Goal. To analyze medical prescriptions in real practice in the treatment of children and adolescents with signs of osteopenia. Methods. A retrospective analysis of 44 medical records of patients aged 0 to 17 years, inclusive, with complaints characteristic of osteopenia, who were prescribed a 25(OH)D test. A content analysis of vitamin D preparations in the pharmaceutical market of the Russian Federation was carried out and the costs for 3 months of treatment were calculated. Results. 6 models of complex treatment of osteopenia in children and adolescents were formed, in relation to the diagnosis and the detected level of 25(OH)D. Each included vitamin D supplementation, a calcium-rich diet, and various physical therapy modalities. Treatment outcomes were monitored every 3 months based on X-ray data and blood 25(OH)D testing. It has been established that the cost of non-drug therapy is more than 80% of the total. The cost of paid treatment for various skel- etal deformities consists of the costs of medical services, non-drug and drug therapy and amounts to 25805.4 rubles. ± 488.1 rub. Conclusion. Real medical practice in the treatment of osteopenia in children and adolescents is based on current clinical algorithms and recommendations. The appointment of treatment is carried out not only on the basis of the diagnosis, but also the value of laboratory tests.

The review considers conceptual models for the formation and evaluation of the quality of pharmacy organizations. The formation and evaluation of the quality of work of a pharmacy organization depends on a set of factors, in particular on the intellectual cap- ital of employees. It is shown that competitor management involves the use of such strategic components in the competition as the intellectual capital of employees of a pharmacy organization, contributing to the competitiveness of the organization in the pharmaceutical market and reducing risk events.

The purpose of the study: to study the activities of the pharmacy, a structural unit of JSC «Pharmacy» (Tyumen region) in terms of providing social pharmaceutical assistance to citizens. Materials and methods. The object of the study was the activities of Pharmacy No. 173, a structural unit of the joint-stock company «Pharmacy» in the Tyumen region, indicators of all areas of preferential medication provision for citizens, including a new direction for providing people who have had acute cardiovascular diseases, the amount of funds allocated from the budgets of all levels for the provision of medication care , data from the regional Register of Persons, volumes of dispensed medicines for the preferential category, the number of secured prescriptions, their average cost. The following methods were used in the study: situational-logi- cal, content analysis, structural, system, computer technologies. Results. The directed activity of the pharmacy of finished dosage forms to ensure the availability of drug assistance to privileged categories of citizens, its social significance, the indicators for the number of secured prescriptions, the volume of dispensing of preferential drugs, including those who have had acute cardiovascular diseases, have been studied. Pharmacy of finished dosage forms, which carries out pharmaceutical activities in the retail trade of medicines for medical use, storage, dispensing of medicines, performs an important social function in dispensing medicines by prescription for free or with a 50% discount from the cost, includ- ing state social assistance (34, 04% of the total volume of dispensing), medications as a measure of social support in the treatment of certain diseases (58.63%), expensive drugs for the treatment of 14 nosologies (2.92%), certain drugs for the treatment of people who have had acute heart vascular diseases (4.41%). To perform this function of drug supply, a specialized pharmacy department was organized, equipped with a software product for recording incoming prescriptions, including in electronic form, for creating registers of issued prescriptions, and recording the movement of preferential drugs. The activities are carried out by pharmacists, pharmacists - technologists trained in consulting on the provision of social assistance to certain categories of citizens To comply with the regulatory and legal regulation of the provision of this activity, the standard operating procedure (SOP) «Dispensing med- icines to preferential categories of citizens» is applied. The standard operating procedure describes the step-by-step procedure for dispensing medicines on prescriptions of medical professionals to citizens who are entitled to receive free or discounted medicines. Conclusion. It should be noted the undoubtedly significant role of the pharmacy, pharmaceutical workers in the system of providing benefits in the field of drug provision. Arterial hypertension is a widespread disease and a major risk factor for the development of lethal cardiovascular pathologies. To reduce this risk, timely detection of arterial hypertension, control of blood pressure levels and effective drug therapy for high blood pressure are necessary. Relevance. Study of population consumption of drugs used for hypertension in the retail sector of the pharmaceutical market in the Russian Federation for the period from 2017 to 2021. Methods. We used the methods of retrospective, predictive, structural, logical, comparative, graphic, marketing, pharmacoepi- demiological analyzes when conducting this study. The results were processed using the normalization method and methods of mathematical statistics. Results. The analysis of the structure and study of the characteristics of the population consumption of drugs used for arterial hy- pertension in the territory of the Russian Federation for the period 2017-2021 was carried out. The main share of sales falls on monopreparations: 92.04% - in physical terms, 74.55% - in monetary terms. In the rare pharmacy sales of antihypertensive drugs in the Russian Federation, the share of fixed combinations is 7.96% in physical terms and 25.64% in monetary terms. The most popular antihypertensive drugs in both in-kind and monetary terms over the estimated period are ACE inhibitors and β-blockers. In the subgroup of fixed combinations, “angiotensin II receptor blocker + diuretic” and “ACE inhibitor + diuretic” are in the lead. In the total consumption of antihypertensive drugs, the share of multicomponent fixed combinations accounts for only 0.44% in physical terms, but in monetary terms this figure is 2.51%. According to the results of the survey, doctors were more likely to be conservative in prescribing antihypertensive drug therapy, preferring single drugs, and medical specialists receive information about new drugs mainly at conferences, when meeting with medical representatives, and also from the Internet. Conclusion. Based on the results of the study, an educational program for medical and pharmaceutical specialists “Modern range of drugs prescribed for the prevention and treatment of hypertension” was developed.

