Introduction: The number of patients receiving chronic opioids undergoing spinal surgery has increased lately. Elevate records in visual analog scale (VAS) and opioid consumption are higher in this group of patients. Material and methods: Following the recommendations of the American Pain Society (APS), a new intra and postoperative analgesic protocol based on the use of ketamine was designed in 2018. Intraoperatively, a bolus of 0.5 mg/kg of ketamine was administered, followed by a dose infusion of 0.2 mg/kg/h until the surgical wound was closed. During the first 48 postoperative hours, a patient-controlled analgesia pump (PCA) of morphine-ketamine was maintained along with a multimodal analgesia regimen with paracetamol and dexketoprofen. An effectiveness analysis comparing the new protocol (ketamine group) with the previous one (control group), based on the use of tramadol 100 mg / 6h or PCA morphine, was done. During the first 48 postoperative hours, NVS records, need of rescue analgesia, morphine bolus or continuous morphine infusion (control group) or ketamine morphine infusion (ketamine group) were analyzed. Results: The patients in the ketamine group had lower NVS records than those in the control group during the first two postoperative hours (p = 0.001) and lower morphine rescues needs on the second postoperative day (p = 0.003). The need for continuous morphine-ketamine perfusion was significantly lower than the need for continuous morphine perfusion on the control group (p = 0.011). Conclusion: The protocol based on the use of ketamine, managed to improve the control of postoperative pain and significantly reduce the consumption of opioids in the first 48 hours after the intervention.

Introduction: The Sandifer's Syndrome was described in 1964 by the neurologist Paul Sandifer when detected a disorder of the upper gastrointestinal tract that generated gastroesophageal reflux disease, especially in new-borns and children. It tends to present with esophagitis, iron deficiency anemia and paroxysmal postural movements. These symptoms are related with the ingestion and they use to remit during the sleep. This syndrome generates to the child a psychomotor delay and, therefore, the Early Care is essential to detect and to identify all the signs early; always focusing on the process of learning and consolidation of psychomotor achievements. Thus, the treatment consists of stimulating the child trough the active game to get the psychomotor progression, working on perceptual and sensory changes, and avoiding pain during the intervention. Case report: 8-month-old lactating child with psycmotor delay referred to Early Care from Hospital. The baby presents neck hyperextension, cervicoaxial hypotonia, rejection of the prone position, opisthotonus in supine position and sporadic regurgitations. The child can not move neither maintains the sedestation. Furthermore, he does not hit toys; he has good contact but does not point out objects and does not emit disyllables. Comments: After fourteen months of Early Care (Physiotherapy), the toddler can up and down the stairs grabbed to the railing with one hand and he gets up from the ground practically alone. He can make more complex lace with dominance of the right hand. The toddler understands concepts of permanence and of action-reaction; and he repeats many words and emits more than ten words spontaneously. The family collaboration by applying the recommended patterns has been a key piece in the treatment. The guidelines applied in various settings facilitated the achievement of motor skills and the consolidation of stages.

Hemos evaluado el efecto de la lidocaina intravenosa en perfusion sobre las diferentes caracteristicas de la fibromialgia. Metodos: Despues de la aprobacion del comite de etica de nuestro hospital y el consentimiento informado de cada paciente, se incluyeron 62 pacientes con criterio de fibromialgia segun la American College of Rheumatology. Se realizaron perfusion de lidocaina a dosis creciente desde 2 mg/kg hasta 5 mg/kg durante 10 dias. En todos los casos se rellenaron los siguientes cuestionarios en el dia 0 (pretratamiento), dia 10 (postratamiento) dia 30 y dia 90: Health Survey SF-12 (SF-12), Fibromyalgia Impact Questionnaire (FIQ), Brief Pain Inventory (BPI), Big Five Inventory (BFI), Beck Depression Inventory II (BDI-II), Medical Outcomes Sleep Scale (MOS), Patient Improvement Expectations (EXPEC). Resultados: De los 62 pacientes, 9 abandonaron el estudio por efectos adversos intolerables y 5 se perdieron o no completaron los cuestionarios en los siguientes meses. Finalmente completaron el estudio 48 pacientes, 46 mujeres y 2 hombres con una edad media de 55 (36-70) anos. Encontramos una mejoria en los cuestionarios BPI, BFI y BDI-II en el dia 10 que no se mantienen a los 30 dias. Discusion: En nuestro estudio el tratamiento con perfusiones de lidocaina en pacientes con fibromialgia objetivo una buena eficacia inicial que no se mantuvo con el paso del tiempo. EnglishABSTRACT Introduction: Fibromyalgia is defined as a pain syndrome characterized by generalized chronic pain, fatigue, sleep disturbances, anxiety and depression and, at times, an insufficient and frustrating treatment response. It is a benign condition chronically suffered by many patients, which forces the medical community on an ongoing search for an optimal treatment. We evaluated the effect of intravenous lidocaine on the different characteristics of fibromyalgia. Method: This is a prospective, longitudinal study. We recorded data for 48 patients diagnosed with fibromyalgia syndrome according with the American College of Rheumatology criteria (ACR 1990). Patients with abnormal electrocardiogram or abnormal blood electrolytes were excluded. Included cases received an increasing dose of 2 mg/kg up until 5 mg/kg of an intravenous lidocaine perfusion during 10 days. We evaluated the effect of intravenous lidocaine on the different characteristics of fibromyalgia. Method: This is a prospective, longitudinal study. We recorded data for 48 patients diagnosed with fibromyalgia syndrome according with the American College of Rheumatology criteria (ACR 1990). Patients with abnormal electrocardiogram or abnormal blood electrolytes were excluded. Included cases received an increasing dose of 2 mg/kg up until 5 mg/kg of an intravenous lidocaine perfusion during 10 days. The 20th century has been an exceptional witness to the great advances and findings in the neurophysiological understanding and the multidisciplinary treatment of chronic pain. However, interest in the different form of expression in the painful perception between men and women and the specific psychological and social determinants associated with gender roles are relatively recent. In this article, an unsystematic review of the psychological and social differential determinants is carried out following the Neuromatrix model. Finally, the knowledge of those biases that we professionals have involuntarily is crucial to address the avoidable inequalities in treatment and the recovery of the health of men and women. Finally, recommendations are offered from the field of Clinical Psychology.
