Evaluating the effectiveness of healthcare services is an urgent task because it allows us to identify and analyze the existence of health potential risks and organizational difficulties, and it also assesses in developing preventive measures to eliminate them. Currently, there is no regional unified system to evaluate the existing organizational issues of the regional oral and maxillofacial surgery service. Purpose: accordingly, this study aims to determine an evaluation criterion for the organization of regional oral and maxillofacial surgery service and use an expert assessments method. Materials and methods. We conducted a study in 2019–2022 on the base medical organizations of the Volga Federal District using an expert evaluation method. In the first phase, we selected 60 participants in the organization of oral and maxillofacial surgery service, both at the level of the medical organization and at the regional and federal levels. The experts proposed 64 criteria for evaluating the organization of regional oral and maxillofacial services, which were included in the map of expert evaluation. In the second phase, we selected 21 experts by the inclusion criteria and scored the proposed criteria. To evaluate the questionnaire results, 95 percent of the confidence interval (MDI) was calculated using Fisher’s method. Kendall’s coefficient of concordance (W) was used to determine the consistency of experts’ views on the importance of the criteria chosen. The estimation of the concordance coefficient was considered relevant if the probability of the first type of error was less than 0.05. Results and discussion. Analysis of the results and their ranking by experts made it possible to identify the 20 most significant indicators and observe more than 8 points and a concordance correlation coefficient of 0.7 or more. According to experts, for organizing the provision of oral and maxillofacial surgery service, the most important issue is the availability of regulatory documents, regulations for routing patients, rules for the provision and infrastructure of oral and maxillofacial surgery service, the recommended standard of medical equipment and recommended staffing standards, staffing, the availability of emergency round-the-clock dental service, equipping a panel of devices, sets of surgical instruments for surgical interventions in the maxillofacial region, organizing the provision of care in oral and maxillofacial surgery for children and the disabled in the departments, regular monitoring of patient satisfaction by surveys. Conclusions. Significant criteria for assessing the organization of regional maxillofacial surgical service were identified, which made it possible to identify existing shortcomings in the organization of activities and outline ways to eliminate them.

Improving the accessibility of medical care is one of the tasks to achieve the goals of state policy in saving the population and human development of a number of strategic documents of the Russian Federation. Purpose of the study is to analyze the non-medical aspects of ensuring accessibility of medical care, their normative legal regulation in territorial planning in healthcare at the regional level. Materials and methods. The source of information was legal acts from the reference legal system ConsultantPlus. Descriptive and analytical methods were used in the study. Results. It is shown that the issues of ensuring accessibility of medical care and creating conditions for its provision and development are attributed by various legislative and regulatory acts both to the powers of public authorities of the subject of the Russian Federation and to the powers of local governments within the competence defined by the legislation of the Russian Federation, but these provisions of powers are fixed differently and require amendments and clarifications. The expediency of making amendments to normative acts of different levels and departments within the framework of clarifying the norm on creating conditions for the provision of medical care to the population on the territory of a municipal district or municipal, urban district in terms of providing transport services to the population and organization of transport services, parking space with the possibility of getting to a medical organization for all groups of the population has been determined. Within the framework of regulation, other social guarantees for children of medical workers and norms on providing medical workers with housing (office housing, under a social rent agreement, provision of relevant subsidies and payments for the purchase of housing) need to be specified. The presence of sufficient regulation in terms of the rules of construction and maintenance of roads in the legal field was determined, however, the federal laws and subordinate regulations cited in the study lack specificity and correlation of these legal norms in relation to medical organizations. Conclusions. The necessity to improve the normative basis of non-medical aspects of state and municipal provision of accessibility of medical care to the population in terms of correlation between constitutional norms and legal acts of different departments has been determined in order to specify measures aimed at improving accessibility in the provision of medical care regarding transport accessibility, social guarantees for medical personnel and other measures to develop infrastructure.