Acute respiratory viral infections (ARVI) are the most common diseases of the human population. Due to the high virulence of pathogens, in some cases they can acquire the character of an epidemic or pandemic. The diversity of the clinical picture, the com- plexity of predicting outcomes makes the problem of rational choice of a drug for the primary treatment of acute respiratory viral infections a key one and poses a difficult task for the pharmacist - the correct, efficient, safe and effective use of a wide range of drugs available on the domestic pharmaceutical market for the treatment of acute respiratory viral infections. During the pharma- cy consultation, the pharmacist has to find out the patient’s complaints, his morbid background, the presence of drug intolerance, as well as a number of other factors, taking into account the pandemic of a new coronavirus infection. Medicines for the treatment of acute respiratory viral infections are presented in most cases as combination drugs that have several mechanisms of action. The possibility of influencing several manifestations of the disease is provided through the use of Septolete® Total, which has not only a favorable efficacy / safety profile, but also distinct economic advantages over analogues, as well as a potential virucidal effect against the causative agent of a new coronavirus COVID-19 infections.

Abstract The Aim. Pharmacoeconomic assessment of the oral antiplatelet availability of various drug manufacturers and dosage forms based on the analysis of price characteristics. Materials and methods. The analysis included drugs from the group of platelet aggregation inhibitors (excluding heparin) (ATC code B01AC): clopidogrel, clopidogrel + acetylsalicylic acid (ASA), ASA and dipyridamole in various dosages and number of tablets per package. The cost values were estimated by the resources of large resources "Apteka.ru", "009.rf" with the average value calculations in the eight federal district’s capitals of the country. Results. The cost of drugs in the analyzed group varied depending on the drug manufacturer. The highest amount of the cost variation was in the ASA tablet form (100 mg) – 407.47%, followed by the tablet form of clopidogrel (75 mg) – 311.16%, ASA (50 mg) – 172.20%, dipyridamole (75 mg) – 117.36% and the lowest values for the tablet forms of dipyridamole (25 mg) – 68.19% and the combination of ASA with clopidogrel (100mg + 75mg) – 59.13%. The highest Ca.s values were in ticagrelor (90 mg) – 24.92% and prasugrel (10 mg) – 7.87%, the lowest in ASA (50 mg) – 0.1%. According to the linear index of distribution, the drugs ASA (100 mg) – 89%, clopidogrel (75 mg) – 70%, and less accessible ticagrelor (60 mg) – 18% and prasugrel (10 mg) – 14% had a higher physical availability. Conclusion. Based on the calculated values a high availability of such antiplatelet drugs as ASA (50 mg and 100 mg), clopidogrel (75 mg) was revealed. According to the results of the study, the antiplatelet drugs ticagrelor (60 mg and 90 mg) and prasugrel (10 mg) have the least level of availability.

