Selection of antihypertensive therapy for elderly patients 老年患者降压治疗的选择

У продолжительности жизни и, как следствие, увеличение числа пациентов старше 60 лет приводит к тому, что врач первичного звена здравоохранения должен быть готов к оказанию надлежащей помощи этой многочисленной группе пациентов. Так, в США примерно 30–40 % обращений к врачам общей практики приходится на возрастную группу старше 65 лет, а основной причиной является АГ [1]. Хорошо известно, что распространенность АГ с возрастом неуклонно растет и 50 % больных АГ находятся в возрастной группе старше 60 лет [2]. В 2009 году был опубликован Кокрейновский обзор 15 исследований, включивших примерно 24 тысячи пациентов старше 60 лет. В данную работу вошли исследования, в которых сравнивалось назначение различных антигипертензивных препаратов и плацебо у пациентов как старше 65, так и старше 80 лет. Для пациентов возрастной группы 65–80 лет, которым назначалась антигипертензивная терапия, получили данные о снижении как общей сердечно-сосудистой заболеваемости и смертности (RR 0,72 [0,68; 0,77]), так и общей смертности (RR 0,90 [0,84; 0,97]) [3].
