Abstract: The book ‘Near to The Wild Heart’ was the first novel of the writer Clarice Lispector, it tells the story of Joana, a character that since childhood shows herself provocative, questioner and uncertain of self and from the own world. Lispector’s character reveals, since the first phrases from the book, a suffering for something that her own can’t understand. A silent and constant pain. It remained for the character, in the meantime, to wait. We seek to analyze the descriptions about the suffering of the character, associating with the psychosocial representation of the woman in the work of Clarice as a representative of The Generation of 45. This article followed a method of documentary analysis, using, in addition to Lispector’s masterpiece, bibliographic studies related to the themes of the research. The character Joana faces a discomfort of imprecise location and undefined location. Yet, a suffering that transgress in time. A wild time is drawn in the work, in which Joana lives in streams of events. The work is a rupture of linearity. The childhood wouldn’t be only a memory of the character, but the own present alive. We highlight as a latent difficulty the reduced amount of research in this methodological line that could contribute to the inferences about the literary work of Lispector. Certainly Joana faces a process until then unrecognizable in Brazilian literature: the pain of the lack of the “I”. Knowing that something is missing and awaiting the revelation of this lack in silence, reverberates in the character a psychic pain. Abstract: Morgagni Hernia (MH) is a defect between the transverse septum and the sternum, failing the fusion of the sternal and costal parts of the muscle, representing 3 to 4% of the hernia’s diaphragmatic lesions of congenital origin. It is present in 90% of the cases on the right side, 8% on the left side and 2% on the bilateral side. The aim of this article is to report a case on late presentation of MH and to emphasize the importance of this pathology to be analyzed with differential diagnosis in patients with respiratory symptoms. The subject is a patient, male, 62 years old, truck driver, married, with history of dyspnea on medium exertion and mild chest discomfort from 2 years. After 1 year of these symptoms, he evolved with major dyspnea on minor exertion, intense chest pain, without improvement with analgesics or with body positions, making it difficult to perform his usual activities as work and leisure. Multislice Computed Tomography of the Thorax, which showed: diaphragmatic hernia in the left hemithorax, with a ring/diaphragmatic fault located in the anterior and medial pericardial third, and an intrathoracic hernia, notably in the base and middle third in the anterior, lateral and posterior regions, with contents mesenteric and intestinal. The patient underwent thoracotomy in the sixth left intercostal space. A large amount of omentum, transverse colon, stomach, pulmonary atelectasis and small pleural effusion were found. The omentum resection was performed, located hernial ring in the antero-medial portion of the diaphragm, reduction of the hernia, placement of the Malex mesh and apposition of two anterior and posterior thoracic drains. Reports of patients initially diagnosed with pneumonia or treated as chronic dyspeptic, who progressed to intestinal necrosis, respiratory failure or other complications due to late diagnosis, emphasize the relevance of the knowledge of MH as a differential diagnosis of respiratory and gastrointestinal symptoms.

Carbohydrate intolerance is relatively common in childhood, but its diagnosis and management are still quite precarious. Hereditary fructose intolerance (HFI) is an autosomal recessive disease that results in deficiency of the enzyme aldolase B, which contributes to the onset of gastrointestinal and metabolic symptoms, triggered by the ingestion of foods high in fructose, sucrose or sorbitol. Methodology: For the accomplishment of such a study a search of the literature was done from August to September of the year 2018 with publication period of a maximum of 10 years. The theoretical reference was elaborated through the collection of relevant scientific articles on the subject, made in the electronic databases: Scientific Electronic Library Online (SciELO), Pubmed, EBSCOhost and CAPES, from descriptors generated by DeCS: "Fructose Intolerance"; "Child" and its correspondents in English. Thus, 81 articles were obtained and, from the title of the literature and its abstracts, were used. 19 Ademias, articles that were related to the topics covered in this study or whose sample was not composed by humans were also discarded. The diagnosis of HFI is based on the suggestive clinical picture initiated after the ingestion of the fructose, sucrose and sorbitol already mentioned, associated with the use of invasive and noninvasive examinations, but the confirmation is based on the response to the improvement of the symptoms after the restriction of the ingestion of such food, which constitutes the best therapy. Conclusion: Based on the consequences of inadequate management of HFI, it is of fundamental importance that the affected children have an early diagnosis, associated with an adequate nutritional monitoring, which enables an improvement in the quality of life of these individuals, besides preventing important repercussions such as renal and hepatic impairment. INTRODUCTION: Realistic simulation is increasingly present in academic curricula. The traditional method of teaching is being gradually replaced by new methodological approaches, which reliably reproduce real situations and produce technical knowledge and skills in medical students in the medical emergency.Realistic simulation is increasingly present in the academic curriculum. The traditional method of teaching is being gradually replaced by new methodological approaches, which reliably reproduce real situations and produce technical knowledge and skills in medical students in the medical emergency. The aim of this study was to carry out a systematic review of articles on realistic simulation as an educational tool in emergency medicine. A systematic review of articles on realistic simulation as an educational tool in emergency medicine, published in the corresponding period from January 1th, 2014 to October 28, 2018, was carried out in the SCOPUS database. The keywords were "simulation training", "medical students" and "emergency medicine". The research strategy resulted in 124 references. After the search for the title and summary of the citations considered for eligibility based on the inclusion criteria of the study, 106 articles were excluded, and 18 articles were retrieved and included in the final sample. The studies were distributed in 5 categories to better systematic review of the organizational quality, they are: Realistic simulation associated with the ultrasonography in the emergency; Realistic simulation and electronic devices; The effectiveness of realistic simulation in resource optimization; Impact of simulated training on student confidence; Realistic simulation as a tool in the development of skills and knowledge. This study presents the main aspects of realistic simulation as an educational tool in emergency medicine. It can be concluded that this new teaching approach has several benefits for students to learn, such as: stimulating students in the learning process, retaining technical skills in procedures, providing a safe environment for users to repeat optimization of time and financial resources.Keywords: realistic simulation, emergency medicine, educational, systematic review