PURPOSE. Study of the potential of pharmacy chains for the development of pharmaceutical literacy of the population through Internet and online resources. METHODS. The objects of the analysis were the Internet sites and online services of the largest pharmacy chains and services for remote ordering of pharmacy assortment goods (TOP 20). The basis for inclusion in the analysis was the data of the analytical agencies DSM Group, RNC Pharma and AlphaRM. Methods of criteria, logical, comparative and content analysis were used to conduct the study. The authors have developed their own evaluation scale focused on evaluating the content of the Internet sites of pharmacy chains in relation to the information and educational component for the population. RESULTS. Based on the consolidation of data from 3 analytical agencies, the final list of objects of analysis included 18 pharmacy chains and 3 services for remote ordering of pharmacy assortment goods. The average score on the evaluation scale for the totality of pharmacy chains was 9.13±0,74 out of 23 possible. The presence of websites and telephone help service for all objects of research, their features are revealed. The assessment of the availability and information content of mobile applications of pharmacy chains was carried out. The analysis of the information content of pharmacy networks in social networks and Internet messengers was carried out. Conclusions are made about the quantitative and qualitative component of the service for the population "Question – answer". The presence and features of information and educational materials for the population on the Internet sites of pharmacy chains are analyzed. A study of instructions for the medical use of medicines was conducted: quantitative components, relevance, compliance with the State Register of Medicines of the Russian Federation. CONCLUSION. The conducted research allowed us to draw conclusions about the insufficient degree of involvement of pharmacy chains in the process of pharmaceutical education of the population and the insignificant use of the possibility of influence of pharmacy chains on the development of pharmaceutical literacy of the population through Internet resources. The article provides recommendations on possible ways to fully develop pharmaceutical education of the population in the Internet space.

The objects of the analysis were the Internet sites and online services of the largest pharmacy chains and services for remote ordering of pharmacy assortment goods (TOP 20). The basis for inclusion in the analysis was the data of the analytical agencies DSM Group, RNC Pharma and AlphaRM. Methods of criteria, logical, comparative and content analysis were used to conduct the study. The authors have developed their own evaluation scale focused on evaluating the content of the Internet sites of pharmacy chains in relation to the information and educational component for the population. RESULTS. Based on the consolidation of data from 3 analytical agencies, the final list of objects of analysis included 18 pharmacy chains and 3 services for remote ordering of pharmacy assortment goods. The average score on the evaluation scale for the totality of pharmacy chains was 9.13±0,74 out of 23 possible. The presence of websites and telephone help service for all objects of research, their features are revealed. The assessment of the availability and information content of mobile applications of pharmacy chains was carried out. The analysis of the information content of pharmacy networks in social networks and Internet messengers was carried out. Conclusions are made about the quantitative and qualitative component of the service for the population \"Question – answer\". The presence and features of information and educational materials for the population on the Internet sites of pharmacy chains are analyzed. To Develop classification of legal acts regulating the activities of hospital pharmacies and identify significant problems in them for drug provision enhancement in health facilities. Methods The main regulatory framework governing the hospital pharmacies activities was carried out using the computer help system "Con- sultant Plus" and Internet search engines Google and Yandex. The first group (group No. 1) includes terms that affect the actual subject of the study – hospital pharmacy and synonyms. The second group of words used (group No. 2) is common terms of hospital pharmacy activities and drug supply in the health facilities. Results We developed classification of regulatory legal acts that regulate the work of hospital pharmacies by the method of regulation (regulating directly and indirectly) and by content (regulating general aspects and applied aspects of drug provision in the health facility. Regulations governing general aspects, the definition of the hospital pharmacy as a pharmacy organization, the classifi- cation of pharmacy organizations, the status of a hospital pharmacy, the list of goods provided by a hospital pharmacy, as well as pharmaceutical works performed. Legal acts regulating applied aspects include groups of documents regulating the main process- es (drug supply) and regulating the supporting processes (personnel policy, provision of material and technical resources). Conclusion We identified non-regulated stages of drug supply specific for health facilities. In connection with the abolition of Soviet period legal acts important issues, personal and material and technical resources, were excluded from the legal framework, which was not compensated by the emergence of relevant legal acts. To date, a systematic approach has not been developed in the legal regula- tion of the hospital pharmacy, there is no continuity of legal acts and uniformity of terminology.

