Through the use of case studies, this article explains the purpose and importance of the use of advance care plans in care homes and the potential consequences of not having an advance care plan in place regarding the care of individual residents. It explores reasons why they may not be completed or used effectively, and considers why the design of the advance care plan is important and needs to be specific for care home use.
