Enfrentamiento anestésico de paciente con picnodisostosis: a propósito de un caso picnodisostosis患者的麻醉对抗:关于一个病例

Pycnodysostosis is a rare autosomal recessive disease with osteoarticular manifestations of great relevance in anesthetic practice. People with this disease are more prone to fractures and craniofacial anomalies that anticipate a difficult-to-manage airway. We present the case of a 19-year-old woman with pycnodysostosis who underwent a reductive mammoplasty under general anesthesia.

Tendencias en procedimientos meniscales y reconstrucción de ligamentos de rodilla en Chile: estudio transversal entre 2004-2019 智利半月板手术和膝关节韧带重建的趋势:2004-2019年横断面研究

Purpose: The aim of this study was to describe the incidence rate per 100,000 inhabitants (IR) of knee arthroscopic procedures and knee ligament reconstruction in Chile, between 2004 and 2019. Methods: Cross-sectional study. Patients who underwent a meniscal surgery and patients undergoing knee ligament reconstruction were identified in the database of the Chilean department of statistics and health information (DEIS) between 2004 and 2019. Results: The DEIS reported 149,734 patients, 169244 arthroscopic procedures with a significant upward trend in the IR, from 24.61 in 2004 to 80.6 in 2019. Related to knee ligament reconstruction, 42,026 procedures were found in 38,171 patients, with a significant upward trend in the IR, from 4.08 in 2004 to 24.62 in 2019. Conclusions: Knee arthroscopy and ligament reconstruction are in an upward trend in Chile. An evident inequity in access was found public insurance needs to improve the quality and access to both procedures. An updated, national registry is urgently needed to improve data, allowing better health policies.

Teledermatología sincrónica en tiempos de Covid-19. Percepción del dermatólogo Covid-19时代同步远程皮肤病学。皮肤科医生的看法

Teledermatology (TD) is a remote medical care tool that experienced a great boom during the COVID19 pandemic, resulting in an increase in both, the number of remote consultations and the number of dermatologists who practice it. Several studies have reported a high degree of satisfaction among patients attended by this method. However, few studies have done so in dermatologists. We set out to investigate the experience of Chilean dermatologists with the tool by means of an online survey. Results. A total of 156 surveys were completed. The number of dermatologists using TD increased from 19.9% to 80.8% during the pandemic. They cited the absence of the physical examination-dermatoscopy as the main limitation to developing TD and reported feeling more comfortable with some pathologies. Dermatologists feel moderately comfortable and confident with the tool, however, they experienced varying degrees of stress and frustration. Only 40.8% plan to continue using TD in the future. Conclusion. Synchronous TD has clear limitations, however, these should emerge as an opportunity to consider in its development and its platforms, in order to generate a more comfortable tool for more dermatologists and thus extend its use, as an important tool for equal access to health care in Chile.

The use of facemask has become a key element to prevent the infection of Covid-19. Its prolonged use, both, by health personnel and the general population, has caused the appearance of multiple adverse effects at the facial level; highlighting the report of symptoms such as discomfort with the mask, facial itching, redness, erythema, among others. The appearance of dermatological diseases such as acne, allergic and irritative contact dermatitis, and exacerbation of pre-existing pathologies such as acne, rosacea and seborrheic dermatitis are also frequent. Knowledge of these complications is important for their prevention and proper management.

The sustainability of the Universidad de Chile Clinical Hospital must be understood comprehensively; providing higher quality and safety for our patients, ensuring highest standards of medical care. The quality of care must be understood not only in complying with the minimum standards to accredit and grant GES benefits, establish agreements with health insurers and be financially competitive; It must be incorporated into its management the dimensions of quality - effectiveness, efficiency, accessibility, safety, equity and patient centered care- minimizing the costs of non-quality work. Our Clinical Hospital, as our country’s main training center in healthcare professions, must include training at the undergraduate students’ curriculum in quality and patient safety issues.
