Infiltrative cardiomyopathy 渗透性的心肌病

Infiltrative cardiomyopathies are comprised of a variety of inherited or acquired conditions. Myocardial involvement is often part of a complex systemic disorder, but sometimes the disease can manifest predominantly in the heart. Clinical manifestations are heterogeneous, from mild symptoms to early cardiac death. A multimodality imaging approach by means of echocardiography, cardiovascular magnetic resonance (CMR), and molecular imaging is necessary; a comprehensive assessment of morphologic, functional, tissue, and metabolic changes is the key to improve the pathophysiologic understanding of these diseases, favouring early differential diagnosis and risk stratification of patients. Furthermore, quantitative, non-invasive assessment of tissue pathology is now available and plays a crucial role in the follow-up of patients affected by infiltrative cardiomyopathies, improving their management and potentially guiding the development of existing and new therapies.

Constrictive pericarditis 缩窄性心包炎

Constrictive pericarditis is a pericardial syndrome where the pericardium becomes relatively rigid and inelastic, may be thickened and calcified or not, and impairs mid to late diastolic filling. Constrictive pericarditis is the final pathway of several different diseases or causes, usually starting from pericarditis and pericardial effusion, and progressing towards pericardial fibrosis and calcification. Constrictive pericarditis is commonly the final evolution of any type of pericarditis and pericardial effusion. The risk of developing such evolution is especially related to the aetiology. The risk of progression is especially related to the aetiology: low (<1%) in viral and idiopathic pericarditis, intermediate (2–5%) in immune-mediated pericarditis and neoplastic pericardial diseases, and high (20–30%) in bacterial pericarditis, especially purulent pericarditis. It is important to recognize transient (subacute) constrictive pericarditis early on in the process versus calcific chronic constrictive pericarditis.

Nuclear cardiology (PET and SPECT)—basic principles 核心脏病学(PET和SPECT) -基本原理

Historically, radionuclide imaging has been the key to the success and widespread clinical application of non-invasive techniques for detection of myocardial ischaemia. Today, the use of myocardial perfusion single photon emission computed tomography (SPECT) for the diagnostic and prognostic workup of coronary artery disease is supported by a very large body of evidence. Positron emission tomography (PET) as an advanced, quantitative technique is increasingly penetrating the clinical arena. The field is responding to recent trends by adopting new technology and algorithms for reduction of radiation exposure and by developing a toolbox for targeted molecular imaging of key biologic mechanisms involved in heart disease and therapy response. By this, nuclear cardiology will continue to serve as a role model for the success of functional imaging in heart disease. This chapter gives an overview over the basic technology, including novel methodologic developments.
