Based on a clinical situation in child psychiatry, it is possible to explore the counter-transferential reactions elicited by family, historical, and cultural contexts. Recognizing these reactions allows for a better understanding of parent-child dynamics and promotes appropriate institutional work.

A clinical case illustrates how cultural countertransference sheds light on the difficulties of forming alliances between families, institutions, and adolescents in vulnerable situations. The development of these internal movements, supported by mediation, makes it possible to transform the educational posture into a space for decentering, affiliation, and co-construction.

In psychiatry, dual pathology is increasingly common and requires an integrated approach. Tobacco use is ubiquitous and cannabis exacerbates all psychopathologies. Neurobiological mechanisms explain why these substances continue to be used. In 2025, the 19th MAEVAS Day, an annual event demonstrating the strong commitment of the Paris Psychiatry and Neuroscience University Hospital Group and its healthcare management to promoting healthcare practices, was held on the theme of "Providing care with empathy: the art of understanding and soothing." Empathy, beyond sympathy or simple compassion, is a demanding but essential attitude in healthcare.
