BACKGROUND: The use of platelet-rich plasma (PRP) and its modifications are a promising treatment method for chronic inflammatory diseases of the cornea. AIM: To evaluate and compare the clinical and morphological features of the corneal regeneration during experimental chronic persistent corneal erosion with the use of PRP and two-component autofibrin glue (TAFG). MATERIALS AND METHODS: The studies were conducted in 18 rabbits (36 eyes). After modeling chronic persistent corneal erosion, the animals of the 1st group were treated with TAFG; animals of the 2nd group with instillations of PRP, animals of the 3rd group with standard medical therapy. RESULTS: By Day 14 of the experiment, the restoration of the transparency of the cornea, no erosion and new vessels in animals of the 1st and 2nd group were noted. This pattern persisted until Day 60. By Day 30 of the experiment, the presence of corneal clouding in optical zone, neovascularization over the whole cornea and an epithelial defect (3,85 [3,50; 4,47] %) in animals of the 3rd group were recorded. By Day 60, a complete epithelialization of the cornea was observed. CONCLUSIONS: The use of PRP and TAFG allows reducing the time of corneal epithelialization by 2,5 times in conditions of experimental chronic persistent corneal erosion. There were no significant differences between the use of PRP (within 5 days) and TAFG, however, treatment with an adhesive composition is more convenient for both the doctor and the patient a single use that does not require multiple use of expensive consumable materials.

BACKGROUND: Anti-angiogenic therapy for diabetic macular edema (DME), recognized as the gold standard, is not always effective. When compensating for the general somatic status, it is necessary to search for local causes of DME resistance to anti-angiogenic therapy. AIM: To study the effectiveness and features of anti-angiogenic therapy for DME in normal and pathological vitreoretinal interface (VRI). MATERIALS AND METHODS: Patients who received anti-angiogenic therapy for 12 months, in addition to the standard examination, underwent optical coherence tomography with an assessment of morphometric parameters and VRI. RESULTS: In addition to the groups of normal and pathological VRI, a group of transformation from pathological to normal VRI was identified. Visual acuity increases with normal VRI, decreases with pathological. OCT scores decrease in both groups. In the transformation group, an increase in vision and a decrease in OCT parameters were observed only after VRI transformation. CONCLUSION: The pathologic condition of the VRI reduces effectiveness of anti-angiogenic therapy for DME, except for 10% of cases in which VRI is transformed into normal within 5-6 months from the start of treatment. These data should be taken into account when choosing a treatment strategy for DME.

BACKGROUND: Ocular prosthetics remains an important and actual task in the rehabilitation of patients with anophthalmos of various origins. AIM: To increase the effectiveness of cosmetic prosthetics in patients at the formation of a primary and/or delayed supporting stump with simultaneous reconstructive procedures on the orbital bone structures. MATERIALS AND METHODS: A retrospective analysis of patients with subatrophy and anophthalmos with deformities of the orbital walls was carried out. Results were analyzed according to the criteria for the severity of clinical signs enophthalmos, deepening of the superior orbitopalpebral sulcus, prosthesis mobility. RESULTS: 22 patients with orbital bone deformities were operated and examined; they underwent surgery on the orbital walls in combination with evisceration with posterior scleral pole resection, neurectomy and implantation of a spherical endoprosthesis (in modifications) in 12 patients and delayed stump plasty in 10 patients with anophthalmos. It was possible to eliminate enophthalmos in 18 patients, to correct the retraction of the upper eyelid on the prosthesis side in 19 patients, and to significantly increase the mobility of the prosthesis in 9 patients. CONCLUSIONS: Carrying out a combined surgical procedure aimed at restoring a voluminous mobile primary or delayed supporting stump and eliminating the deformation of the orbital walls with restoring the lost volume creates conditions for optimal individual ocular prosthetics, significantly reduces prosthesis enophthalmos, increases its mobility and improves cosmetic results of rehabilitation.

BACKGROUND: Adult-onset vitelliform macular dystrophy is one of the pathologic conditions , which successfully masks as age-related macular degeneration. AIM: To assess the incidence of the adult-onset vitelliform macular dystrophy among patients of a major ophthalmological clinic in the Far Eastern Federal District of Russia. MATERIALS AND METHODS: We revealed the cases of adult-onset vitelliform macular dystrophy among 1000 patients aged 40 years and older who addressed for various ocular complaints. RESULTS: Adult-onset vitelliform macular dystrophy was found in 2 male patients aged 43 and 66 years. In the macular area of both eyes of the 1st patient, round yellow lesions in the foveola, local detachment of neuroepithelium, optically dense material deposits on the outer photoreceptor segments were detected (vitelliruptive stage of adult-onset vitelliform macular dystrophy). In the left eye of the 2nd patient, in the foveola, a single round yellow focus of 200 m was found, optically dense vitelliform material was detected between the pigment epithelium and neuroepithelium layers without any signs of choroidal neovascularization (vitelliform stage of adult-onset vitelliform macular dystrophy). CONCLUSIONS: The incidence of adult-onset vitelliform macular dystrophy among patients with various ocular conditions was 0.2%. Morphological changes in the macula consist in presence of deposits of vitelliform material, localized between the neuroepithelium and retinal pigment epithelium layers.

Glaucoma is one of the leading causes of irreversible blindness in the world. Reducing intraocular pressure is the only way to slow down the progression of glaucomatous optic neuropathy. Minimally invasive glaucoma surgery aims to provide a safer way of reduction of intraocular pressure than traditional methods, and at the same time it is capable to reduce dependence on antihypertensive therapy. Cyclodestructive high-precision method of reducing the production of aquоeus humor occupies a confident position among modern minimally invasive glaucoma surgery methods. The data obtained as a result of studying the literature confirm our idea on the endoscopic laser cyclodestruction method as a minimally invasive, safe, reliable antiglaucomatous component of the combined surgical treatment of cataract and glaucoma.

