A Literature Review of the Treatment of Black Triangles 黑三角形处理的文献综述

Black triangles are a result of periodontal disease and can also be a response to treatment and return of health. They can also be a result of orthodontic treatment and tooth and root shape and position. They are perceived as unaesthetic and there is an increase in the demand for treatment. This paper will look at some of the treatment options that are available to treat black triangles. The treatment is often multidisciplinary and can involve orthodontics, surgery and restorative. As aim of this thesis is to aid the prediction of black triangles, as a solution to them needs to be sought. This paper explores the treatment of black triangles to help the clinician give the patients the options if it is predicted a black triangle may be present. Method: An OVID MEDLINE search was undertaken using the term Black triangle AND treatment. This yielded 32 papers and of these 14 were related to dental black triangles. These 14 papers were the hand searched for their cited references and this yielded an additional 17 papers. Conclusion: The treatment for black triangles can be difficult and in the case of surgical management very unpredictable. More research into simple predictable management needs to take place.

5-year Results of Neoss Dental Implants Restored at Implant-Level. A Retrospective Follow-Up Study 新生牙种植体在种植体水平修复的5年结果。回顾性随访研究

One way of reducing the cost for screw-retained implant-supported dental prosthesis is to avoid the use of prosthetic abutments. However, concerns have been raised that this might lead to complications such as extensive marginal bone loss resorption and implant loss. The aim of the study was to retrospectively evaluate a cohort of consecutive patients treated with implant-level prosthetic constructions after 5 years in function. A total of 49 consecutive patients previously treated with 102 hydrophilic dental implants (Neoss Proactive, Neoss Ltd, Harrogate, UK) in two private dental clinics were included in the study. Fifty-four implants were installed in maxillae and 48 in mandibles to replace single teeth (n = 21), to support partial bridges (n = 26), total maxillary bridges (n = 2), or mandibular overdentures (n = 2). The majority of patients (n = 37) had implants placed in healed sites without any adjunctive procedures. In 12 patients, implants were immediately placed in extraction sockets or in conjunction with maxillary sinus floor augmentation. A submerged healing period of 3 to 4 months was used before healing abutments were connected to the implants. Impressions were taken after 7 to 10 days. Baseline (abutment connection surgery), 1and 5-year intraoral radiographs were used to measure marginal bone levels and calculate bone loss. Two mandibular implants were lost (2%) during the 5 years in function. The average marginal bone loss amounted to 0.7 ± 0.7 mm after 1 year and 0.9 ± 1.1 mm after 5 years. There was no correlation between insertion depth and bone loss. It is concluded that the use of screw-retained implantlevel prosthetic constructions resulted in high implant survival rate and minimal marginal bone loss after 5 years in function.

Extraction of Remaining Teeth and Same Day Loading of Neoss Proactive Dental Implants with a Full-arch Fixed Provisional Bridge. A Survival Analysis 用全弓固定临时牙桥拔除残牙及当日装填新牙期主动种植体。生存分析

Removal of remaining teeth and same day loading of an implant-supported full-arch bridge may be a sensible treatment modality for many patients presenting with a severely diseased partial dentition. The aim of the present study was to retrospectively analyse 30 consecutive patients (21 female/ 9 male, mean age 61.9 + 11.1 years) subjected to removal of all remaining teeth, placement of four to six implants and loading of a full-arch provisional bridge the same day. A total of 156 dental implants (Proactive Straight, Neoss Ltd, Harrogate, UK) 3.5 to 5 mm in diameter and 9 to 13 mm in lengths were placed in 21 maxillae and 14 mandibles. Provisional acrylic bridges were fabricated in the in-house dental laboratory and fitted after a few hours from the surgical procedures using screw retention. The fabrication of the definitive prostheses was initiated between three to six months from the implant placement. A total of four implants failed in three patients during the initial healing period with provisional bridges in place, giving a cumulative survival rate of 97.3 % during a mean follow-up period of 3.5 + 1.0 years (range 2–5 years). Two failures occurred in the maxilla as a result of fracture of the provisional bridge and two in the mandible due to infection. These three patients had new implants placed and could maintain the repaired or a new provisional bridge during the additional healing period. A total of seven provisional acrylic bridges fractured. No implant failures were observed after placement of the permanent fixed bridges. Few minor other complications occurred during the follow-up. It is concluded that the evaluated treatment concept resulted in a high implant survival rate and few complications after a follow-up of 2 to 5 years. Although not quantified, the positive effects on self-esteem and psychosocial wellbeing was obvious. The Effect of Eruption Level and Angulation of Third Molars on Interdental Root Proximity: A Case-Control Study 第三磨牙出牙水平和出牙角度对牙根接近的影响:一项病例对照研究

This study was designed to assess the possible relationship between the level of eruption or impaction status and angulation of third molars (TMs) and the interdental root proximity (RP) in this quadrant, in adults with healthy periodontium and in patients with history of periodontitis. Root proximity was studied in the interdental spaces of 250 patients with periodontitis and 80 subjects with healthy and intact periodontium. Linear measurements were performed in digital radiographs. Root proximity was detected when the distance at the closest proximity level between the roots of adjacent teeth was < 0.8 mm. Eruption level was classified as full or partial, based on occlusal plane. Impaction status was assessed as semi or complete, based on coverage by soft tissues or alveolar bone. The Z-test, X2, Fisher’s exact test and Mann-Whitney test were used for the statistical analysis. In total, 718 TMs and 4,308 interdental spaces were studied in 330 subjects. In subjects with healthy periodontium, the angulation of mandibular TMs statistically significantly influenced the presence of root proximity in the interdental space between first and second molars. In periodontitis subjects, TM angulation statistically significantly affected the presence of root proximity in the interdental space between first and second molars in the maxillary left and mandibular right quadrants as well as in the interdental space between the mandibular right premolars. The eruption level or impaction status of TMs statistically significantly affected the presence of root proximity only between the first and second mandibular right molars both for subjects with healthy periodontium and for periodontitis patients. Within its limits, this study found that the angulation and eruption level or impaction status of third molars seem to be related to root proximity among the adjacent teeth, especially between lower first and second molars, irrespective of the presence or absence of periodontitis.

Current Status of Local Drug Delivery Systems in the Treatment of Periodontal Diseases 局部给药系统在牙周病治疗中的现状

Periodontal disease includes many pathological conditions affecting the periodontium, but gingivitis and periodontitis are the more common types. Gingivitis is a progressive condition, although it is reversible, when left untreated it can lead to periodontitis. Microbial species have a major role in the aetiology of periodontitis. Method: Information was derived from research papers using PubMed, Science Direct, and Google Scholar using the keywords local drug delivery system, treatment, and periodontal disease. The search included articles up to 2018 with majorly in vivo studies in patients with periodontitis. The usage of local drug delivery systems with controlled or sustained release mechanisms may provide slightly better therapeutic effect in comparison with patients who undergo scaling and root planning only. Conclusion: The present review of the literature of the currently available local drug delivery systems in the treatment of periodontal diseases.
