MEDICAL GUIDELINES FOR AIRLINE TRAVEL: MANAGEMENT OF IN-FLIGHT CIRCULATORY ARREST 航空旅行医疗指南:飞行中循环停止的管理

In recent times, air transport has become more common, and so the number of passengers using it has increased. As travellers increased, so did the risk of any adverse event related to the health or life of those on board. The staff of the aircraft should be properly trained to be able to help the victim and at the same time be able to stay calm on board the plane. It is also not uncommon for a person with medical education to be present on board the plane – it is importantthat healthcare professionals know their rights and obligations arising from the situation. The paper presents the procedures applicable to the cabin crew, health care workers present on board and passengers who are at increasedrisk of a life-threatening episode during the flight.

PNEUMONIA IN THE COVID-19 ERA – EMERGENCY ROOM PHYSICIAN’S PERSPECTIVE. PART I – ETIOLOGY AND EPIDEMIOLOGY COVID-19时代的肺炎——急诊室医生的视角。第一部分:病因学和流行病学

Lower respiratory tract infections are one of the most common causes of death. From many years Poland was ranked fourth among the EU countries with the highest number of deaths from pneumonia. No observational studies among Polish patients with pneumonia have been conducted so far that would enable to determine the true scale of the problem, and no risk and etiological factors that are specific to the population of the country have been defined. Despite continuous advances in medicine, managing pneumonia remains a challenge for the clinician, especially in the Emergency Department. The greatest challenge is the correct diagnosis of pneumonia, qualification for admission to the hospital ward, as well as outpatient treatment and immediate treatment. Problems in patient care may include: non-specific symptoms, especially in elderly patients who are the most common ED patients, a limited number of tests that can be performed on ED in a short period of time, no clear recommendations regarding the management of the patient, a wide spectrum of pathogens involved in the pathogenesis of pneumonia (and new pathogens, e.g. SARSCoV- 2), bacterial resistance to antibiotics and finally emphasis on profitability of therapy. The paper describes the epidemiological data and possible etiological factors of community acquired pneumonia. In second part, we will focus on the diagnostic possibilities (including SARS-CoV-2 infections) and the issues related to the treatment.

ASSESSMENT OF INTERVENTIONS COMPLETED BY THE VOIVODESHIP RESCUE SERVICE IN KATOWICE IN THE SILESIAN VOIVODESHIP IN 2018 DUE TO INJURIES 评估2018年西里西亚省卡托维兹省省救援服务因受伤而完成的干预措施

Aim: To assess the trips made by Emergency Medical Teams due to injuries (ICD-10 code: S) completed by Voivodeship Medical Rescue Teams in Katowice in 2018.Material and methods: Analysis of the characteristics of the trips was done based on information contained on “Emergency ambulance travel cards”. Statistical analysis was carried out using the t-Student, ANOVA variance as well as the Tukey post-hoc tests (p<0.05).Results: The whole number of trips made by the Emergency Medical Teams in the Silesian Voivodeship totalled 211,896, of which due to the reported injury, the total number of trips was 202,946 cases (183,827 calls were in town – 91%; 19,119 calls were out of town – 9%; p<0.05). The most common cause for calls were due to head injuries (S03 – dislocation, sprain and strain of joints and ligaments of the head as well as S04 – cranial nerve injuries). Statistically significant differences in the number of trips depending on cause identification (p<0.05) were determined, as well as in comparison to the number of cases identified as S03 and S04 between the in town and out of town territory (p<0.05). The median arrival time to an in town site was 7 minutes and 42 second, whereas in out of town sites it was 13 minutes and 19 seconds.Conclusion: The characteristics of the trips made by Emergency Medical Teams depends on the time, and what is affected by organizational changes in the emergency medical system.

