Barotrauma y ventilación mecánica en pacientes críticos COVID-19 COVID-19危重患者气压创伤与机械通气

. Manifestations of SARS-CoV-2 pneumonia can rapidly progress to acute respiratory failure and acute respiratory distress syndrome that should receive timely ventilatory support invasive mechanical ventilation. Barotrauma is a complication that should be considered in this type of patient due to the risk of worsening the clinical course, increasing mortality risk. The mechanism of how barotrauma occurs in COVID-19 patient is related to lung damage associated with mechanical ventilation and structural changes caused by COVID-19 complications. Worldwide evidence on therapeutic management on barotrauma in COVID-19 patient is limited. Clinical evolution should be monitored due to the possibility of cardiovascular and respiratory complications, which must be resolved in a timely manner. The objective of this review is to inform about the current knowledge described in literature of this complication in COVID-19 patients, its incidence, pathophysiology, diagnosis, treatment and prognosis. Conclusions: Barotrauma presentation frequency increases in COVID-19 infected patients. Clinical manifestations can be nonspecific, so the diagnosis should have high suspicion. . Thymus tumors are the most frequent in the anterior mediastinum. Congenital mediastinal cysts occur 3 to 6%. Their posterior location is extremely rare, and therefore the clinic that accompanies them; however, it should be considered among the differential diagnoses, as is the case in the case presented below in a patient with a history of neurofibromatosis in a reference institute. This is a 53-year-old woman with a posterior mediastinal cyst, undergoing thoracotomy resection, which in the definitive pathology study was a thymic cyst, with good results and with follow-up without recurrence.

Utilidad de la biopsia pulmonar transtorácica con aguja de Tru-cut y de Chiba en el mismo paciente, guiadas por TC para el diagnóstico de tumoración pulmonar periférica 在ct引导下,trucut和千叶经胸肺活检在同一患者诊断外周肺肿瘤中的应用

La biopsia pulmonar transtoracica con aguja fina tipo Chiba o gruesa y cortante tipo Tru-cut, guiadas mediante tomografia computada de torax, son empleadas para el diagnostico de lesion pulmonar periferica. Objetivo: Comparar la certeza diagnostica de la biopsia pulmonar transtoracica, con ambas agujas en el mismo paciente, y evaluar si la combinacion mejora la certeza diagnostica sin aumentar las complicaciones. Material y metodos: De abril del 2013 a abril del 2016 en un hospital de segundo nivel de atencion se estudiaron 49 pacientes adultos con lesion pulmonar periferica, con guia de tomografia computada, se tomaron biopsias con aguja de Chiba, seguida de Tru-cut, las cuales fueron analizadas por dos patologos distintos. Resultados: Se eliminaron 2 pacientes por no tener resultados completos. De los 47 estudiados, 34 (72%) fueron del genero masculino y 13 (28%) del femenino, edad de 42 a 86 y media 68.9 anos. Con la tecnica de Chiba se diagnostico el 95% de los pacientes; 41 malignos, 4 benignos y 2 falsos negativos que requirieron cirugia menor en quirofano (toracoscopia y minitoracotomia) para su diagnostico. Con la de Tru-cut el 87%: 38 malignos, 9 negativos a malignidad: 9 benignos (en 2 de ellos fueron malignos con Chiba) y 5 falsos negativos (3 diagnosticados con Chiba y las 2 de cirugia menor); y 4 patologias benignas (2 infecciosas, 1 hamartoma y 1 atelectasia pulmonar). Las estirpes histologicas mas frecuentes con ambas tecnicas fueron epidermoide y adenocarcinoma. Las causas benignas fueron hamartoma, infecciosa y atelectasia pulmonar. Chiba reporto sensibilidad 0.95, especificidad 1, VPP1, VPN 0.66, falsos negativos 0.04. Mientras que la de Tru-cut fue de 0.89, 1.1, 0.44 y 0.11, respectivamente. Cardiopulmonary exercise testing (CPET) makes it possible to evaluate, simultaneously, the physiological integration of the cardiovascular, respiratory, metabolic, musculoskeletal and neurosensory systems during exercise. CPET provides a physiological response profi le that is useful for diagnosis and follow-up of patients in diverse clinical scenarios. Technological advances have facilitated the performance and interpretation of this test; however, in Mexico it is still little-utilized, in part because it is not widely-known. The objectives of this document are, fi rst, to present a general view of the practical aspects of CPET to pneumologists and the medical community as a whole, and, second, to describe the standard procedure for successful test performance. Finally, the text elucidates basic aspects of the interpretation of test results.
