Tratamiento sintomático en migraña. Fármacos utilizados y variables relacionadas. Resultados de la encuesta europea Trabajo y Migraña 偏头痛的对症治疗。使用的药物及相关变量。欧洲工作与偏头痛调查结果

Goals: To know the use of migraine crisis symptomatic treatments in different countries of Europe and the differences observed depending on social and demographic variables. Material and methods: Cross-sectional observational study by anonymous web survey to 3,342 patients from Spain, Italy, France, Portugal, Ireland, United Kingdom, Germany and other countries of the European Union. Study variables: age, gender, country, type of location, level of studies and rural or urban area. The symptomatic treatments that are collected are: simple analgesics, non-steroidal anti-inflammatory drugs, triptans, other treatments, several treatments, without treatment, ignorance of symptomatic treatment. Results: For simple analgesics, the largest consumers are between 41-60 years (p < 0.0001). Spain and Germany are the countries with the highest use (p < 0.0001). For the anti-inflammatories, the greatest use is between 21-60 years (p < 0.0001). Spain, Italy and Germany are the countries that make the most use of them (p < 0.0001) and higher consumption in patients with higher education (p < 0.003). Triptans use shows a relationship with age and gender, greater use between 21-60 years (p < 0.0001) and in women (p < 0.0001). By countries, the highest consumption is in Germany, the rest of the European Union countries, Spain and the United Kingdom (p < 0.0001). Greater consumption in patients living in cities with more than 500 inhabitants (p < 0.010) and with medium or higher studies (p < 0.0001). In the group of middle and upper studies, we find, by contrast, the highest percentage of patients who do not know what a symptomatic treatment is (p < 0.0001). Conclusions: The result varies according each country greatly and they are modified depending on different social and demographic conditions, being the age, gender, rural or urban environment and cultural level the factors that most affect the use of each symptomatic medication for the migraine crisis.

Efecto analgésico del parche de capsaicina 8 % a través del receptor vaniloide TRPV-1, en un paciente con enfermedad de Parkinson y dolor neuropático localizado 8%辣椒素贴片通过香草醛TRPV-1受体对帕金森病和局部神经性疼痛患者的镇痛作用

Nowadays differentiation between central and peripheral origin in Parkinson's disease pain is considered as a clinical challenge. We present a long evolution Parkinson's disease case of a 71 years old woman. Treatment undergone by the patient consists in deep brain stimulation (DBS), drugs dopamine agonist, monoamine oxidase inhibitor-B (MAO-B) and catechol-O-methyltransferase inhibitor (COMT).The patient was referred to the pain unit due to an outbreak of continuous pain in the upper outer region of the left knee and no response to pharmacological nocioceptive pain treatment. The patient was diagnosed with neuropathic pain after reviewing her clinical history, the physical examination and application of the DN4 scale. The chosen treatment was the application of a 179░mg capsaicin patch over the region where she was experiencing the pain and it resulted in an improvement from week one which still persisting twelve weeks later. Treatment with capsaicin patch was selected with the aim of avoid possible drugs interactions with her treatment. As a conclusion, the high topical concentration of capsaicin has been found to be effective and safe in a patient with complicated treatment (DBS plus usual treatment).

Rib fractures have a high prevalence in our society. An adequate and precocious analgesic control is essential to avoid respiratory complications and favor a rapid functional recovery in patients with rib fractures. Different strategies have been used for analgesic control, from intravenous drugs to more classic regional techniques such as epidural catheter or thoracic paravertebral block. Ultrasound-guided blockade of the deep plane of the erector spine muscle (ESPB) is an effective alternative in the management of acute pain derived from rib fractures, allowing physiotherapy and early rehabilitation.

Escuela de pacientes con dolor crónico no oncológico y tratamiento opioide 非肿瘤性慢性疼痛和阿片类药物治疗患者学院

Pain is one of the disorders that most affects and worries people and is the accompanying symptom that most often motivates medical consultation. The frequency of chronic pain in the spanish adult population is very high and constitutes a general publich health problem. The school of patients has demonstrated its effectiveness in the therapeutic approach of certain processes (arterial hypertension, mellitus diabetes and others). We present the first school of patients with nononologic chronic pain and opioid treatment implemented in the Public Health System of Andalusia, which has allowed training patients with opioids treatment for their pain processes with a very high satisfaction degree.
