Teleradiology in Tripura: Effectiveness of a Telehealth Model for the Rural Health Sector 特里普拉邦的远程放射学:农村卫生部门远程医疗模式的有效性

With the aim of facilitating early diagnosis and improving health care outcomes in the rural areas of northeast India, Teleradiology Solutions in Bangalore, Karnataka established an alliance with 26 Community Healthcare Centers (CHC), District (DH), and Sub-district (SDH) hospitals located over 3000 km away in Tripura. Digital Imaging and Communications in Medicine (DICOM) images of radiographs of patients from the sites were uploaded to a cloud-based Radiology Information System and Picture Archiving and Communication System (RIS-PACS) for interpretation by expert radiologists at a Teleradiology reporting hub in Bangalore. Over a period of 3 years, starting from January 2018, 78622 studies were interpreted via teleradiology. All age groups and study types were included. The mean turnaround time (TAT) for the report to reach the site once the images had been received in Bangalore for all studies was 3.19 hours (95% confidence interval [CI]: 3.22 - 3.16). This meant that patients could consult their physician with the report on the same day, enabling delivery of optimal care at the same visit. We conclude that teleradiology has proven to be a value-added service in rural healthcare in the remote North East of India.

Sensors Based Wearable Device for Hemophilia Patients in Programmed Sports Therapy 基于传感器的可穿戴设备用于血友病患者的程序化运动治疗

Hemophilia is a genetic disorder where internal bleeding occurs at the joints like knee, ankle, elbow, shoulder, hip etc. This internal bleeding is due to a defect in clotting factors. Tibio-femoral joint is more prone to the disorder. Total knee replacement is a safe and proven surgical procedure for hemophiliacs. After the surgery, the ‘World Federation of Hemophilia’ suggests a programmed sports therapy. The exercises are done once after the bleeding have stopped. If any bleed occurs during therapy, the exercise must be stopped as it induces infection. As a result, deformity and stiffness occur in muscles. Due to stiffness, the patient’s position is so that he applies more force on the toes. An improved wearable device is proposed which will measure the range of bending in knee joint, measure the force applied at the toes against the ground and measure the position of the joint while the person does the sports therapy exercises. The proposed work will inform the change in positions and related measurements to physician’s mobile phone via IoT. With this proposed wearable device, the physician could monitor the patient’s status from any place with the help of the internet. The usage of the proposed device in the knee joint will eradicate 97.6% of issues faced by the Hemophilia patients as they are unaware of the bleeding condition because of their wrong positions.

Belinostat Therapy for Peripheral T-Cell Lymphoma Belinostat治疗外周t细胞淋巴瘤

Peripheral T-cell lymphoma (PTCL) is a type of aggressive heterogeneous non Hodgkin type of lymphoma with poor historical outcomes. The responses to conventional chemotherapy remain poor, and many rebound types of cancer growth are seen after initial therapy. The belinostat drug works by inhibiting histone deacetylase and is approved for refractory or relapsed PTCL. The metabolism of Belinostat drug is done by Cytochrome p450, which takes place in the liver. Besides this, no fixed dose combination of belinostat is available for its use as inducer or enzyme inhibitor. Belinostat is well tolerated with mild side effects, including vomiting, nausea, pyrexia, fatigue, and anemic condition in patients undergoing therapy for PTCL. Belinostat therapy is an effective and safe option to treat PTCL. This article mainly deals with pharmacology, drug therapy, efficacy, and applications of belinostat against relapsed or refractory (PTCL).

Multidimensional Evidence Generation: A Paradigm Shift Post-Pandemic Enhancing Policy Implementation for India’s Health System 多维证据生成:大流行后的范式转变,加强印度卫生系统的政策实施

COVID-19 is having a profound impact on healthcare and society. The crisis has drastically shifted our focus and defined a new normal in terms of way of thinking and process of evidence gathering. Across the landscape of health, the response in future will see transformation on data, methodology, and experiences based on all stakeholders. The future of health care not merely will depend on convenience and less expensive technology but also will be multidimensional evidence-based while enhancing patients’ personal health and needs in real time. This will require integrated evidence-generation rather siloed functioning of departments, disciplines, markets, and geographies. There will be radical ways to operate that will smoothly connect evidence generating process from randomized clinical trials to patients’ medical and non-medical history, health economics and outcomes research, market access, and precision prevention strategies. That is, the paradigm shift incorporating partnership model will simultaneously enhance data engineering platforms, advanced analytics capabilities, modeling-based decision process, and cross-functional collaborations.

