Background: Medical examination detects illness or disease risk factors in individuals otherwise well to treat early on and minimize morbidity and death. In spite of recommended GDM screening during prenatal care worldwide and local standards, it has been shown that the procedure has not been properly implemented. A further difficulty might be the lack of consensus worldwide and on local consistence on GDM screening and diagnosis guidelines. Methods: The study was a hospital based cross-sectional and descriptive study that employed both quantitative and qualitative methods of data collection. A descriptive study was concerned with describing the characteristics of a particular individual, or of a group in a given situation. The study was guided by the primary objective of assessing the extent of GDM monitoring among women receiving antenatal care at St. Francis Hospital Nsambya in Kampala, Uganda. Both Capillary and venous blood samples in which plasma/serum was taken from pregnant mothers and their glucose levels estimated using a One touch (Roche Diagnostics) code 298 and plasma calibrated glucometer and Roche Cobas C311fully automated Chemistry Analyser respectively. Both whole blood glucose and plasma/serum glucose was determined as per the principle of enzyme hexokinase (HK) catalyzes the reaction between glucose and adenosine triphosphate (ATP) to form glucose-6-phosphate (G-6-P) and adenosine diphosphate (ADP). In the presence of nicotinamide adenine dinucleotide (NAD), G-6-P is oxidized by the enzyme glucose-6-phosphate Dehydrogenase (G-6-PD) to 6-phosphogluconate and reduced nicotinamide adenine dinucleotide (NADH). The increase in NADH concentration was directly proportional to the glucose concentration and was measured spectrophotometrically at 340 nm. The data was analysed using the McNemar's test to determine the sensitivity, specificity and the direct cost for performing capillary glucose. Receiver operator characteristic and Area under curve analyses to analyze the predictive power of the constructed equation Sensitivity, specificity, false positive rate and false negative rate was computed to validate the capillary method in testing/detecting Gestational Diabetes Mellitus. Data analysis was performed using the SPSS package Results: The results presented below for both capillary and Venous Blood Glucose were based on the principle of glucose oxidase test. Cutoff for the different blood sugar level were as follows; Normal A1C test being < 5.4% and Diabetes being >5.4%. Based on the Venous Blood Glucose tests method, 89.0% (154/173) respondents tested normal, while 11.0% (19/173) had diabetes. However, capillary blood glucose testing method identified 78.6% (136/173)5 respondents as being normal compared 21.4% (37/173) having diabetes. Results obtained from the process indicate capillary glucose blood test for gestation diabetes having very low sensitivity i.e. 30% for gestation diabetes. The findings of the study will help to create

Objective: The purpose of the study is to determine the effect of Neurodevelopment Treatment Vs Body Weight Supported Treadmill Training along with conventional therapy on lower extremity among chronic stroke patients. Study Design: Randomized Controlled Trial (RCT). Place and Duration of study: This study took place n Lahore, Pakistan and the duration of the study was 6 months. Method: This study is a Randomized Controlled Trial utilized a non-probability purposive sampling technique, the primary outcome measure was Berg Balance Scale and Functional Gait Assessment which were used to check the balance and gait. The findings were then statistically analysed by non parametric tests Mann Whitney U Test and Friedman Test were used for between group and within the group comparisons respectively. Results: A total of 46 patients participated in the study among which after randomized allocation of 23(50%) in NDT and 23(50%) in BWSTT group. NDT group showed better results than the BWSTT group and the p value is 0.000 which is less than 0.05 when compared between the groups for balance at 12 weeks. NDT group showed better results than the BWSTT group and the p value is 0.001 which is less than 0.05 when compared between the groups for mobility at 12 weeks. Therefore, NDT is better than BWSTT for the improvement of both balance and mobility. Conclusion: Both of the treatment approaches have proven to be effective in treatment of balance and gait among chronic stroke patients but Neurodevelopmental Treatment has proven to be significantly more effective than Body Weight Supported Treadmill Training and has more significant results. Discussion: In the current study where NDT and BWSTT are compared to each other shows that NDT is highly effective for the treatment of balance in post stroke patients as compared to BWSTT that is only effective within the group but not as effective as NDT. Similarly Grozdek Covcic et.al in 2021 did a research on “Effects of Bobath treatment and specific mobilizations on gait in stroke patients” which indicated that Bobath technique in combination with soft tissue mobilization shows better results for the maintenance of balance and mobility in post-stroke patients. Also the Bobath treatment helped with increased AROM of the patient [1].

