Background: Urinary incontinence is a common problem among older women. The objective of this study was to determine the prevalence of urinary incontinence among older women aged 55 years and over in the Klang valley, the most densely populated urban area in Malaysia, and to determine any potential ethnic differences.Methods: This study used cross-sectional data from the Malaysian Elders Longitudinal Research (MELoR) study. A total of 863 participants were included, with a mean age of 67.97 ± 7.50 (S.D.). They were selected by simple random sampling from the electoral rolls of three parliamentary constituencies.Results: The prevalence of urinary incontinence was 30.8%. Mixed urinary incontinence (UI) was present in 44.7%, stress UI in 39.1% and urge UI in 16.2%. Ethnic Malays (38%) were more likely than ethnic Indians (32.3%) and Chinese (29.7%) to have UI. Ethnic Malays were also more likely to have stress UI and Indians were more likely to have urge UI .Conclusion: The prevalence of UI in this study was comparable to other Asian and worldwide studies. The significant association between ethnicity and UI, however, has not been reported in any previous studies. Further studies should identify factors which may determine these ethnic differences in UI.

Aortic dissection (AD) is a rare cardiovascular emergency with a high rate of mortality. Most patients present with typical signs and symptoms of AD. However, atypical presentation increases risk of delay in diagnosis and treatment. Patients may present with syncope, seizures or other neurological symptoms which may deceive the diagnosis of AD. Dissection can be in the acute or chronic phase, but there has been no documentation of an acute dissection which occurs in a chronic dissection. We hereby to report a case of an acute on chronic thoraco-abdominal dissection.

Background: Oral hygiene is importance in maintaining a good oral health as it may affect the general healthof the body. Adolescence features a stage of significant caries activity for countless individuals. Thus, thepurpose of this study was to assess the knowledge, attitude and practice among adolescence suburban schoolchildren towards oral hygiene.Methods: A cross-sectional study in which self-administered questionnaire was distributed to 300 studentsat selected suburban public schools in Selangor based on stratified sampling. The questionnaire comprised ofsocio-demographic characteristics, knowledge about oral hygiene, attitude towards oral hygiene and practicefor prevention of oral disease. Students who had previously resided in urban areas for a minimum period of1 year in the past 5 years were excluded. Data were analyzed by using descriptive analysis and Chi-squaredtest in SPSS version 21.Results: A total of 252 answered questionnaires were returned. The results of the study indicated low level ofawareness for oral hygiene; 75.4 % showed lack of knowledge on dental plaque, 81.3% did not use floss, only46.0 % brush their teeth twice per day and 27.8% visited a dentist last 6 months ago. A good attitude towardsoral hygiene is still lacking, nevertheless a majority of the respondents have slight negative view toward dentalvisit which is mostly influenced by their anxiety.Conclusion: This study revealed deficient of oral hygiene awareness among adolescence suburban schoolchildren in certain aspects of dental hygiene. It therefore highlights that urbanization does not always correlatewith high level of oral hygiene awareness.

Purpose: Most of postpartum women have to take at least one medication; so safety of medication needs to be considered when prescribing and dispensing the medication to them. In 2017, 11% of 815 questions received by Pharmacy Drug Information Services at seven clinics in Dungun were related to medication use in breastfeeding. Thus, this study was carried out to evaluate attitude of healthcare providers (HCPs) and to investigate the effect of knowledge about medication use in breastfeeding among HCPs; pre and post educational intervention. Method: An interventional study was conducted among medical officers, assistant medical officers, pharmacists and pharmacist assistants from seven clinics in district of Dungun, Malaysia. The questionnaires were distributed during pre and post-intervention period. The interventions in this study were continuous medical education (CME) and booklet regarding medication use for breastfeeding women. Data were entered and analyzed using Statistical Package for Social Studies, (SPSS) and presented as frequencies, means, and standard deviations. Result: A total of 50 HCPs were enrolled in this study and over 20% of them advised mother to discontinue breastfeeding whenever she had to take any medication. The knowledge of HCPs about medication use in breastfeeding women was significantly improved (Z= -5.917, p<0.001) after the educational intervention. Conclusion: CME and simplified booklet tend to have positive impact on the knowledge of HCPs regarding medication use in breastfeeding.