Actuality. To develop effective tools for protecting the mental health of adolescents, medical workers need to clarify the leading reasons for its formation in modern conditions, predictors and criteria for the risk of its deterioration. Currently, there are no integrated medicalpsychological- social monitoring studies of adolescent mental health, including dynamic monitoring of psychological, pedagogical, social and medical indicative indicators. The purpose of the study is to review scientific publications devoted to the analysis of the state of mental health of children and adolescents in the Russian Federation. Research methods. Using the search image: («mental health of children and adolescents», «organization of psychiatric care for children and adolescents», «addictive behavior of children and adolescents», «risk factors for mental health of children and adolescents»), a collection of 63 relevant articles published by domestic and foreign authors over the past 15 years (2013–2023) have been formed, indexed in the bibliometric databases WoS, Scopus, RSCI, and in the search engines PubMed, Google Scholar. Research results. The performed analysis allows us to note that such issues as the manifestation of clinical mental disorders, their predictors with age-related detail, and the development of integral indicators of mental health deterioration have not been sufficiently studied. Studies indicating the presence of problems in the system of mental health protection of minors during their studies in general education organizations do not provide specific recommendations for their solution.. Conclusion. To develop effective tools for the protection of adolescent mental health, medical professionals need to clarify the leading causes of its formation in modern conditions, predictors and criteria for the risk of its deterioration. This will make it possible to fill the existing deficit in the algorithms of medical support for adolescents during their studies in general education organizations and medical, psychological and pedagogical integration.

Building a system for measuring the value of a model of a medical organization implementing a project involves choosing a mutually appropriate model of the optimal prototype, by assessing and preparing tools based on it for effective project management. The purpose of the study is to formalize the requirements for searching and visualizing the optimal project prototype using the principles (adequacy, value, abstraction and simplicity) applied to determining the model of a medical organization implementing a regional project. Materials and methods. Using the analytical research method, an attempt was made to determine the correspondence between the required accuracy and the initial data of the object (MO implementing the project) by formalizing the requirements of the stages of constructing a mathematical model to find the optimal prototype. Results. Determining the adequacy of the goals when the properties of the model coincide with similar indicators of the modeled object is a necessary condition for assessing the value of the model, giving an understanding of the degree of reliability and approximation of the calculation results to experimental (pragmatic) data. The totality of data from the model of a medical organization implementing a project in conjunction with its elements that interact with the environment as a single whole is the principle of adequacy (value) of the project model for finding the initial parameters of the prototype. The principle of abstraction from minor details of the model, or simplification, while maintaining the essential properties of the system, is an important condition for its optimization. Structuring the interconnections of relationships, functions of objects and subjects of relationships during the empirical deployment of the initial model to a prototype (working model of the project) for a subset of possible stakeholders (customer, developer, performer, patient) provides feedback and transformation of the model at an early stage. The principles of abstraction from minor details of the model (simplification) and simplicity are important conditions for the formation of assumptions for the search for a prototype. Conclusions. Thus, finding a compromise between the desire to create a complete description and obtaining the required results when constructing a simple project model is possible by taking into account the essential parameters specified in the framework of assessing their impact on the efficiency of the system.

