Quality of life of postmenopausal women with teriparatide, denosumab and alendronate: One-year prospective study with a propensity score-matched comparison 绝经后妇女使用特立帕肽、地诺单抗和阿仑膦酸钠的生活质量:为期一年的前瞻性研究,倾向评分匹配比较

Background/purpose: To evaluate and compare the effects of parathyroid hormone analogues, receptor activators of nuclear factor kappa-B ligand inhibitors and bisphosphonates on the quality of life of postmenopausal women. Methods: A prospective observational study of 23 matched postmenopausal women was conducted with propensity score analysis on quality of life at one-year follow-up. Visual analogue scale for back pain and outcome scores were carried out as the quality of life or treatment adherence measurements. Results: Teriparatide use was associated with significant improvements in visual analogue scale, EuroQol 5-level 5-dimension general health status and Osteoporosis Assessment Questionnaire physical function, whereas denosumab and alendronate groups only demonstrated improved Osteoporosis Assessment Questionnaire scores but worsened back pain. Baseline average visual analogue scale back pain predicted one-year average back pain progression (partial eta squared = 0.617, p = 0.001). Conclusions: One-year continuous teriparatide treatment is most effective in improving quality of life outcomes in postmenopausal osteoporotic women. Baseline average visual analogue scale back pain remained the only predictive factor for one-year back pain progression.

Review Article On Recent Trends in Surgical Techniques for High-grade Spondylolisthesis 高度椎体滑脱手术技术的最新趋势综述

Introduction: High-grade spondylolisthesis is defined as cases with more than 50% displacement and spondylolisthesis with Meyerding grade III and higher. The surgical management of high-grade spondylolisthesis is highly controversial. Many surgical methods have been reported like posterior in situ fusion, instrumented posterior fusion with or without reduction, combined anterior and posterior procedures, spondylectomy with reduction of L4 to the sacrum (for spondyloptosis), posterior interbody fusion with trans-sacral fixation. Minimally invasive transforaminal lumbar interbody fusion for high-grade spondylolisthesis has also been recently mentioned in literature. This study aimed to review the recent literature which has described the surgical outcomes associated with various surgical techniques used for high-grade spondylolisthesisMaterials and Methods: Recent articles were searched on search engines such as PubMed, Google Scholar with the use of Keywords like ‘High-grade Spondylolisthesis’, ‘Surgical Techniques’ ‘complications’.Discussion: The surgical management of high-grade spondylolisthesis is an area of significant controversies. The literature is replete with regards to need for reduction, need for decompression, levels of fusion, the nature of instrumentation, surgical approaches including open, minimally invasive, and “mini-open” procedures, as well as various techniques for reduction of the slip and fusion strategy. The three basic options of high-grade spondylolisthesis include in-situ fusion, partial reduction and fusion, and complete reduction.Conclusion: Various techniques have been described for high-grade spondylolisthesis. Spine Deformity Study Group (SDSG) classification gives guidelines about balanced and unbalanced pelvis and advising reduction and fusion in case of unbalanced pelvis for correction of biomechanical and global sagittal alignment. Functional Outcomes of Distal Humerus Fractures Fixed by Orthogonal Double Plating 正交双钢板固定肱骨远端骨折的功能效果

Background: Distal humerus fractures constitute 2% of all fractures in the adult population. Surgical intervention is indicated in most cases, and these fractures are often complicated by difficulty in fracture-site exposure, comminution in the metaphyseal or articular region, and the presence of osteoporotic bone. The objective of the study was to assess functional outcome of operative fixation of distal humerus with locking plate and screwsMaterials And Methods: This is a retrospective study of 30 patients with distal humerus fractures fixed with orthogonal plating from 2018-2019 in KIMS Hospitals, Kondapur, Hyderabad. Functional outcome was assessed using Mayo Elbow Performance score along with other major and minor complications were noted.Results: There were 18 male patients and 12 female patients. Average age of the patients was 39.68 years with age ranging from 18 years to 80 years.16 cases were right humerus and 14 were left humerus involved. All cases we operated through trans olecrenon approach and chevron osteotomy technique, fracture stabilised by orthogonal locking compression plate. 6 fractures were C1, 15 were C2 and 9 were C3 types according to AO classification. Complications were noted in 5 patients. 13.3% patients were rated as excellent, 63.3% were rated good, 16.6% were rated as fair, 6.8% were rated as poor according to Mayo Elbow Performance Scoring.Conclusion: Orthogonal dual plate configurations can provide anatomical reconstruction and stable fixation of Type C intra articular distal humeral fractures and allow early mobilization of the elbow after surgery which result in good functional outcomeKeywords: Distal humerus fractures, Trans Olecrenon, Chevron Osteotomy, MEP score.

An Overview of Avascular Necrosis of Femoral Head Management 股骨头缺血性坏死的治疗综述

Purpose: The aim of this article is to provide an updated review on most commonly used modalities to treat avscular necrosis of femoral head. A thorough literaturesearch was performed using predetermined criteria.Recent Findings: Systematic evaluation for decisions about management demonstrated the following:(1) Though efficacy is vague and controversial non-operative treatments, pharmacological treatments or core decompression can be used in symptomatic medium-sized pre-collapse lesions.(2) In spite of known complications to no vascularized bone graft,vascularized bone grafts and osteotomies -medium to larger-sized lesions can be dealt with them(3) In post-collapse stages, they should be treated with joint preserving techniques, such as hip surfacing, nonvascularized bone graft,vascularized bone grafts(4) If post-collapse and acetabular involvement ensues, arthroplasty is indicated.(5) Cell-based and biological forms of treatment in form of bone marrow aspirate, stem cells, BMP have shown inconclusive results, and further research is needed to assess their role in the management of this condition.Methods: Recent articles were searched on search engines like PubMed, Google Scholar and references of different articles were checked.Summary: There is a need to overview the management options for avascular necrosis of the femoral head. This review discusses different management modalities available along with their advantages and disadvantages.Keywords: AVN femoral head, Non-operative treatment, Biophysical management, Operative treatment