ANALYSIS OF ORGANIZATIONAL COMPONENTS OF EMERGENCY MEDICAL CARE FOR PATIENTS WITH ACUTE CORONARY SYNDROME IN POLTAVA REGION OF UKRAINE 乌克兰波尔塔瓦地区急性冠状动脉综合征患者急诊医疗护理组织组成分析

The aim of our research was to study the structure of patients with acute coronary syndrome and determine the organizational components of medical care for patients with suspected acute coronary syndrome in Poltava region.Material and methods: For our study was on the one hand statistical reports of health care institutions for 2016-2018, and on the other – a survey of doctors and paramedics of emergency medical care teams on tactics management of patients with AMI. The methodological apparatus used to achieve the set goals was based on the determination of relative values and analysis of time series.Results: The number of patients hospitalized with acute coronary syndrome is growing every year: from 82 people in 2012 to 489 in 2018. According to the monitoring conducted in Poltava region, emergency medical care for patients with acute coronary syndrome with ST segment elevation in 2018 in the cities was provided in the amount of 717, while in rural areas – 288.Conclusions: Data from the analysis of emergency medical care for patients with acute coronary syndrome showed that among patients with ST-segment elevation, urban residents predominate; in 90.6% of cases, the time to the patient’s hospitalization was less than 120 minutes.

ANALYSIS OF ACTIVITIES IN THE FIELD OF HEALTH PROMOTION, PROPHYLAXIS AND PREVENTION OF MENTAL DISORDERS AMONG CHILDREN AND ADOLESCENTS IN POLAND AND WORLDWIDE 对波兰和全世界儿童和青少年促进健康、预防和预防精神障碍领域的活动进行分析

Aim: The study aims to compare Polish methods of health promotion and prevention (prophylaxis) of mental disorders among children and adolescents with solutions have their effect in other countries when assessing the effectiveness (in both groups).Material and methods: Based on legal acts concerning mental health protection and data on preventive programs conducted both in Poland and in other countries, not only the methods of operation and the differences between them were compared but also recommendations were made regarding making possible changes to the current activities in Poland.Results: In territorial self-government units, where help is offered mainly to teenagers, introducing the intervention may be too late. Besides, due to the limited number of places offering such aid, incl. municipal programs, the issue emerges – concerning the ensuring of appropriate preventive measures – which results in the deterioration of the situation in this age group category and eventuate in the escalation of the current situation.Conclusions: To prevent the widespreadness of the disorders mentioned above, it is necessary to modify and extend the scope of preventive measures and activities regarding mental health promotion in Poland.

METHOD OF ERYTHROCYTE PROTECTION IN URGENT CARDIAC SURGERY 急诊心脏手术中红细胞保护的方法

Aim: The use of a simple, safe and convenient method of treatment extracorporeal circuit with «adaptation composition » (AdC) for the reduction of negative impact on the state of erythrocytes and tissue hypoxia. Material and methods: The research included 150 patients. They were divided into two groups. The first group included patients who underwent surgery with the treatment of an extracorporeal circuit by AdC, the second group was the control group. Сomplete blood count parameters, plasma free hemoglobin concentration, erythrocyte mechanical and osmotic resistance, erythrocyte membrane permeability and acid hemolysis were studied. Results: Use of AdC prevents adsorption the blood cells on the surface of the extracorporeal circuit and protects the erythrocytes during cardiopulmonary bypass in urgent cardiac surgery. Conclusions: The treatment of oxygenator with AdC reduces the negative influence СРВ on state of RBC. Membranes of erythrocytes were more resistant to traumatic factors in the group with AdC.