Objective The purpose of the study is to determine the effect of Proprioceptive Neuromuscular Facilitation Vs Balance Exercise along with Conventional Therapy for balance and gait in Chronic Parkinson’s patients. Study Design Randomized Controlled Trial (RCT). Place and Duration of study This study took place in Lahore, Pakistan and the duration of the study was 6 months. Method This study is a Randomized Controlled Trial utilized a non-probability purposive sampling technique, the primary outcome measure was Berg Balance Scale and Functional Gait Assessment which were used to check the balance and gait. The findings were then statistically analysed by non parametric tests Mann Whitney U Test and Friedman Test were used for between group and within the group comparisons respectively. Results A total of 60 patients participated in the study among which after randomized allocation of 30(50%) in PNF group and 30(50%) in Balance T group. The data shows that there is a big difference mean value difference between the two groups i.e. Balance T group showed better results than the PNF group and the p value is 0.000 which is less than 0.05 hence suggesting that the difference between both the experimental the groups after 12 weeks is highly significant. Hence we are rejecting the Null hypothesis and accepting the alternating hypothesis. . The data shows that there is a difference mean value difference between the two groups i.e. Balance T group showed better results than the PNF group and the p value is 0.492 which is more than 0.05 hence suggesting that the difference between both the experimental the groups after 12 weeks is non-significant. Hence we are rejecting the Alternate Hypothesis and accepting the Null Hypothesis. Conclusion Both of the treatment approaches have proven to be effective in treatment of balance of lower limb among chronic Parkinson’s patients but the Balance Exercises has proven to be significantly more effective than of Proprioceptive Neuromuscular Facilitation and show effective results in the treatment of balance more efficiently but among two of these treatment strategies none of them has been proven to be better than other for gait. Discussion In the current study where PNF and Balance exercise are compared to each other shows that Balance exercise is highly effective for the treatment of balance in chronic Parkinson’s patients as compared to PNF. However Del Carmen et.al. Conducted a research in 2019 i.e. “the development of ballet exercises with Proprioceptive Neuromuscular Facilitation techniques for patients with Parkinson’s disease” which indicated that of Proprioceptive Neuromuscular Facilitation technique also improve balance and gait in Parkinson’s disease patients [1].

Background: Chronic diseases lead to both financial and organisational burdens on the health system. One of the possible solutions to keep health care affordable is self-management. However, non-adherence is a recognised problem with regard to self-management, especially in long-term treatment. Once it is clear who is or is not adherent, care can potentially be tailored accordingly. To predict adherence over time, it may be useful to understand how this variable might change over time: It is unclear whether adherence is a constant, or rather increases or decreases or fluctuates. The aim of this study is to determine the course of exercise adherence over a 12-month period in patients with chronic obstructive pulmonary disease (COPD) receiving prolonged rehabilitation. Methods: In this prospective cohort study participated 196 patients with COPD, who were undertaking pulmonary rehabilitation (PR), for at least one month, in 53 primary physiotherapy practices in The Netherlands and Belgium between January 2021 and August 2022. Outcomes were measured at baseline, and at 3, 6 and 12 months. The primary outcome was exercise adherence, and secondary outcomes were exercise capacity and health related quality of life. Multilevel regression analysis with adherence at the first level and patients at the second level were used for analyses of the data. Results: There was no significant change in exercise adherence over time (p = 0.89). Also, exercise capacity (p = 0.59) and health related quality of life (p = 0.24) remained stable. Conclusions: Adherence is constant over a period of 12 months in patients with COPD receiving prolonged pulmonary rehabilitation in a primary physiotherapy practice in The Netherlands and Belgium. Also, measured health outcomes remained constant in this same period. Possibly, this will allow for better tailored care over 12 months and lower health care costs.