A Review on Continuous Glucose Monitoring (CGM): Perspectives on Glucose Biosensors 连续血糖监测(CGM):葡萄糖生物传感器的研究进展

This review highlights the need for continuous monitoring devices (CGM) in comparison with the self-monitoring of blood glucose (SMBG), with a little focus on various marketed CGM devices. It provides significant attention to the latest generations of glucose biosensors and enzymatic glucose bio sensing. The advantages and limitations of various physiological samples for continuous glucose monitoring are discussed. Also, discussed are the use of semipermeable membranes to improve the biosensor’s sensitivity and selectivity while retaining its stability.

Analysis and Classification of Muscular Paralysis Disease using Electromyography Signal with Machine Learning 肌电信号与机器学习对肌肉麻痹疾病的分析与分类

Electrical activity of the muscles is characterized by Electromyography (EMG) signals. The EMG signal analysis form the basis for the diagnosis of muscular paralysis. The EMG signals from amyotrophic lateral sclerosis (ALS), and Myopathy are considered to analyze the paralysis. The statistical analysis of EMG signals from ALS, Myopathy, and Normal conditions, aid in the analysis and classification of paralysis. This work intends to analyze and classify the paralysis and normal conditions using EMG features extracted in time and frequency domains. Twelve statistical features are extracted from the EMG signals considered. Machine Learning techniques and deep learning techniques (DLT) are employed to perform the classification. multi-layer perceptron (MLP), support vector machine (SVM), random forest (RF), gradient boosting (GB), and nearest neighbor (NN) classifier models are used for the classification. The accuracy of the classifiers is calculated. The accuracy values obtained are 72% for MLP, 73% for SVM, 72% for RF, 71% for GB, and 69% for NN. The performance accuracy is better in SVM compared to other classifiers.

Design and Developing a Device for Migraine Treatment: A Review 偏头痛治疗装置的设计与研制综述

Migraine is usually somatosensory input to the head involving pseudo-unipolar trigeminal and upper cervical branches. It is a very common disorder involving visual perception difficulties, thereby causing cognitive decline. Advancement in migraine treatment is always a crucial point of research for every neuro-ophthalmologist.Results: Many novel treatment approaches have received Food and Drug Administration approvals for well-outlined therapies in migraine treatment. In this review, we summarize this treatment methodology and prototype a device that can cater to the limitations of already available migraine treatment devices in the market. Many novel and stirring therapies continue to evolve for the therapy of migraine. Though highly advanced and sophisticated, this therapy gets knock-down due to many limitations. This limitation renders them from being effective. Being on track with this commonly occurring disorder is important in reducing the disability from it.

Advancement in migraine treatment is always a crucial point of research for every neuro-ophthalmologist.Results: Many novel treatment approaches have received Food and Drug Administration approvals for well-outlined therapies in migraine treatment. In this review, we summarize this treatment methodology and prototype a device that can cater to the limitations of already available migraine treatment devices in the market. Many novel and stirring therapies continue to evolve for the therapy of migraine. Though highly advanced and sophisticated, this therapy gets knock-down due to many limitations. This limitation renders them from being effective. Knowledge is the stepping stone to innovations. Innovation in healthcare is much needed not just due to the technological development happening globally but also due to the new situations because of emerging diseases life matters and the care processes changing in the healthcare systems nationally and globally. Innovations are result of thoughtful ideation prototyping, testing, validation, batch production, and monitoring of clinical results. The documentation and analysis of the results and the entire innovation process pathway is necessary and critical to the success of innovation. The foremost tool available to ensure this is a publication of these results in credible journals and ensuring that addition is done to the world-body of knowledge.