Various dimensions of trust in the health care system 医疗保健系统中信任的各个方面

Trust is one of the most important factors in building effective and long-lasting relationships in the entire healthcare sector. Trust becomes a valuable ally in situations of high risk and uncertainty as well as the increasing complexity of tasks that accompany the daily work of all medical professionals, especially paramedics, due to the nature of their work in the medical rescue system. Mutual trust is the basis of social capital thanks to which it is possible to achieve mutual benefits, easier to coordinate activities, create new quality and solutions through cooperation or strong ties. It is a kind of glue that binds various organizational and system links, thanks to which it is easier to plan and introduce necessary improvements and changes within individual medical units or the entire health care system. The high degree of trustincreases the quality of clinical communication with the patient and the effectiveness of medical care and strengthens the employees’ motivation and willingness to cooperate. Creating social capital based on trust is in the interest of all internal and external stakeholders of the entire health care system, thanks to which joint, cross-sectoral projects for health in the public dimension are possible.The aim of the article was, on the one hand, to systematize the theoretical and practical knowledge related to the multidimensionality of trust in the health care system, but on the other, to identify and analyse the factors building trust that should be discussed and, above all, applied in everyday professional practice and social space of all employees of the health care system. with particular emphasis on paramedics.

TRIAGE AND EMERGENCY MEDICAL MANAGEMENT OF THE ACUTE RADIATION SYNDROME 急性放射综合症的分诊及紧急医疗管理

Introduction: Medical personnel of the state Emergency Medical Services (EMS) will be the first element involvedin providing assistance to victims of radiation incidents. Triage, initial diagnosis and further patient medical managementwill depend on their actions. In many radiation incidents, initial radiation triage and early diagnostics will becarried out in the emergency department (ED) or trauma center (TC). Extended triage, diagnosis of ARS and propertreatment will be continued in specialist centers.The aim: To present a model of patient medical management including initial triage and early diagnosis of ARS thatcan be carried out at the ED and TC.Material and methods: The initial medical procedures prepared by expert groups were presented. The choice ofprocedures was made in terms of the possibility of their application by the EMS teams in Poland. Particular attentionwas paid to the method of initial triage, based on the clinical condition and laboratory diagnostics.Results: Based on the material collected, the path of the initial medical procedure was presented. Variants of theradiation triage, interpretation of clinical parameters and results of laboratory tests are presented. The methods ofinitial treatment and the method of qualification for specialist treatment, and new methods of treating patients werealso described.Conclusions: An adequate evaluation of radiation incidents, determining the absorbed dose of ionising radiation andARS, as well as initial triage seem to be crucial skills of the EMS workers.

Simple Emergency Triage (SET) – the new perspectiveon mass casualty incident triage 简单紧急分类(SET) -大规模伤亡事件分类的新视角

The aim: Incidents with large number of casualties present a major challenge for the emergency services. Incident witnessesare always the first on scene. Authors aim at giving them an algorithm arranging the widely known first aid rulesin such way, that the number of potential fatalities before the services’ arrival may be decreased.Material and methods: The authors’ main aim was creating an algorithm for mass casualty incident action, comprisingelements not exceeding first aid skill level. Proceedings have been systematized, which led to creation of mass casualtyincident algorithm. The analysis was based on the subject matter literature, legal acts and regulations, statisticaldata and author’s personal experience.Results: The analysis and synthesis of data from various sources allowed for the creation of Simple Emergency Triage(SET) algorithm. It has been proven – on theoretical level – that introducing an organized way of proceeding in masscasualty incident on the first aid level is justified.Conclusions: The SET algorithm presented in the article is of an implemental character. It may be a supplement to basicfirst aid skills. Algorithm may also be the starting point for further empirical research aimed at verifying its effectiveness.

KOS heart infarction - Where are we going? KOS心脏梗塞-我们要去哪里?

The introduction of a new patient care model after acute coronary syndrome (ACS) in Poland as part of the Comprehensive Patient Care Program after a Heart Attack (KOS) resulted in improved patient treatment quality and reduced mortality due to diseases of the cardiovascular system within 12 months after the onset of heart attack – these are the first results after two years of functioning of this model. The program has been operating since October 1, 2017, and was introduced to improve the care of patients after cardiovascular incidents and its main goal is to improve the quality of treatment and reduce patient mortality. The program has already been corrected due to limitations and problems reported by the cardiologists.
