCl (>2%). Conclusion: Low concentration (≤2%) NaOCl is as effective as high concentration (>2%) to achieve disinfection of the root canal. However, an increased success rate is associated with the use of low concentration (≤2%) NaOCl. Response to pulp vitality is observed to be superior with low concentration (≤ 2%) NaOCl although it might be attributed to a longer follow-up period.

Evaluation of Local Drug Delivery System Containing 1% (W/V) Chitosan Gel Used as an Adjunct to Scaling and Root Planning in Chronic Periodontitis– A Split Mouth Study 含1% (W/V)壳聚糖凝胶局部给药系统用于慢性牙周炎刮治和牙根规划辅助的评价-一项裂口研究

Introduction: Periodontitis is defined as an inflammatory disease of supporting tissues of teeth,In the treatment of periodontitis systemic antibiotics has potential side effect such as the development of resistant bacteria. These limitations can be avoided by application of the antimicrobials in local drug delivery (LDD) systems directly into the periodontal pocket. Natural polymer chitosan which is found in abundance in nature has antibacterial, antiinflammatory properties and is biodegradable and biocompatible. Materials and Method: A randomized controlled split-mouth study was designed. 30 patients were randomly selected.A resorbable chitosan gel of 1% (w/v) was placed in the selected test sites one week after scaling and root planing (SRP). Gel was injected into the periodontal pockets using a blunt cannula till the pocket was completely filled. Periodontal dressing was applied after placement of the gel. The ancillary parameters were assessed. Results: Statistically significant difference was present in mean plaque index scores and modified gingival index scores at 1 month and 3 months from baseline in both the groups as chitosan shows a strong activity in reducing dental plaque against various periodontal pathogens. Statistically significant difference was present in pocket depth between group I and group II at 1 month and 3 month. Statistically, significant difference was present in Relative Attachment Level between group I and group II at 1 months and 3 months. Discussion: In this study Chitosan biopolymer, used in a gel form, (1% w/v) demonstrated the effectiveness for treatment of localized pockets in patients with chronic periodontitis.the results suggest chitosan has its own properties which causes reduction of pocket depth. Conclusion: Chitosan is an effective local drug delivery agent for chronic periodontitis patients.
