Резюме Актуальность. Среди факторов, повышающих риск сердечно‐сосудистых осложнений (ССО) у больных с остеопоро‐ зом, рассматривается повышение артериальной жесткости. Цель. Изучить возможные взаимосвязи между артериальной жесткостью и показателями нарушенного минерального обмена костной ткани у больных высокого сердечно‐сосуди‐ стого риска. Материалы и методы. У 137 пациентов с высоким риском развития ССО по шкале SCORE и отсутствием ИБС измерены скорость пульсовой волны на каротидно‐феморальном (СПВкф, аппланационная тонометрия) и плече‐ лодыжечном (СПВпл, объемная сфигмография) сегментах, индекс аугментации (ИА), минеральная плотность костной ткани в поясничном отделе позвоночника (МПКТпозв) и шейке бедренной кости (МПКТшб) (DEXA). В зависимо‐ сти от степени нарушения МПКТ пациенты были разделены на 3 группы: с нормальной МПКТ (n=29), остеопоро‐ зом (n=45) и остеопенией (n=63). Результаты. У больных из групп с остеопорозом и остеопенией наблюдались ста‐ тистически достоверно большие значения СПВпл – 14,1 (12,8; 16,4) м / сек и 13,5 (12,5; 15,7) м / сек соответственно по сравнению с пациентами без нарушения МПКТ – 12,9 (11,8; 13,9) м / сек. Значения СПВпл зависели от возраста, АД, ИА, скорости клубочковой фильтрации и уровня маркера костного обмена – щелочной фосфатазы (ЩФ) (для всех p<0,05). Наблюдалась обратная корреляция концентрации ЩФ и значений МПКТпозв (p<0,05). Выявлена статистиче‐ ски значимая обратная корреляция ИА со значением МПКТшб. Заключение. У больных высокого сердечно‐сосудистого риска с нарушением МПКТ обнаружены более высокие значения артериальной жесткости по сравнению с пациентами с нормальной МПКТ. Выявленные корреляции между показателями артериальной жесткости и минерального обмена костной ткани могут свидетельствовать о наличии патогенетических связей между поражением артериальной стенки и костной ткани у данной категории больных.

Background . Clinical significance of (AF) is related primarily with its thromboembolic complications. The probability of thromboembolic complications increases during cardioversion. Aim . To compare the incidence of detecting contraindications for cardioversion (left atrial appendage thrombosis or high-degree spontaneous echocontrast) using transesophageal EchoCG in patients with persistent, non-valvular AF receiving pre-cardioversion therapy with warfarin and new oral anticoagulants (NOACs) (dabigatran and rivaroxaban). Materials and methods. Transesophageal EcoCG was performed for 208 patients with persistent, non-valvular AF; during preparation for cardioversion, 140 of them received warfarin and 68 received NOACs). Results. The anticoagulant preparation was con-sidered inadequate for patients receiving warfarin and only for 38.6% of patients receiving NOACs. When the anticoagulant preparation was adequate, contraindications for cardioversion were identified in 16.3% of patients receiving warfarin and 9.5% of patients receiving NOACs. Among patients receiving inadequate anticoagulant therapy, contraindications for cardioversion were identified in 37.3 % of cases. Conclusion. Using NOACs 7.91 times decreased the probability of inadequate anticoagulant therapy. With the adequate NOAC therapy, the probability of remaining contraindications for cardioversion was 2.9 times lower than with the adequate warfarin therapy.