Background: Those with increased dynamic knee valgus are vulnerable to increased risk of non-contact kneeinjuries. However, studies on the top down kinetic chain of lower limb mechanics during dynamic motionssuch as single leg squat (SLS) among trained males were scarce.Objective: The objective of the study was to evaluate the relationship between isokinetic hip and knee strengthand frontal plane projection angle (FPPA) of the knee joint during SLS.Methods: Thirty-two male junior athletes (twelve cyclists, ten runners and ten squash players) were screenedfor excessive dynamic knee valgus (DKV) prior to participation. Only those within the normal value of DKV wereincluded. Their hip and knee isokinetic strength in sagittal plane were evaluated at 60o/s of angular velocityfor both legs using dynamometer. Two dimensional knee FPPA was evaluated during SLS at 60o of knee flexion.Pearson correlation was evaluated between knee FPPA during SLS and isokinetic leg strength.Results: Correlations between knee FPPA and hip and knee isokinetic strength were not statistically significantexcept between knee flexion peak torque/body weight (r = -0.35, p = 0.05) and hamstring to quadriceps ratio(r = -0.39, p = 0.03) of non-dominant leg.Conclusions: Isokinetic hip and knee strength and knee FPPA during SLS was correlated only for non-dominantleg during SLS among male junior athletes. DKV during SLS may be reduced through strengthening the musclesaround hip and knee joints.

Background: Despite regularly participating in international and national level multisport events, there is still limited data on the pattern of injuries and illnesses and factors associated with injuries and illnesses in Malaysia. Such information is crucial to instil preventive measures because sustaining injuries during competition could hamper the athlete’s performance. Methods: This cross-sectional study investigated the incidence, patterns and risk factors of injuries and illnesses among athletes throughout SUKIPT 2018, from the 2nd to the 10th of February 2018. All injuries and illnesses treated by tournament medical personnel were reported using a standardised online injury reporting form. This form was adapted from the injury surveillance form used by the International Olympic Committee. Results: A total of 6071 athletes from 80 contingents took part in SUKIPT 2018. During the nine days of competition 323 injuries and 48 illnesses were reported, resulting in an incidence of 5.3 injuries and 0.8 illness per 100 athletes. Approximately 6% of the athletes sustained at least one injury or illness. Conclusion: In summary, the incidence of injuries and illnesses among athletes during SUKIPT was 5.3 and 0.8 per 100 athletes, respectively. Muscle strain/rupture/tear was the most common pattern of injury while collision with another athlete was the most frequent mechanism of injury. Meanwhile, the respiratory system was the most commonly affected by illness and infection was the most prevalent cause of illness.

Such information is crucial to instil preventive measures because sustaining injuries during competition could hamper the athlete’s performance. Methods: This cross-sectional study investigated the incidence, patterns and risk factors of injuries and illnesses among athletes throughout SUKIPT 2018, from the 2nd to the 10th of February 2018. All injuries and illnesses treated by tournament medical personnel were reported using a standardised online injury reporting form. This form was adapted from the injury surveillance form used by the International Olympic Committee. Results: A total of 6071 athletes from 80 contingents took part in SUKIPT 2018. During the nine days of competition 323 injuries and 48 illnesses were reported, resulting in an incidence of 5.3 injuries and 0.8 illness per 100 athletes. Approximately 6% of the athletes sustained at least one injury or illness. Conclusion: In summary, the incidence of injuries and illnesses among athletes during SUKIPT was 5.3 and 0.8 per 100 athletes, respectively. Muscle strain/rupture/tear was the most common pattern of injury while collision with another athlete was the most frequent mechanism of injury. Meanwhile, the respiratory system was the most commonly affected by illness and infection was the most prevalent cause of illness.","PeriodicalId":39135,"journal":{"name":"Journal of the University of Malaya Medical Centre","volume":" ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-11-05","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"45998956","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Cryptococcus neoformans is an encapsulated fungal pathogen that caused severe disease primarily in immunocompromised patients. Adherence and internalization of microbial pathogens into host cells are often begin with engagement of microbes to the surface receptors of host. However, the mechanisms involved remain poorly understood. In this study, we investigated association of cell surface determinants of C. neoformans with mammalian cells. Our results showed that treatment with trypsin, but not paraformaldehyde or heat killing, has reduced host-cryptococci interaction, suggesting the involvement of cell surface proteins (CSPs) of C. neoformans in this interaction. We extended our investigations to determine roles of CSPs during cryptococci-host cells interaction by extracting and conjugating CSPs of C. neoformans to latex beads. Conjugation of CSPs with both encapsulated and acapsular C. neoformans increased the association of latex beads with mammalian alveolar epithelial cells, alveolar macrophages and monocyte-derived macrophages. Further examination on the actin organisation of the host cells implied the involvement of actin-dependent phagocytosis in the internalisation of C. neoformans in CSP-conjugated latex beads. We hypothesized that CSPs present on the cell wall of C. neoformans mediate the adherence and actin-dependent phagocytosis of cryptococci by mammalian cells. Our results warrant further studies on the exact role of CSPs in pathogenesis during cryptococcosis.