Demographic aging of the population is especially pronounced in rural areas and has regional differences. Purpose of the study: based on the analysis of medical and demographic indicators of mortality and morbidity of the rural population over working age in the Republic of Tatarstan (RT), to develop proposals for the health of rural residents. Methods and materials: analytical, statistical, Rosstat data and statistical materials from the Federal State Budgetary Institution “TsNIIOIZ” of the Russian Ministry of Health (2019–2022). Results. During the years of pension reform (2019–2023), the number of rural population in the Republic of Tatarstan increased slightly – from 922,3 to 930,3 thousand (0,9%), while the share of the rural population over working age decreased from 28,6 up to 27,5% (in the Russian Federation the figure is lower – 25,5%). The mortality rates of the rural population in the Republic of Tatarstan were significantly higher for all years of analysis: in 2013 they were 15,5‰ for the rural population, 11,0 for the urban population. During the years of COVID‑19, mortality rates increased and amounted to 18,9‰ for the rural population in 2021, 14,4‰ for the urban population, and decreased in 2022 and amounted to 14.1 and 10,3‰, respectively. The primary morbidity rate of the rural population in the Russian Federation (2022) was 64755,2 per 100 thousand rural population, in the Republic of Tatarstan it was lower – 52598,4‰îî. There is a large difference in the indicators in the constituent entities of the Russian Federation – 13,6 times. The newly identified morbidity in the rural population over working age in the Republic of Tatarstan is lower than 48996,2‰îî than in the entire rural population 52598,4‰îî, while the older population has the highest rates of primary morbidity due to diseases of the circulatory system, diseases of the bones, muscular system and connective tissue, diseases of the genitourinary system, diseases of the nervous system, neoplasms, etc. In this age group, the incidence of COVID‑19 is higher. The results of the study should be taken into account for the development of management decisions at the regional level, as well as when implementing programs for rehabilitation and medical examination of older patients.

One of the objectives of the Federal Project «Fighting Cancer» is to achieve «The proportion of malignant neoplasms detected at early stages, Percentage.» One method to achieve the set goals is to improve the efficiency of preventive medical examinations and screenings of certain adult population groups. The study aims to provide a methodological rationale for calculating the efficiency indicators of preventive medical examinations and screenings of certain adult population groups in terms of detecting malignant neoplasms. Materials and methods. For conducting the study, we used normative legal documents regulating the procedures for conducting preventive medical examinations and screenings of certain adult population groups, as well as industry-specific statistical report forms № 131/o «Information on conducting preventive medical examinations and screenings of certain adult population groups» from 11 subjects of the Northwestern Federal District for the period 2017–2022. Results. According to the analysis of the indicator «The share of residents of the Russian Federation subjects undergoing PME (Preventive Medical Examination) and SCS (Screening for Certain Groups) compared to the number of residents fitting the criteria for PME and SCS», this indicator has practically not changed over the period from 2017 to 2022, being 0,41 in 2017 and 0,42 in 2022. The coverage by preventive examinations and screenings of certain population groups in the Russian Federation subject in the Northwestern Federal District from 2017 to 2022 decreased from 0,92 to 0,75 of what was due. When calculating the actual coverage by preventive examinations and screenings of certain adult population groups in the Northwestern Federal District, the indicator slightly decreased from 0,37 in 2017 to 0,32 in 2022. The «overall» contribution of preventive medical examinations and screenings of certain adult population groups in the Northwestern Federal District slightly increased from 0,11% in 2017 to 0,14% in 2022. When analysing the «crude» index of the contribution of PME and SCS in detecting malignant neoplasms from 2017 to 2022, an increase from 3,34% to 7,6% is noted. The standardised contribution of preventive examinations and screenings of certain adult population groups to the diagnosis of breast cancer malignant neoplasms increased from 3,7% in 2017 to 8,5%, and the diagnosis of malignant neoplasms of colon, rectum, and anal canal (C18-C20) for the same period increased from 4% to 7,9%. Conclusion. Regardless of which indicator is used to calculate the efficiency of conducted preventive measures, manual data entry and the formation of statistical forms 131/o must be stopped. Manual data entry can lead to data distortion and introduce significant fluctuations in indicators when comparing the same indicator among different subjects of the Russian Federation.

The article presents an analysis of modern foreign scientific literature on the effect of hormonal contraception on the health of teenage girls; states the need to support expanded clinical research before implementing recommendations for the use of hormonal contraceptives among adolescents and to allocate funding for programs to promote, encourage, and distribute hormonal contraception among adolescents as the main means of preventing abortion. Given that WHO recognizes the lack of strong evidence of funding for programs promoting hormonal contraception promoted by international organizations, aimed primarily at the poor and marginalized sections of the population, in our country these programs should be considered unacceptable, and especially in general educational organizations. Today, more than ever, joint efforts are needed on the part of health authorities, education, family and society to promote preventive educational and health activities in order to inform adolescents about the risks of hormonal contraception on health, as well as to develop in young people a responsible attitude towards their health, starting a family and future motherhood.

