Erectile dysfunction is more common than realised and can be the first indication of systemic disease. Penile prosthesis is a low‐risk intervention with high patient satisfaction, previously deemed an endstage form of management. However, improvements in the devices and a recent American Association of Urology recommendation could change that.

Immune checkpoint inhibitors (ICIs) have had a significant impact on the prognosis of people with a wide range of tumours. Melanoma in particular is one of the malignancies for which immunotherapy can achieve excellent outcomes in the adjuvant and advanced settings. Here the authors discuss the use of ICIs and the benefits they can offer.

Pancreatic cancer carries a very poor prognosis, with five‐year survival currently approaching 10%. Late presentation, lack of effective treatment options and a tumour microenvironment that subverts immune mechanisms are some of the reasons behind this. Here the authors discuss how advances in our understanding of tumour biology, identification and surveillance of high‐risk groups, improvements in surgical outcomes and the availability of new chemotherapeutic options give us grounds for optimism.

Fetal ultrasound can identify babies who are at risk of developing renal failure. However, renal damage before birth is difficult to reverse and end‐stage renal failure may not be preventable.

Recent events have left the NHS starved of funds and the immediate future does not look too bright financially either. The organisation is also suffering from low staff morale and burnout of its workforce along with ever‐increasing demand. Indeed all the pressures now facing the NHS point towards a need for far‐reaching reform.

Obsessive‐compulsive disorder (OCD) commonly affects males and can manifest with a range of urological symptoms directly linked to the content of the OCD. In Part 1 of this article, the authors discussed the recognition and diagnosis of OCD. In Part 2 here, they consider its treatment and related sexual dysfunction.

Premature ejaculation is a common male condition worldwide with prevalence estimates varying from 30% to 85%. Owing to the lack of a clear definition and a clear understanding of the potentially multifactorial pathophysiology, treatment remains challenging. The aim of this systematic review was to evaluate the non‐pharmacological treatment modalities for PE to inform clinical practice and patients.

As medical technology develops, cancer can be detected sooner and potentially be treated more effectively. However, the benefits of early detection are not clear. Here the authors discuss whether early detection of kidney cancer is leading to overtreatment of some patients and how newer diagnostic techniques may help improve patient‐centred management of the disease.