According to the American Heart Association, cardiovascular diseases are in the first place among the diseases that cause the most deaths in the world, responsible for 40,6% (95% confidence interval [CI] 24,5%-54,6%) of the world’s total deaths. There are many risk factors for CVD, including: smoking, alcohol, high blood pressure, high blood cholesterol, being physically inactive, obesity, diabetes and stress. It should be underlined that stress is taken into account less than others. The effect of chronic and acute stress on the occurrence of CVD is slightly different. Acute stress causes a high amount of catecholamines to be secreted in the blood. These hormones can cause many changes in the body, including tachycardia, high blood pressure, coronary vasoconstriction and/or stress cardiomyopathy. Stress cardiomyopathy, also known as Takotsubo syndrome (TTS) is an interesting side effect of acute stress, that causes changes in heart anatomy and physiology. Suddenly experienced strong emotional stress causes the left ventricle to change shape and resemble a balloon. The pathophysiology of the syndrome is not well understood, but many experts assume that high catecholamine levels can induce this pathology. Also, the condition is thought to be responsible for 2% of all acute coronary syndrome cases presenting to hospitals. Chronic stress affects cardiovascular system a little differently. According to a longitudinal and cohort study, constant stress increases risk of the heart attacks, angina and stroke. We know amygdala, an area of the brain that plays an important role in stress responses. When we experience constant stress, our amygdala sends signals to the bone marrow to produce more white blood cells, which then causes leukocytosis. This in turn causes the arteries to become inflamed and can lead to some cardiovascular and other problems like i mentioned before: Heart attacks, angina and stroke [1-4].

Virtual simulation technology of radiotherapy has completely changed the way we teach and learn. In the context of medical school education, the technology bridges the gap between abstract knowledge and ordinary life. Technology advances the possibilities available to mankind and are essential to medical professors in the teaching. However, educators are faced with the challenge of guiding students through this transition and ensuring they don’t lose valuable time using their devices. The rise of technology in learning has also carried some disadvantages with it, some of which might be overlooked. This study aimed to determine the attitude and motivation of medical professors to adopt virtual simulation technology of radiotherapy in clinical teaching context. Descriptive statistics, t-test analysis, one-way ANOVA, Pearson-r correlation, and chi-squared test were used in this study. It is recommended to upskill the technological and pedagogical knowledge and skills of faculty member, crucial especially in the context of a post-pandemic education.

Nurses represent the largest number of health care providers and they are an essential part of the healthcare workforce working on the frontline during the COVID 19 pandemic. They are in direct contact with patients, providing high levels of care to promote patient safety and positive care outcomes while at the same time placing their own health and well-being at considerable risk. The Aim: To investigate the cause of COVID 19 infection amongst nurses. Methods: A self-administered online questionnaire was administered to the targeted sample who participated on a voluntarily basis. Result: 123 surveys were completed. The majority of the participants were compliant with infection precaution protocols. More than half of them were aware of the source of infection. Conclusion: Nurses are well oriented in dealing with Covid 19 crisis. However, more awareness is needed regarding infection precaution and reducing level of anxiety.

Background Web based learning can provide nurses with the opportunity to develop new knowledge and skills for management and prevention of diseases [1] including the emergence outbreak. However, with regard to covid-19 outbreak in Zanzibar only traditional method of face-to-face was employed to train nurses which resulted into a very low number of nurses who received proper training on prevention and management of covid-19. Aim To design and tests the effect of web based training tool on knowledge and skills for pandemic preparedness and responses among frontline nurses from Zanzibar. Methods A pre-posttest single group quasi-experimental study was conducted in set of 139 nurses from Zanzibar to compare change in knowledge and skills followed by web based educational intervention. A web based training system was developed by researcher, learning content were adapted and modified from CDC, WHO and MoHCDGEC Tanzania. A computer-generated questionnaire with a total of 60 items was used and data were analyzed using SPSS software. Results Findings from the study indicates significant change in the knowledge scores followed by web based intervention with mean difference of 3.58 (P < 0.001) and cognitive skills scores, mean difference of 3.07 (P < 0.001). Only baseline scores were found to be significant predictors for changes among covariates (β = -0.66 and P < 0.001) for knowledge and (β= -0.62, and P <0.001) for cognitive skills. The range of system rating by users was varied across quality and usability variables from (M = 2.50, SD = 1.265) to (M = 4.04, SD = 1.188) indicating the level of satisfaction toward the developed web based system. Conclusion The developed web-based system was successful improved nurses’ knowledge and skills for pandemic preparedness and response. Nurses positively evaluated the system in term of quality and usability.