BACKGROUND: The most characteristic manifestation of endocrine ophthalmopathy (EO) is the proptosis development in a patient. It is possible to correct the displacement of eyeballs by performing an orbital decompression surgery. AIM: To evaluate the clinical efficacy of treating patients with endocrine proptosis using transconjunctival internal decompression of the orbit. MATERIALS AND METHODS: The study included 86 orbits of 43 patients with bilateral proptosis. All patients underwent MSCT examination, proptosis was detected due to an increase in the soft tissue component. Transconjunctival decompression of both eye sockets was performed according to the described method, and the amount of eyeball displacement after surgery was investigated. RESULTS: Patients complaints about constant pressure behind the eye, observed in 39 patients, disappeared during the first day after surgery in 21 patients, in the remaining patients they gradually disappeared within a week. In 32 patients with preoperative diplopia after surgery, in 27 it completely disappeared, in the remaining 5 it remained in the extreme positions. 6 months after surgery, a decrease in proptosis amount from 21.1 1.5 mm to 20.6 1.5 mm was noted, visual acuity was 1.0 0.02, decreased visual acuity remained in 3 cases due to the incipient cataract, IOP decreased from 20 1.2 mm Hg to 19 1.3 mm Hg. There were no eyeball movement restrictions at the control examination. CONCLUSIONS: Internal (soft tissue) decompression of the orbit is an effective method for proptosis correction exophthalmos in patients with lipid EO form. Carrying out the decompression surgery through the conjunctival access allows to constantly monitor the shape and size of the pupil in the operated orbit, to conduct controlled hemostasis of the orbital tissues. The use of preliminary calculations of the volume of soft tissues to be removed (according to MSCT) makes it possible to obtain a predictable effect of the surgery in variable degrees of proptosis. Transconjunctival decompression helps avoiding cicatricial processes of the eyelid skin.

In 32 patients with preoperative diplopia after surgery, in 27 it completely disappeared, in the remaining 5 it remained in the extreme positions. 6 months after surgery, a decrease in proptosis amount from 21.1 1.5 mm to 20.6 1.5 mm was noted, visual acuity was 1.0 0.02, decreased visual acuity remained in 3 cases due to the incipient cataract, IOP decreased from 20 1.2 mm Hg to 19 1.3 mm Hg. There were no eyeball movement restrictions at the control examination. \u0000CONCLUSIONS: Internal (soft tissue) decompression of the orbit is an effective method for proptosis correction exophthalmos in patients with lipid EO form. Carrying out the decompression surgery through the conjunctival access allows to constantly monitor the shape and size of the pupil in the operated orbit, to conduct controlled hemostasis of the orbital tissues. The use of preliminary calculations of the volume of soft tissues to be removed (according to MSCT) makes it possible to obtain a predictable effect of the surgery in variable degrees of proptosis. In the present article, cases of lacrimal apparatus conditions emerging after a new Coronavirus infection COVID-19. The aim of the study is to determine the causes of epiphora in patients after Coronavirus infection. 26 patients (30 eyes) were examined, aged from 28 to 70 years, complaining of tearing, which emerged for the first time ever not earlier than 514 days from the onset of the laboratory-confirmed Coronavirus infection COVID-19, which had a mild or a moderately severe course and was accompanied by anosmia. In patients, following conditions of the lacrimal system were revealed: in 22 patients, there were pathological changes of the horizontal portion of lacrimal pathways; in 6 people dry eye syndrome was diagnosed: in 3 people, it was of mild severity, manifested by hyperlacrimia, 3 people had moderate severity of the dry eye syndrome. As concomitant, following signs were revealed: in 7 patients rhinologic conditions were present, in 2 people neurologic signs. In the examined group of patients with epiphora, we found that in 1.53 month after Coronavirus infection COVID-19 with anosmia, as a common sign of the disease in more than a half of cases, a development of pathological changes of the horizontal portion of lacrimal pathways was revealed.

AIM: To work out the technique of saturation of the preserved amniotic membrane (AM) with platelet rich plasma (PRP) lysate and to evaluate the growth-stimulating effect of a combination of AM and PRP lysate in vitro. MATERIALS AND METHODS: In the experiment, AM samples preserved in 3 ways were used: silicate drying, lyophilization, cryopreservation. PRP lysate was prepared on the basis of volunteers blood. During the exposure of AM with PRP lysate, the optimal saturation time of canned AM with lysate was determined, the volume of lysate that 1 cm2 of AM could adsorb was estimated. The growth-stimulating effect of AM transplants was evaluated in the culture of human buccal epithelium. The dynamics of cell growth was evaluated after 1, 2 and 3 days from the moment of sowing. RESULTS: In the presence of PRP lysate, the mass of silicatedried AM increased 4.2 times, lyophilized AM 4.8 times, cryopreserved AM 1.8 times. AM samples obtained by lyophilization adsorbed PRP lysate most effectively. Five minutes of exposure with PRP lysate are enough to fully saturate the AM. AM without PRP lysate did not give a growth-stimulating effect. CONCLUSIONS: When comparing experiments with PRP lysate without AM and AM with PRP lysate, it was found that the greatest stimulation of cell growth occurred when using lyophilized AM and PRP lysate. Saturation of cryopreserved AM with PRP lysate was ineffective, and when using silicate-dried AM impregnated with PRP lysate, the greatest growth-stimulating effect was observed on the 1st day.