The purpose of the study was to analyze the problems of dosing injectable drugs for general anesthesia in a pediatric surgical hospital. Methods: questioning, logical analysis, economic modeling. Results: As a result of a survey of anesthesiologists at a pediatric surgical hospital, problems with dosing drugs for general anesthesia for children in a pediatric surgical hospital were identified, due to the lack of special pediatric dosages and concentrations of injectable drugs. Injectable drugs have been identified that cause difficulties in individual dosing for children due to the lack of the required form of release: INN fentanyl (ampoules), propofol, ketamine, rocuronium bromide (ampoules and vials). Losses in the primary packaging of these medicines with a single use in young children are up to 50%-90%, which leads to both material losses and environmental risks when disposing of residues. An analysis of instructions for medical use showed that pharmaceutical manufacturers do not provide for an indication of the period and conditions for storing these medicinal products after the first opening of the vial. Conclusion: A set of proposals has been developed to optimize the circulation of injectable drugs for general anesthesia used in children: the advantages of using drugs in vials compared to ampoules are shown, and the primary packaging “vial” for fentanyl solution for injections is recommended; recommended the introduction of a requirement for registration of injectable drugs for general anesthesia in vials to indicate the period and storage conditions after the first opening of the primary packaging; on the label applied to the primary packaging, it is necessary to indicate the date and time of opening the vial;in the registration dossier of injectable medicinal products for general anesthesia, it is necessary to provide data confirming the stability after the first puncture of the capping of the vial during the recommended period and storage conditions of the medicinal product.

The organization of activities for the formation of sanitary literacy of patients with chronic diseases is a socially significant problem, which is very important for patients of the older age group who have many chronic diseases. When analyzing data compliance, it is necessary to identify and take into account the influence of a number of health-saving predictors. The aim of the study is to conduct a multifactorial analysis of health-saving predictors for patients with chronic diseases and to develop methodological foundations for the organization of their medical and pharmaceutical support. Materials and methods. The study of health-saving predictors for patients with chronic diseases was conducted on the basis of the results of an assessment of the behavior and lifestyle of patients with type 2 diabetes mellitus. The empirical database is formed by the results of a survey of 411 patients. Methods of statistical analysis using software products Statistica 10, Microsoft Excel were used. Methods of mathematical statistics were used (Student's two-sample test to assess significant intergroup differences; analysis of average ranks and analysis of variance). Results and discussion. By means of multivariate analysis of health-saving predictors for patients with diabetes mellitus, the most significant parameters of health-saving of patients with chronic diseases have been identified. The systematization of predictors on the basis of "controllability" was carried out. Unmanageable predictors of a patient's health include gender, age, level of education and income; conditionally controlled predictors include the functional state of the body, the presence of comorbidity and/or polymorbidity, the severity of polypragmasia, the duration of the disease; controlled predictors include the level of sanitary literacy and patient compliance, the presence of self-control skills of the disease, the level of social adaptation and the lifestyle. Conclusion. A logical and functional model of strategic management of medical and pharmaceutical care for patients with chronic diseases has been developed and a generalizing cumulative indicator (patient's health saving coefficient) has been formed on the basis of controlled predictors of health saving, which allows to evaluate the health saving potential of a particular patient.