The purpose of this study is to analyze the consequences of government decisions on remuneration and implementation of special social benefits. The key provisions of the new regulatory legal acts in this area are considered. An assessment of the upcoming changes in the wage system is given, in terms of the possible impact on the stimulating qualities of the wage system. Recommendations have been developed on possible options for actions by public authorities of the subjects of the Russian Federation and state (municipal) institutions in order to achieve the established share of salaries in the amount of 50% of the total salary. The impact of the changes under consideration on the preparation of the transition to a new wage system in healthcare is assessed.

The organization of prevention and treatment of acne in adolescents, necessary to improve the quality of medical dermatological care, has not been studied enough. The purpose of the study: аnalysis of current and potential opportunities for organizing the prevention and treatment of acne in adolescents. Results. The pathophysiological mechanisms of acne development are being reviewed, considering the characteristics of the puberty period, however, the trends in introducing new approaches to the organization of prevention and treatment of acne in adolescents are not expressed, with the exception of antiandrogen therapy. The official organization of treatment for adolescents with acne considers that any stage of acne development is multifactorial and is based on an inflammatory process. The officially recommended main method of treating acne in adolescents is the combined use of topical agents, especially the combination of benzoyl peroxide and the retinoid adapalene. In complex cases, topical treatment is supplemented with systemic therapy with retinoids (isotretinoin). Prevention of side effects of topical treatment (dry skin, hyperemia or cheilitis) is achieved through the use of moisturizing cosmetics or cream with dexpanthenol, respectively. Officially, in the field of acne prevention and therapy, such methods of influencing acne vulgaris as dermatocosmetics and corneo-or enzyme therapy are not used. The use of local and systemic pre- and probiotics for the treatment of acne has not been introduced. This reveals a connection between the development of acne and the intestinal microbiome. The use of local antiandrogens and insulin-like growth factor 1 inhibitors to control sebum production, as well as selective retinoic acid receptor agonists with a better safety profile compared to retinoid therapy for acne, may be promising for the prevention and treatment of acne in adolescents. Findings. Organization of effective treatment of acne in adolescents should include their clear instructions and correction of dietary habits. Prevention of acne today is practically not organized in the field of medical services.

The article presents current information on trends in the consumption of organic food products in the world, information from foreign studies on their impact on public health. The market for these food products has grown significantly since the late 20th century and today represents a multi-billion dollar industry with distinct production, processing, distribution and retail systems. The relevance of studying the prevalence of consumption of organic food products among the population of various countries is increasing due to the increase in the modern world of demand and motivation among social groups of the population, especially among adherents of a healthy lifestyle. The purpose of the study is to review and analyze modern scientific foreign literature on the development of the organic food market from the standpoint of health conservation and medical and social perspectives. As a result of the review, it was revealed that the understanding currently clearly manifested in various social circles of the need to improve the level of quality of health of the individual and the population as a whole, the demand for the production of safe, micronutrient-rich, healthy food products, especially products of plant origin, actualize the expansion of production organic food products. In many countries around the world, there is an increase in consumer demand for organic food products; their regular consumers are people, mainly women, who care about their health, are physically active, have a higher level of education and income, and live mainly in high-income countries. The certain high cost of organic food products, depending on labor-intensive production methods and certification costs, often leads to unequal access to organic food products. The positive impact of organic foods on health status and the risk of developing chronic diseases is associated with a higher content of biologically active compounds and a lower content of substances harmful to health. Overall, large intervention and population-based studies are needed on the health effects of organic foods, comparing the quality, safety, bioactive compounds, nutritional and phytochemical value of foods in conventional and organic production systems using new research methods.