Throughout history, women have been facing several situations of disregard for basic human rights. In times where entire populations are forced to leave their homes and countries to achieve protection and a minimum condition for survival, as in the case of armed conflicts and political crises, gender relations become even more evident, as women are easy targets of physical and psychological aggression. In these times of greater scarcity of basic resources, they are responsible for caring for the most vulnerable families, the elderly and children. The countries of origin of the refugees - Syria, Iraq, Venezuela, among others – also face problems like inequality and patriarchalism. Refugees are sometimes subjected to sexual violence and, in order to protect themselves, are also forced to marry as children, for instance. These women and girls are not generally seen as human beings but as objects of exchange and weapons of war. It is necessary for international authorities to guarantee protection, shelter and care for refugee women without being required in return for such needed assistance.

The causes of forgetfulness episodes in the elderly can occur for a variety of reasons, among which we highlight the presence of dementia, which can negatively infer senile cognition, memory, language and communication. Dementia is a condition in which the individual loses part of the brain functions, causing neurological deficits that directly affect his quality of life. This study discusses the importance of Neuropsychological Assessment as a fundamental tool for the diagnosis of degenerative disease, before the complaints of oblivion related to the advancing age. The results indicated that the use of this method of examination can indicate the therapeutic intervention in an adequate time, allowing the improvement of the quality of life for the elderly.

This study reported the case of a patient with a diagnosis of colonic and renal synchronic tumors: Moderately differentiated adenocarcinoma of sigmoid and renal carcinoma of the synchronic clear cell variant in a 66-year-old female patient who were surgically resected in a single approach in an Oncology service in the interior of the State of Ceará, Brazil. For data collection, records of the medical record, interview with the patient and the surgeon responsible for the case, photographic record of the diagnostic methods to which the patient was submitted, and literature review were used. It followed all the ethical principles established by law 466/2012. From this study it was concluded that: a) the incidence of colorectal cancer and synchronic renal cell carcinoma ranges from 0.03 to 4.85% in the analyzed literature, b) risk factors for synchronic tumors include genetic and environmental factors; c) renal cell carcinoma is usually diagnosed by performing preoperative tests for staging of abdominal carcinomas. Still, large prospective studies are necessary to define the best therapeutic strategy, however, some authors have confirmed that simultaneous laparoscopic resection of both neoplasms is possible and presents low morbidity and mortality. Rett syndrome has yielded many articles over the years since its discovery. This is a production that aims to recover some concepts already brought by other authors, to fix the genetic aspect of the affection, as well as the clinical evolution and associated diseases. Methodology: The screen study is the result of consultations in national and international databases, most notably PubMed and Scielo as a source of academic articles. In the consultations we used the following keywords: Rett Syndrome, Mental Deficit, Multiple Deficit, Epileptic Crisis, and Genetics, we still serve three journals: Genomics & Genetics Weekly, Genetic, Social and General Psychology Monographs and Journal of Medical Genetics of the English equivalent as descriptors: Rett Syndrome, Mental Limitation, General Limitation, Epilepsy and Genetics. In our consultation we found 185 articles and of these, we use 32 to compose our bibliographic reference. Keywords: Rett syndrome. Mental deficiency. Multiple Deficit. Epileptic
