Orthopedic custom-made shoes and orthopedic innershoes are indicated when the patient can no longer be helped satisfactorily with simpler orthotic devices, i.e. with foot orthoses and adjustments to the ready-made shoe.

The aim is to restore mobility in cases of moderate to severe damage to the bones, ankles, muscles, ligaments or tendons of the foot. The task of an orthopedic shoe fitting is to enable and maintain an appropriate walking function and reduce the symptoms of discomfort when walking and standing; ultimately, to enable participation in public life.

In the orthopedic shoe, all functional elements of foot orthoses, the adjustments to the ready-made shoe and the functional replacement of the foot lower leg can be integrated in one aid. Today, the orthopedic shoe can also be manufactured in an appealing design and appearance.

The indication for an orthopedic shoe can be given if a need-based supply, for example, with insoles, adjustments to the ready-made shoe or orthoses is not sufficient or possible.

Although insoles or orthopedic shoes are regularly prescribed by physicians in the clinic and in the practice, the legal framework is often not known to everyone. Essentially, the regulations for the care of patients in the state health insurance system are formulated in the German Social Code V (SGB V). The roles and responsibilities of physicians as prescribers, orthopedic shoe technicians as service providers, and insurance companies as reimbursors are clearly defined. The purpose of this paper is to provide a brief overview of the legal requirements for the provision of orthotics and footwear.

The shoe has been a element of constant clothing for mankind for thousands of years. As long as shoes exist, mankind has tried to improve the comfort and function by modification on and in the shoes. The development of shoe design and manufacture is constantly evolving along with the materials available as well as with fashion tastes. This includes the development of insole care. However, the basic concepts and possible functions of shoe fittings and insoles are not generally known. The purpose of this manuscript is to provide a brief overview of insole and shoe care.

The provision of insoles in elite sports presents special challenges for orthopedic shoemakers. Although the supply process is essentially no different from that for everyday patients, it is important to consider important, sport-specific aspects with regard to the diagnosis and analysis. Knowledge of functional anatomy, pathology, and biomechanics is essential to meet the individual needs of athletes and avoid downtime in the tightly scheduled competition calendar due to incorrect treatments. The aim of this article is therefore to outline selected features of sports analysis based on an overview of the general standard of care and to highlight their implications for insole provision, using case examples.
